[Longitudinal study of bronchial hyperreactivity using the carbachol test with school children].

STUDY DESIGN: Bronchial hyperresponsiveness of 476 schoolchildren (10.8 +/- 2.3 years) was studied three times at 12 months' intervals. The cumulative dose of 400 micrograms carbachol was applied in 50 + 50 + 100 + 200 micrograms steps. A fall of FEV1 of at least 15% was regarded as positive reaction. The test was save, as no severe obstruction was observed, only three children withdrew because of unpleasant cough. RESULTS: Reactivity was observed in 19.1, 10.0, and 5.2% of children at the occasion of the first, second and third test (sensitivity/specificity for prevalence of physician diagnosed asthma: 70/83, 35/91, and 24/96%, respectively). Reactivity was age dependent (7-11 years: 35%, 12-16 years: 18%), not influenced by sex, and the relative risk to be reactive was 1.9 in children 2-3 weeks after a respiratory tract infection. CONCLUSION: For epidemiological purposes carbachol provocation test--like other unspecific bronchial provocation tests--is inappropriate as a single test to classify individuals as asthmatics.

Characteristics of cystic neoplasms of the pancreas and results of aggressive surgical treatment.

Twenty-one patients with pancreatic cystic neoplasms (PCNs) were treated from 1970 to 1991. Their mean age was 54 years (range: 30 to 78 years), and 15 (71%) were women. Symptoms were present for a mean of 18 months (range: 5 to 60 months) and included pain (95%), abdominal mass (52%), weight loss (38%), and jaundice (14%). Nine patients had had previous operations and were either misdiagnosed or incorrectly treated; another seven patients had preoperative misdiagnoses of pseudocysts. There were six (29%) serous cystadenomas and two (10%) mucinous cystadenomas. These were treated by excision (n = 2), distal pancreatectomy (n = 5), or pancreatoduodenectomy (n = 1). No recurrence or malignant degeneration occurred during the mean follow-up of 9 years (range: 1 to 19 years). There were 13 (62%) patients with mucinous cystadenocarcinomas. Of these 13 patients, 3 had unresectable tumors, underwent palliative procedures, and died at 4, 7, and 9 months, respectively. Ten patients underwent pancreatoduodenectomy (n = 4), distal (n = 4) pancreatectomy, or total (n = 2) pancreatectomy: 1 died of recurrence (survival: 8 months), and the remaining 9 patients had a mean survival of 6 years (range: 2 to 20 years) without recurrence. This experience suggests that patients with PCNs have a good prognosis and are curable if the cysts are diagnosed early and completely resected.

Early childhood risk factors for sensitization at school age.

Early childhood risk factors for current sensitization were investigated by use of cross-sectional data of a longitudinal study in Southwest Germany. Information was gathered by questionnaires from 1812 families of whom 1470 children 6 to 8 years old were tested by means of a skin prick test (SPT) with seven aeroallergens. Groups with sensitization (n = 201; positive SPT to grass pollens 6.6%, Dermatophagoides pteronyssinus 6.5%, Dermatophagoides farinae 4.4%, cat dander 4.6%, any of the tested allergens 13.7%) are compared with children without sensitization (n = 1269). As risk factors for any sensitization parental atopy (odds ratio [OR]/95% confidence interval [95%CI]: unilateral 1.9/1.3 to 2.6; bilateral 2.8/1.5 to 5.2), low gestational age (1.9/1.1 to 3.2), and male gender (1.6/1.2 to 2.3) are statistically significant in multiple logistic regression. Former cat ownership is significantly related to sensitization to cat dander (2.7/1.4 to 5.5). Breast feeding, maternal smoking habits after the child's birth, prior exposure to pets, and social class are not important. In conclusion, our data suggest parental atopy, low gestational age, and male gender as independent risk factors for sensitization to aeroallergens at school age.

Longitudinal variability of skin prick test results.

The skin prick test (SPT) is a commonly used procedure for assessing a specific sensitization. The longitudinal variability of test results is of interest for clinical as well as epidemiological investigations. The sensitization to four common aeroallergens (grass pollen, birch pollen, Dermatophagoides pteronyssinus, cat dander) is investigated within the framework of three consecutive SPTs at 11-month intervals for a population of 587 schoolchildren. The prevalence of sensitization based on a weal diameter of at least 2 mm was between 12.9% (cat dander) and 23.9% (grass pollen) in the initial testing. The positive predictive values of the initial SPT were between 75.3% (birch pollen) and 88.2% (cat dander) for the two subsequent SPTs. In the case of initially negative tests with positive second and third SPTs the incidence ranged between 3.2% (cat dander) and 4.3% (birch pollen) per year. A clear increase in the intensity of reaction in subsequent tests was observed in a number of probands testing positively in the initial SPT. In conclusion, our data indicate a high long-term stability of a specific sensitization to aeroallergens in SPT.

[The special problem of insect venom allergy in childhood].

Natural history of hymenoptera sting allergy is less life threatening in children than in adults. We evaluated a commonly used risk score (allergic reaction to the index sitting, sensitization measured by skin prick test and specific IgE, and specific IgG - maximal 8 points) for its predictability of forthcoming allergic reactions. 93 children with low scores (less than 7 points) experienced stings in field, 96% were save from severe systemic reactions. Of 119 children with high scores (greater than 6 points) receiving rush hyposensitization only 32% developed systemic reactions. We conclude that the score appropriately identifies individuals not to be hyposensitized, but that it overestimates the number of children with a need for hyposensitization. We therefore developed a diagnostic scheme including challenge stings to identify those children who really need hyposensitization.

Nosocomial outbreak of neonatal gastroenteritis caused by a new serotype 4, subtype 4B human rotavirus.

A nosocomial outbreak of rotavirus gastroenteritis involving 52 newborns occurred between June and September 1988 at the University Children's Hospital of Freiburg, Federal Republic of Germany. Stools from 27 representative patients were examined for rotavirus serotypes, using a monoclonal antibody-based enzyme-linked immunosorbent assay. The electropherotype was also examined by polyacrylamide gel electrophoresis of genomic RNA. As many as 18 patients were found to be infected by serotype 4, subtype 4B strain, and in all of them the same electropherotype was detected. Although rotavirus from the remaining nine patients could not be typed, the electropherotype in four was identical to that of the serotype 4, subtype 4B strain. Thus, most of the patients in the outbreak were infected by the same rotavirus strain. Retrospective epidemiological studies showed that the 4B strain began to circulate at the hospital in January 1988, whereas only rotavirus serotypes 1, 3, and 4A were detected in 1985-1987. The primary case of the outbreak was presumably a newborn with acute gastroenteritis, admitted to the hospital from a small maternity unit in the same urban area. During the outbreak, 12 of 44 healthy newborns in the nurseries of the Children's Hospital and other maternity hospitals were found to be asymptomatic rotavirus carriers, and in three of the newborns the same 4B strain was detected. This is the first reported outbreak caused by a serotype 4, subtype 4B strain.

Ionized calcium, parathormone, and mortality in critically ill surgical patients.

A prospective study measured ionized calcium and parathormone sequentially at 48- to 72-hour intervals in 25 surgical intensive care unit patients. Twelve patients (48%) died at mean day 40 and median day 26. Levels of ionized calcium, parathormone, blood urea nitrogen, creatinine, albumin, magnesium, and phosphate for patients who lived were compared with levels for patients who died. The incidence of hypotension, renal failure (creatinine greater than or equal to 3.0), and bacteremia, as well as the amount of red cell, crystalloid, and colloid administration for the two groups was compared. Hypotension, bacteremia, red cells, crystalloid, and colloid were no different. On days 1 and 2 ionized calcium levels were significantly lower and parathormone levels significantly higher in nonsurviving patients; this difference persisted through days 3 and 4. Blood urea nitrogen and creatinine levels increased early in nonsurviving patients but renal failure, which occurred in nine nonsurviving patients, did not develop until mean day 14, median day 18. The phosphate level was slightly higher but still within normal range in nonsurviving patients. By days 5 and 6 ionized calcium and parathormone levels were no different in nonsurviving patients, despite there being no improvement in renal function. Magnesium and albumin levels were no different between groups. Ionized calcium levels are lower and parathormone levels higher early in nonsurviving patients. This difference is not readily explained by associated clinical conditions, including renal dysfunction. Although etiology remains unclear, low ionized calcium and elevated parathormone are early predictors of mortality in critically ill surgical patients.

[Colloid cysts of the 3d ventricle: problems of early intravital diagnosis].

Basing on two own cases with acute signs of increased obstructive intracranial pressure the problem of early diagnosis of colloid cysts of the third ventricle is discussed with special reference to case reports in the literature. In the absence of typical clinical signs and symptoms, there are acute and chronic constellations of signs and symptoms that point to a suspicion of an obstruction of liquor flow by a colloid cyst. It is explained which importance CT and MR assume in the diagnosis of these benign tumours that present a very varied neurological pattern. Special reference is made to the presentation of neuroradiological differential diagnoses and the application of specific examination methods, such as tomography in very thin layers and allowing some time to elapse after contrast medium administration has been effected.

Glucose uptake and flux through phosphofructokinase in wounded rat skeletal muscle.

Skeletal muscle injured with lambda-carrageenan has increased aerobic glycolysis. To assess the regulation of this process, the tissue concentrations of glycolytic intermediates, the flux through phosphofructokinase (PFK), and the intracellular concentrations of PFK effectors were examined in wounded rat skeletal muscle and in macrophages, the predominant inflammatory cell in the early stages of this wound model. Autoradiography demonstrated increased 2-deoxy-D-glucose uptake in wounded tissue compared with nonwounded muscle. 2-Deoxy-D-glucose was localized to the cellular infiltrate. The glycolytic intermediate concentrations demonstrated a facilitation of PFK in macrophages and wounded tissue as compared with nonwounded muscle. Wounded tissue had twice the flux through PFK compared with nonwounded muscle (10.0 +/- 0.6 wounded vs. 4.9 +/- 0.4 mumol.h-1.g-1 nonwounded). Macrophages had the highest flux through PFK (63.7 +/- 5.7 mumol.h-1.g-1) and when coincubated with muscle, the combined flux through PFK was equal to that of wounded muscle. The increase in glycolysis associated with wounded tissue may be explained by increased glucose uptake and increased flux through PFK by the inflammatory cells present in wounded tissue.
