[Anatomic problems involved in the approach to the internal carotid artery].

The portion of the internal carotid artery (ICA) located above the line joining the tip of the mastoid and the angle of the mandible is in close relation with cranial nerves VII, IX, X, XI and XII as well as the middle ear at the beginning of its intrapetrous course. Several procedures have been described for exposure of the ICA at the base of the skull. Extension of the conventional sternocleidomastoid approach by mandibular subluxation associated with division of the posterior belly of the digastric muscle and the styloid apophysis or its attached muscles allows exposure of most of the ICA. To access the last centimeter of the ICA it is necessary ot drill through the mastoid apophysis and the vaginal apophysis of the tympanic bone. This approach preserves the middle ear and the facial nerve.

[Complications of colonic diverticulosis and emergency surgery. Prospective study of 56 cases].

Fifty six patients, 26 men (69.7 +/- 11.2 years) and 30 women (77.4 +/- 9.2 years) were admitted in the emergency unit for diverticulosis sigmoiditis. In 75% of cases an other pathology was associated. Excepted 11 extended peritonisis, 7 criterae of operation were required to operate on 17 men and 18 women (mean age = 73.7 +/- 12.5 years) in a delay less than 8 days. Reasons were: 12 occlusive forms, 9 peri-sigmoidis abcedations, 3 hemorrhages. Surgical procedures were: 11 Hartmann procedures, 21 resections with anatomosis of the sigmoid colon with 10 non protected and 3 subtotal colectomy. Postoperative mortality was been 11.4%. The global rate of complications was 28.5 and the mean hospital stay was 18.8 +/- 6.3 days. Application of operative criteriae is usefull to shorter significantly the delay of operation and then the length of evolution of the intraperitoneal infection site.

[Treatment of splenic trauma. Apropos of a series of 52 cases].

Our serial is composed of 52 cases analysed after a backward upper two years. Because the happening of flashing infections sometimes many years after splenectomy, the therapic gesture must be prioritally leaded by splenic conservation, possible in all cases except the lesions of type 4 (spleen burst or pedicule extract): the perisplenic resorbable prothesis is a precious contribution. The analysis of results confirms the reliability of splenic conservation (47%) with 91% excellent results.

[Hepatic rupture. Wrapping with absorbable mesh].

Considerable progress in conservative surgery of the spleen and kidney has been achieved in recent years by wrapping the organ in absorbable mesh. Following an experimental study, this technique has been used in 5 cases of rupture of the liver. Each injured lobe can be wrapped in an absorbable prosthetic sheet which is sutured in such a way as to form a purse applied onto the tissue, to control bleeding and bile leakage. The purse is then sutured to the falciform ligament detached from the diaphragm. Under drainage and provided the liver is watched for cavitation by scanning methods, this technique is simple and consistently effective. It avoids sutures, which are imperfect, and resections, which are fraught with dangers when performed in an emergency.

[Acute fatal hepatorenal failure during treatment with mithramycin].

A case of fatal hepato-renal failure occurring during mithramycin treatment is reported. A 64 year-old female patient was admitted to hospital in a state of acute renal failure. She also presented with hypercalcaemia and bilateral pulmonary metastases. She had been operated on 10 years previously of a parathyroid cancer. Despite treatment with mithramycin (total dose 8.25 mg) and haemodialysis, the hypercalcaemia returned; it was then decided to remove the secretory lung metastases (parathormone 420 micrograms X ml-1). 48 hours before surgery, the patient was again given 1.25 mg mithramycin. Immediately after surgery, she developed hepatic failure with massive cell destruction and anuria. The patient died 48 h after the operation. The hepatic and renal complications of mithramycin are discussed.

[Systematization of the musculo-tendinous architecture of the human biceps (musculus biceps brachii)].

The structure of the brachial biceps is studied based upon the bilateral dissections of six subjects. The object of this work consists of determining the distribution of the group and muscular fibers in relation to the two proximal insertions and the two distal insertions. It is possible to describe the subgroups within each structure. The objective of each of these subgroups is considered within the framework of the coordination between the two muscular formation and the double insertions on the structure of the forearm. It is probable that this muscular formation corresponds to two different movements of the forearm during the locomotory progression. This work can be utilised in the sphere of the comparative biomechanical observation. Also electromyographic and the histochemical characteristics of the muscular group.

[Injuries of the colon and emergency surgery].

Tough less frequent that small bowel lesions, injuries and contusions of the colon rank second among the lesions sustained by hollow organs. They can be differentiated into contusions due to pressure in persons with multiple injuries, and penetrating wounds caused by blades or firearms. Although the latter involve multiple sites, they are most often clearly delineated and can be immediately and completely repaired, whereas contusions account for more severe parietal lesions, which sometimes require temporary bypass. The poor reliability of plain radiographs enables clinical investigations to recover an importance they seemed to have lost. Mortality has become very low in younger subjects, but it remains high after 60 years of age, and depends directly on the extent of peritoneal sepsis and on the associated deficiencies.

[Non-invasive dynamic studies of the cervico-thoraco-brachial pathway. Anatomo-clinical correlative study].

Based on a computer assisted analysis of the anatomic and biomechanic features of 200 subjects, the authors propose a dynamic approach to the exploration of the cervico-thoraco-brachial pathway. Results are given for 70 healthy and diseased subjects. Ultrasonic evaluations of the arterial component, dynamic echography of the axillo-subclavian venous component, electrophysiology including conventional electromyography, somesthesic and motor evoked potentials, and advanced imagery provide a panel of complementary diagnostic techniques.

[Kidney traumatism in general surgery (65 cases)].

From September 1989 to September 1994, 65 patients, 15 females and 50 males, with a mean age of 32 +/- 14 years were admitted in the emergency unit for renal trauma. In 38.5% of cases multiple trauma was present. Lesional mechanism was most frequently contusions (49.2%) and deceleration injuries (43.1%). Gross hematuria or microscopic hematuria were presents in 72.3% of cases. The sensibility of systematic abdominal echography was 100% in case of perirenal hematoma and 65% in case of parenchymatous lesions. Only 23 patients were operated (35.3%) because of low blood pressure. None secondary operation was necessary in abstention group. Global mortality was 7.7%. Postoperative morbidity was 20% and specific morbidity in abstention group was 10.7%.
