[Idiopathic soft palate hemiparalysis].

Idiopathic soft palate paralysis is an infrequent clinical entity, both in children and adults. We describe the clinical manifestations in two new cases, manifested by fluid dysphagia, rhinolalia, absent gag reflex, and nasal escape of fluids. The remission period is the shortest observed in published cases. Thirty-three reports have been published in the literature to data, 32 in children and adolescents and one in an adult. This paralysis suggests a viral etiology, affects mainly children, and resolves spontaneously.

[Carbon monoxide poisoning].

OBJECTIVE: To describe epidemiological and clinical data from a series of children hospitalized for acute carbon monoxide (CO) poisoning. PATIENTS AND METHODS: We reviewed the medical records of children admitted to the Pediatrics Department of the Hospital Clinico Universitario de Zaragoza with acute CO poisoning from 1993 to 2003. All the children were clinically monitored and venous blood levels of carboxyhemoglobin (COHb) were determined. RESULTS: There were 14 patients (seven boys and seven girls) aged 1.3 to 13.8 years (mean age: 8.2 years; SD: 3.9). The peak incidence was in the winter months (seven cases in December). All poisonings took place in poorly ventilated rooms and were produced by gas inhalation due to incomplete combustion of organic fuels (charcoal, propane and butane). Clinical symptoms were gastrointestinal (nausea, vomiting, abdominal pain) and neurologic (dizziness, headache and alterations in level of consciousness). COHb levels in blood were elevated (from 4.8% to 27.6% in the first determination). Outcome was favorable in all patients. CONCLUSIONS: Despite environmental improvements introduced in the last few years, CO poisoning is highly frequent in our environment and children are at higher risk. The best preventive measure is proper adjustment of gas appliances.

[Asperger syndrome associated with macrosomia and sexual identity disorder].

Asperger Syndrome is a pervasive developmental disorder of unknown origin, characterized by pedantic language, lack of reciprocity in social interactions, unusual interests, motor clumsiness and normal or above average intelligence quotient, among other symptoms. Since 1994 it has been defined as a specific entity. We describe the case of a boy with this syndrome, with elevated body weight and height and sexual identity disorder. These alterations have not previously been described in the scientific literature on Asperger syndrome.

[Cardiac abnormalities in children with neuromuscular diseases].

BACKGROUND: To evaluate cardiac abnormalities in children with different neuromuscular diseases (NMD) PATIENTS AND METHOD: In 40 patients with NMD, cardiac abnormalities were evaluated by cardiac auscultation, chest X-ray, electrocardiography (ECG), ambulatory Holter ECG and Doppler echography. RESULTS: Sinus tachycardia, high R/S complex, deep Q waves, repolarization disorders, electrical conduction abnormalities, auricular and ventricular extrasystoles, Wolff-Parkinson-White syndrome, mitral valve prolapse and dilated cardiomyopathy were found. CONCLUSIONS: The association of cardiac abnormalities in children with NMD is frequent. Conventional ECG monitorization, Holter ECG and echocardiography allow their diagnosis and follow-up.

[Secular trends of body adipose tissue in prepubertal children].

OBJECTIVE: To determine whether there is a significant difference between anthropometric measurements of prepubertal children and the measurements of another sample with similar characteristics, studied fifteen years ago. MATERIAL AND METHODS: We studied 859 children, 453 males and 406 females, from the age of 5.0 to 10.0 years. Weight, height, arm circumference and skinfold thicknesses in the biceps, triceps, subscapular and suprailiac were determined. Body mass index, body density, total fat and percentage fat were calculated and their values compared with those obtained 15 years ago in 1,617 children with similar characteristics. Difference between means were calculated with Student's single-sample t-test. RESULTS: Weight increased in all the ages, with significant differences in males at 7, 8 and 9 years (p<0.001) and in females after the age of 6 years (p< 0.05). Height significantly increased at all ages and in both sexes, except in females at the age of 5 years. Body mass index increased only at the ages of 8 and 9 years in males (p<0.01). Skinfold thicknesses significantly increased in both sexes and at all ages (p<0.05). Body density decreased (p<0.01) and body fat increased, both in kilograms and in percentage (p<0.01). CONCLUSIONS: During the 15 years studied, height and weight increased significantly. Body mass index did not increase significantly, whereas body fat (skinfold thicknesses and fat compartment) significantly increased.

[Secular increase of adipose tissue in adolescents from Zaragoza from 1980 to 1995].

BACKGROUND: To quantify the differences between anthropometric measurements obtained nowadays in male and female adolescents and those obtained 15 years ago in a sample of similar characteristics. SUBJECTS AND METHODS: We have studied 658 healthy individuals, 329 males and 329 females from 10.0 to 15.0 years of age, from different socioeconomic levels. Weight, height, arm circumference and left skinfold thickness (biceps, triceps, subscapular and suprailiac) have been measured. We have calculated the body mass index (BMI), density, total body fat, percentage of body fat and the body adipose muscular index (BAMI), which is the ratio between body fat (kg) and non fat mass (kg). The values obtained have been compared with those obtained fifteen years ago, in another sample of 1,465 children with similar characteristics. The research team and the material used were the same in both studies. Mean differences were compared using the unpaired t-test. RESULTS: Weight has increased significantly (p < 0.05), except for males at 12 years and females at 13 and at 14 years. Height has increased significantly at all ages and in males and females (p < 0.05). BMI has only increased significantly at 10 and at 11 years in males, and at 10 years in females. Skinfold thicknesses have also increased significantly, except for biceps in males between ages of 12 and 14 years and biceps and subscapular in females at 14 years. Body fat mass and percentage of body fat have also increased significantly and, in consequence, the density has decreased and BAMI has increased both significantly (p < 0.05). CONCLUSIONS: Secular increase in weight and height during fifteen years has occurred. In general, BMI has not increased but skinfold thicknesses and body fat have increased significantly, this fact demonstrates that there has been increase in the body fat compartment.

[Anthropometric and nutritional study of full-term newborns. Evaluation of the submandibular adipose skinfold].

OBJECTIVE: We performed an anthropometric study in a series of normal full-term newborns to evaluate the discriminatory capabilities of the submandibular skinfold compared to other anthropometric parameters. MATERIAL AND METHODS: We studied 794 consecutive Caucasian newborns, 377 males and 471 females, with a mean gestational age of 39.5 +/- 0.9 weeks and adequate weights for age. Anthropometric variables analyzed included weight, length, left arm circumference and bicipital, tricipital, subscapular, suprailiac and submandibular skinfolds. Statistical analysis was performed by using the Student's t-test to evaluate differences between the different parameters and by Pearson's test to look for correlations among them. RESULTS: The weight and lengths of male newborns were significantly higher than those of female newborns (p < 0.005) These parameters showed differences with other series of newborns studied. Measurements of the tricipital, subscapular and suprailiac skinfolds were significantly different between males and females (p < 0.005); however, bicipital and submandibular skinfolds were not. The submandibular skinfold measurement highly correlated with the other skinfold measurements, especially with the bicipital measurement, in both males (0.536; p < 0.0001) and females (0.578; p < 0.001). Submandibular skinfold measurements also correlated with weight and arm circumference in both sexes. CONCLUSIONS: Some somatometric measurements in normal term newborns differ significantly between the sexes, especially weight, length and tricipital, subscapular and suprailiac skinfolds. The submandibular skinfold correlates well with weight, arm circumference and the four routinely measured skinfolds. This finding suggests that measurement of the submandibular skinfold should be included in the evaluation of the newborn's nutritional status.

[Calculation of body fat by skinfold thickness].

This paper describes the method of determining the sizes of the subcutaneous fat folds found in the biceps, triceps, subscapular, suprailiac and submandibular. One is shown how to make the calculation of body fat and the areas of the section of the arm as related to the sizes of the folds and, finally, the author provides some recommendations so that one may correctly evaluate the data obtained.

[Brachial measurements as nutritional indicators in health term newborns].

OBJECTIVE: Our objective was to study the correlations between weight and length and weight index and brachial areas in healthy term newborns as indicators of nutritional status. PATIENTS AND METHODS: We studied 794 consecutive Caucasian newborns, 377 male and 417 females, with gestational ages of 39 +/- 09 weeks and adequate weights. We measured weight, length, weight/length index, body mass index and weight index. Measurements of the arm perimeter and tricipital skinfold were used to calculate the fatty, lean and total areas by two different methods. Mean, standard deviation and percentiles were utilized in the measurements. The Student's t-test was used to calculate differences and Pearson's test for correlations. RESULTS: We found that weight, length and weight/length indexes were higher in males than in females, except for the weight index. Brachial areas and parameter were higher in females, except for the lean area. We did not find significant differences between gender in brachial measurements calculated by the different methods. Among the males, we found a high degree of correlation between weight and the weight/length index (r = 0.963; p < 0.0001), and weight and total body area (r = 0.649; p < 0.0001). Similar data were obtained among females, where in addition a high correlation was found between the fat percentage and the body mass index (BMI, r = 0.223; p < 0.0001), as well as between brachial areas and BMI in both genders (p < 0.0001). CONCLUSIONS: In normal newborns, the brachial areas, taken as indicators of body composition, are statistically significantly correlated with the anthropometric parameters widely used to measure the nutritional status of newborns.

[Body fat mass in boys and male adolescents].

OBJECTIVE: To describe the proportion of body fat mass, quantified from the measurement of 4 skinfold thickness, in a sample of male children of Zaragoza. METHODS: In 1995, we have studied 701 males aged 6.0 to 14.9 years, from 6 schools of the province of Zaragoza. We have measured: weight, height, and biceps, triceps, subscapular and suprailiac skinfold thicknesses. Body density has been calculated from the 4 skinfold measurements, with the formulas of Sarría et al. Fat mass (%), was calculated with the Weststrate and Deurenberg's formulas. In each age group, we present mean, standard deviation and 5, 25, 50, 75 and 95th percentiles. RESULTS: We present mean, standard deviation and 5, 25, 50, 75 and 95th percentiles of body fat mass (%), in each age group. Body fat mass (%) progressively increased until 10.5 years and, after, progressively decrease at least until 14.5 years. CONCLUSIONS: We present data of body fat mass (%), calculated from the measurement of 4 skinfold thicknesses, obtained from a reference population in the province of Zaragoza. These data are useful as more rationale criteria in the diagnosis of obesity in children and adolescents.
