[Bilateral germ tumors of the testis. Report of 5 cases and review of the literature].

OBJECTIVE: To know the prevalence of the bilateral germ cell tumours of testis diagnosed in our Department and to review the literature. MATERIAL AND METHODS: 64 patients diagnosed of a germ cell tumour of the testis were followed during an average period of 51.4 months (1-168 months). RESULTS: 5 (7.8%) patients developed a second germ cell testicular tumour. In one patient the tumours were synchronous while in the remaining four were metachronous, occurring in an average interval of 59 months. One patient with a metachronous tumour died as consequence of the second tumour. In two of the five patients risk factors were identified, one presented testicular atrophy and the second referred history of undescended testis. DISCUSSION: The probability of developing a germinal testis tumour between the patients with history of a previous germ cell tumour of the testis is sensibly greater than between the general population. The prevalence of the bilateral tumours of the testis oscillates between 1-5% and approximately 75% will be metachronous. The principal factor that can predict the appearance of a second testicular tumour is the presence of the carcinoma in situ (Cis) in the contralateral testicular biopsy. Except in cases of testicular atrophy or previous history of undescended testis we do not recommend routine biopsy of the other testis.

[Self-administration of an oil solution in the penis].

We report the case of a patient who has autoinjected himself the penis and the scrotum with a solution constituted by petroleum, vaseline, oxygenated water and ketoconazol gel. Treatment consists in the excision of the injected deposit of lipid material and antibiotic therapy with good result at 4 months. We review the existing literature related with this exceptional pathology.

[Tumors of the upper urinary tract: epidemiology, clinical, and diagnosis].

OBJECTIVE: To review the epidemiological, clinical and diagnostic aspects of upper urinary tract tumors (UUTT). METHODS: The clinical records of 105 patients with UUTT were reviewed. There were 114 functioning units in total. Data on distribution according to sex, age at presentation, involved side, focality, localization, association with bladder tumor, risk factors, clinical features, radiological and histological findings were analyzed. Descriptive statistical analysis was performed and the means and frequency rates were estimated. RESULTS: Of the 105 patients, 88 (83.8%) were male and 17 (16.1%) were female, accounting for a male to female ratio of 5.1:1. The mean age was 68.3 +/- 10.5 years (range 24-88). The tumor involved the left side in 52 cases (49.5%), the right side in 49 cases (46.6%) and 4 cases (3.8%) had bilateral involvement; 84 (80%) were unifocal and 21 (20%) were multifocal. The pyelocaliceal region was compromised in 41.2% (47/114) of the functioning units, the lumbar ureter in 14% (16/114), the sacral ureter in 7% (8/114), the pelvic ureter in 28.9% (33/114) and the entire upper urinary tract in 8.7% (10/114). UUTT was associated with a bladder tumor in 60.9%. The bladder tumor and UUTT presented simultaneously in 26 cases (29.8%). The bladder tumor presented before the UUTT in 35 cases (40.2%) and in 26 cases (29.8%) it presented after. Smoking was found to be the most important risk factor. Hematuria was the most common reason for consultation (67.6%), followed by flank pain (23.8%), and 13.3% were asymptomatic. The most common urographic finding was a filling defect (46.4%), followed by loss of function (36.8%) and hydronephrosis (20.1%). Loss of renal function was observed in 66% of the cases with metastasis. Histologically, 99% were transitional cell carcinoma, basically moderately differentiated (68.8% grade II) and non infiltrating (58.6% pTa-pT1). CONCLUSIONS: Our findings are largely in agreement with the data published in the literature, although we have found a very high incidence of UUTT associated with bladder tumor in our series.

[Fibroepithelial polyps of the ureter: report of three new cases].

The fibroepithelial polyp of the ureter is a benign tumour with a mesenchymal origin. Since its incidence is low, there are very few cases published in the national literature. Presentation of our series, which consists of three cases seen and treated in our service over the last 10 years. Discussion of the most frequent characteristics seen in this type of disease and emphasis on its great relevance for the differential diagnosis with other causes of ureteral repletion deficiency.

[Cancer of the bladder and hemodialysis].

Of 324 patients on long-term dialysis treatment, 14 developed malignant tumors. Of these, 4 had bladder tumors. The increased frequency of malignant tumors (4.3%) is underscored. In our setting however, cancer of the bladder accounts for 28.5% of all tumors whereas in the literature a higher frequency has been reported for lymphoma, lung, thyroid or renal cell cancer. A review of the literature has revealed significant differences in frequency and tumor type reported in patients on long-term dialysis. A two-stage diagnostic protocol to detect bladder tumors in patients on dialysis is described. During diagnostic work up, we attempt not to be aggressive in these patients. Due to the high incidence of bladder tumors, we believe that at least the first stage of this diagnostic protocol should be performed in all patients that are symptomatic or require urologic work up prior to inclusion in a kidney transplant program. We report our experience in 14 cases diagnosed as having tumors in a series of 324 patients on dialysis.

[Prostatic invasion by lymphoma].

Presentation of a case of prostatic infiltration by malignant lymphoma, discovered through manifestation of simple urinary symptomatology. The paper describes the diagnostic process followed, and includes reproduction of the most interesting findings. The rarity of this disorder and the good response obtained from the alternative chemotherapeutic therapy is emphasized.

[Seminal infection and male sterility].

A study is made of 25 patients suffering from a vesicular prostate pathology causing sterility. We have assessed the age, profession, symptoms, genital exploration and ejaculation study including the amount, pH, visocity, fructose, ascorbic acid, count, motility and morphology of the sperms and the high leukocyte reading. A seminal liquid culture has been carried out on 21 patients, alone or in association with cultures of prostate and urine secretion following prostate massaging with positive results in 16 of them (76%). Treatment was begun on 22 patients, in 13 of whom the results are known. Spermiogram readings became normal in 5 (38%) with one pregnancy; there was an associated pathology in the wife in 3 cases. As antibacterians we have basically used trimetoprim-sulphametoxazol associated with erythromycin or not and sometimes, according to the antibogram, with other antibiotics, as well as drugs to clear the prostate area. The best results were obtained in astenozoospermias. The effectiveness of the treatment was equally due to the use of TMP-SMZ on its own or along with erythromycin. The other drugs have not improved the results, except for minocyclin which megativated a sperm culture. As a preliminary result the author presented three patients with sperm infection produced by E. coli at whom the titer of antibodies in serum was investigated.

[Seminal vesicular cysts associated with renal agenesis, ipsilateral ureter and hemitrigone. Report of a case].

A case is presented of seminal vesicular cyst associated with kidney agenesia, ipsilateral ureter and hemitrigon; this is the 17th case presented in the world literature reviewed. A study is made of all the cases published and the authors recommend deferento-vesiculography as the best means of diagnosis and total excision of the cyst as the most effective treatment.

[Parathyroid adenoma and renal lithiasis. Report of a case].

A case is presented of nephrolithiasis in a patient with no other symptoms than the urological ones and in which considerable hypercalcemia led to a study being carried out on his phospho-calcium metabolism and the diagnosis reached was primary hyperparathyroidism caused by a parathyroid adenoma. Surgical treatment was performed on the lithiasis and the adenoma as a result of which the symptoms completely disappeared and the biochemical readings returned to normal.

[Carcinoma of the penis: review of our cases].

A review is made of 36 carcinomas of the penis treated in the Urology Department of the "La Fe" Hospital. The most frequent decade for their appearance is the fifties. We establish their link with restricted hygiene and with fimosis, Queyrat's erythroplasia, leukoplasia and accuminated condyloma. The treatment given depends essentially on the clinical stage. We feel that within stage A, with minimum prepuce lesions, the ideal treatment is local excision and circumcision plus radiotherapy; in the case of minimal gland lesions radiotherapy; in the rest and when the former forms of therapy fail, partial amputation of the penis. We currently treat this stage with cryosurgery although we cannot give any results because of the limited period of development of the same. In stage B, we indicate amputation plus bilateral, ilio-inguinal lymphadenectomy plus radiotherapy. In stages C and D the treatment is palliative, on the basis of polychemotherapy.

[Metastatic tumors of the penis].

The authors present four cases of metastasic penis tumours, two of which followed a prostate carcinoma, one vesical carcinoma and the other a carcinoma of the excretory duct. They make a review of world literature on the subject and study the possible ways of dissemination as well as the clinical picture and differential diagnosis with other pathological processes of the male genital organ and they stress in particular the bad prognosis as the metastases appear at a very advanced stage of the primitive illness.

[Torsion of the hydatid of Morgagni].

The authors present 6 cases of torsion of Morgagni's hydatid treated in the Emergency Urology Department over a two-year period. They stress the importance of the clinical diagnosis, as well as the differential diagnosis with other "acute, testicular bag" processes, in particular with funicular torsion. Surgical treatment is the only way of overcoming the pain, confirming the diagnosis and preventing the risk of secondary, testicular atrophy.

[Treatment of inflammation processes of the lower urinary tract with dimethyl sulfoxide].

We present 8 patients treated with Dimethyl-sulphoxide, suffering from inflammatory processes of the lower, urinary tract which include: interstitial cystitis, urethral syndrome, incrustation of the prostatic cell, actinic cystitis and chronic prostatitis. All the patients have shown an improvement in their symptoms which, along with the absence of side effects and its easy administration, means that the said drug constitutes a means of therapy to be taken into account in the treatment of these inflammatory processes, following the consideration of more aggressive therapies.
