Influence of atropine, pirenzepine and cimetidine on nocturnal gastro-intestinal motility and gastric acid secretion.

The present study was performed to investigate the correlation between gastric H-ion-concentration and interdigestive motility (1) under basal conditions and (2) under the influence of pirenzepine, atropine and cimetidine. During the control period 6.5 (DZ1 5, DZ9 12) MMC's per 12-hour periods (69 +/- 7% gastrointestinal MMC's) were recorded. After administration of pirenzepine and the combination of pirenzepine and cimetidine almost exclusively gastrointestinal MMC's could be identified. On the other hand, atropine did not change the gastrointestinal/intestinal MMC ratio, but reduced total number of MMC's significancy (p less than 0.05). Percentage of observation time with antral pH greater than 4 was significantly increased by the combination of pirenzepine and cimetidine, but not by pirenzepine or cimetidine alone nor by atropine. There was a positive correlation between interdigestive cycle-length and percentage of time with antral pH greater than 4. The effect of pirenzepine on interdigestive motility (1) seems not to be mediated by the inhibition of gastric acid and (2) is different from the effect of atropine.

[Diagnosis of the irritable colon: manometric and myoelectric investigations (author's transl)].

Manometric investigations of patients with irritable colon show an undisturbed cyclic course of interdigestive motility studies of the rectosigmoid in patients with irritable colon show a spectrum of variable frequency of slow membrane potential changes with predominance of a frequency of 3/min. In addition, after a 1000 calorie test meal, patients with irritable colon showed a significantly prolonged increase of motility. As functional disturbances of irritable colon are mainly diagnosed by exclusion of organic changes, further development and rational use of colonic myoelectric methods promises future possibilities for objective diagnosis and control of treatment.

[Growth and metastasis of malignant tumors].

The high life expectancy of today seems to be an important factor for the increasing frequency of malignant disease, which is observed nowadays: the patient lives long enough to develop cancer. Increased knowledge about growth and spread of malignant tumors allows a better understanding of tumor pathogenesis and is a prerequisite for differentiated and effective therapy. Besides it is necessary to improve the early detection rate of cancer and to select and control high risk groups.

[Irritable colon. Possibilities for an objective diagnosis and its value for the therapy].

Diagnosis of functional gastrointestinal disorders consists mainly in exclusion of organic diseases. A more positive diagnosis may be established by the identification of characteristic symptoms. Myography and manometry within the colon sigmoideum reveal significant patterns in irritable bowel patients, which also may allow control of therapy.

Increased duodenal alkali load associated with the interdigestive myoelectric complex.

In six healthy volunteers the interdigestive motor activity and the secretory pattern were recorded in the duodenum by open tip catheters and intraluminal titration, respectively. The duodenal part of the interdigestive myoelectric complex (IDMEC) was regularly associated with a significantly increased alkali load to the duodenum amounting to 2.4 +/- 0.4 mmol per IDMEC. Thus, the intestinal "housekeeper" function of the IDMEC may be brought about by a coupled action of propagated motility and flushing secretion.

[Osteoporosis].

To judge extent and degree of the activity of osteoporosis x-ray examination, histological and biochemical investigations are necessary. They form the basis for a differentiated physical and medical therapy, which, dependent on the cause of the osteoporosis-illness, may influence the advance of the disease.

Functional lymphangiectasia of the duodenal mucosa.

Tiny millet-like prominent villi with dilated lymphatics may be part of a food-induced functional lymphangiectasia. Peroral olive oil load causes diffuse transient lymphangiectasia, the gross aspect, histological findings and electron-microscopy of which is described in detail.
