Near total laryngectomy with cricohyoepiglottopexy (CHEP) allows cure of glottic carcinomas with voice preservation. The subject of this study was to evaluate CHEP in terms of tumour control and functional result in T1 and T2 glottic carcinomas. This study reviewed retrospectively 55 consecutive cases of CHEP performed between January 1, 1981 and September 1, 1992 with the exclusion of post-radiotherapy salvage surgery. CHEP was indicated for a T1a limit to the anterior commissure and/or with dysplasia of the other vocal fold (10 cases), T1b (11 cases) and T2 (34 cases) glottic carcinomas. All our patients have a follow-up of more than five years. The post-operative course after this surgery was generally uneventful. The median time to decannulation was 18 days, to removal of the nasogastric tube was 15 days and to discharge from hospital was 23 days. No significant difference was observed according to the preservation of one or both arytenoid cartilages. The long-term functional result can be considered to be good in three-quarters of cases, with normal oral swallowing and an easily understood voice. The remaining one quarter had a whispery voice and sometimes episodes of aspiration when swallowing liquids. In terms of oncological results, the five-year recurrence-free survival rate was 94 per cent for T1 and 84 per cent for T2. The ultimate tumour control (taking into account four cases of total laryngectomy) was 94 per cent for T1 and 93 per cent for T2. Primary surgery by CHEP therefore appears to be a good option for early glottic carcinomas. The main problem remains that voice recovery is mediocre in one quarter of patients.
We report case of a patient with hypopharyngeal biphasic sarcomatoid carcinoma, with two tumor cell component, epidermoid and spindle cells, treated by surgery only. Two years later, recurrence is noted with totally different histologic form, sarcomatous monophasic, with spindle tumor cells and smooth muscle phenotype with immunohistochemistry, looking like primitive leiomyosarcoma of high malignancy degree. Such monophasic sarcomatous forms of sarcomatoid carcinoma, without epithelial tumor component, are deceptive and can be mistaken with primitive sarcoma. That is why discovery of epithelial differenciation signs, with immunohistochemistry or electronic microscopy, is very important. In our case, while the epithelial differenciation in the recurrent tumor is away, previous history of true biphasic sarcomatoid carcinoma in the same anatomic location, allows to assert recurrence of the same tumor with monophasic sarcomatous expression and smooth muscle phenotype in this case. Therefore in front of "sarcoma of the upper aerodigestive tract", of any immunohistochemical phenotype, monophasic sarcomatous form of epidermoid sarcomatoid carcinoma have always to be evocated and searched.
56 patients with Bell's palsy were tested with electroneuronography (ENoG and Blink reflex (BR) within the 5th and 15th day of onset of facial paralysis. The recovery at the end of evolution was evaluated with the study of facial function. This retrospective study showed that ENoG is able to predict with a very low risk of error a good prognosis when more than 10 percent (strictly, of fibers (non degenerated and blocked) are active. On the other hand only persistence or early recovery of BR can assure a good prognosis whereas the absence do not permit to conclude.
We report 3 cases of head and neck malignant fibrous histiocytoma (MFH). The first case is a xanthomatous subtype of the inferior lip with lymph node metastasis. The two others cases are storiform-pleomorphic MFM of the hypopharynx and Killian area. A summary of actual knowledges about this entity is reviewed.
The authors report 100 ethmoidectomies performed on 50 patients between 01/87 and 09/89. 75% have been treated anteriorly by systemic corticosteroids. 65% have had prior sinus surgery. Follow-up time ranged from 12 to 34 months (mean: 18 months). A regular treatment with intranasal steroid sprays was taken post-operatively by 96% of patients until three months and 48% until 18 months. 72% of patients considered the symptoms relief as satisfactorily (score of satisfaction greater than 8/10). Severe nasal obstruction, noted in 88% cases pre-operatively, reappeared in 26% 18 months post-operatively. Anosmia, encountered in 72% pre-operatively, disappeared post-operatively in one half of the cases with a good conservation of olfaction 18 months later. Anterior and/or posterior rhinorrhea, sneezing were also satisfactorily controlled. Results on facial pain were not significant. Endoscopic evaluation was performed by a physician who was not the surgeon. A normoplastic mucosa covered the ethmoid cavities in 40%. Localized oedema was observed in 38%. An asymptomatic recurrence, discovered on endoscopic examination, was present in 11%. The symptomatic recurrence rate was 11%. A CTscan control, performed the day of the endoscopic examination and analysed few weeks later, reinforced the degree of objectivity. A complete marsupialization of the ethmoid labyrinth without any remaining cell was confirmed in only 30%. Frequence and aspects of the opacities in the great sinuses were the same pre- and post-operatively. So, this study shows that the insufficiencies of the ethmoidectomy are 1) the difficulty to perform a complete marsupialization of the ethmoid 2) persistent opacities in the great sinuses 3) persistence of the mucosa disease after surgery.(ABSTRACT TRUNCATED AT 250 WORDS)
The authors report one case of tonsillar cyst with area of dysplasia in the epithelium, in situ carcinoma, and invasive carcinoma which are peculiar to branchiogenic carcinoma. A tonsillarectomy made ten month after the operation contradict this diagnosis. The criteria for making the diagnosis of branchiogenic carcinoma are discussed so as the clinical sensibility of this cysts. Present imagery is not able to assert the primarity of such lesion. Removing of the tonsil would be made systematically.

We report a retrospective study of 161 cases of vestibular or epilaryngeal cancer treated by horizontal laryngectomy. The different factors affecting prognosis were analyzed statistically. Three groups were distinguished according to the size and primary site of the tumor: (1) a group with a very poor prognosis, which included patients with large tumors of the epilarynx; (2) a group with an average prognosis, which included small tumors of the epilarynx and large tumors with their primary site in the vestibule; (3) a group with a favorable prognosis which included tumors classified as T1 or T2 that were initially located in the vestibule.
Midline granuloma includes diverse clinicopathological entities, such as Wegener granulomatosis, polymorphic reticulosis, lethal midline granuloma and conventional malignant lymphoma of the nose usually of B-cell origin. The authors describe five patients with LMG clinically and pathologically typical. Using an extensive panel of monoclonal antibodies, they demonstrate an "activate" T-cell phenotype observed on the initial lesion of the face in one patient, similar to that found in two patients with LMG but studied after dissemination in peripheral T-cell lymphoma. Furthermore, many atypical cells were found in LMG, and stained with the Ki-67 monoclonal antibody, a marker of proliferating cells. These findings support the view that LMG is closely related to T-cell malignancies. Two of them were treated with recombinant Interferon alpha 2a followed by a response rapidly objective. Immunohistologic studies are very important for confirming the T-cell origin of such a disease and for selecting patients to be treated with Interferon alpha.

A retrospective study was carried out in 58 patients operated between 1978 and 1983 for cancer of larynx of glottic origin and mainly T1 and T2. Functional results were always of good quality, and the recurrence rate significantly lowered by the use of the Majer-Piquet or C.H.E.P. procedure when compared with results after thyroidectomy. A simplified technique is proposed.
Carcinoid tumors of the larynx are very rare: only 11 cases have been described in the available literature. We report another case, and discuss the nature of the cells of origin and the relationship between these tumors and another rare laryngeal tumor, the oat cell carcinoma.
Four cases of a median craniofacial malformation, schizoprosopia, are reported. This rare but compatible with life anomaly is included within the group of neurocristopathies of the nasofrontal bud. Patients affected present signs of more or less severe mediofacial division with, in some cases an associated encephalocele or cranial bone defect. The possible presence of an associated hypopituitarism must be investigated routinely, and substitutive hormone therapy combined with surgical treatment of the various malformations when results are positive.
A total of 74 cases of transmandibular buccopharyngectomy with reconstruction by local or regional flaps were analyzed to compare results of the use of temporofrontal and pectoralis major musculo-cutaneous flaps. Findings in this series suggested superiority for the use of the latter type.
Mucoepidermoid and acini cel carcinoma of the salivary glands are relatively rare and have a tendency to recur locally and sometimes metastasize to lymph nodes and at a distance. Such an evolution is difficult to predict even if one uses anatomo-pathological criteria, which explains the uncertain prognostic of these neoplasms and the difficulty encountered in establishing their therapy. Seven cases of mucoepidermoid carcinoma and five cases of acinic cell carcinoma are reported with statistical, clinical, and anatomo-pathological data. The evolution of these tumors are studied according to the therapy used, which leads us to establish a therapeutic protocol.
