The influence of a number of risk factors on development of tumour recurrence was studied retrospectively in a group of breast cancer patients without axillary lymph node metastases, all treated identically and with long follow-ups. Of 71 patients all data could be retrieved. This group had a median duration of follow-up of five years. Thirteen patients (18.3%) had had a recurrence of carcinoma after a median disease-free period of 41 months (range 3-124 months). These patients at the time when breast cancer was diagnosed had had a lower median age than patients who had remained free of tumour recurrence, 48 (40-70) as against 59.5 (30-81) years. Factors having a statistically significant prognostic importance for recurrence of tumour were age at the time of diagnosis of breast carcinoma, histological tumour grade and nuclear DNA content. Identifying groups of patients with a higher or lower risk of tumour recurrence appears possible by combination of risk factors. Although patients with breast cancer without axillary lymph node metastases are supposed to have a favourable prognosis, there appear to exist subgroups with a raised risk for development of a recurrence. These groups might be prime candidates for prospective randomized studies of the usefulness of adjuvant therapy.
The cellular expression of K-type pyruvate kinase was studied immunohistochemically in several normal and neoplastic tissues of human origin. The authors used the monoclonal antibody, designated as ES1, which was raised against human K-type pyruvate kinase. In contrast to the normal counterparts, a strong immunoreactivity was found in a rhabdomyosarcoma (n = 1), in a carcinoma of the pancreas (n = 1), and in neurofibromas (n = 2). Furthermore, the staining in leiomyosarcomas (n = 2) was shown to be more intense when compared with both normal smooth muscle cells and leiomyomas (n = 2). These findings show that knowledge about the cellular expression of the K-type pyruvate kinase identifies cell types for which its expression serves as oncodevelopmental marker. In addition, these immunohistochemical studies give information whether shifts toward K-type containing isozymes of pyruvate kinase, which are determined by electrophoresis in whole cytosolic extracts of various tumors, are due to an altered gene expression or due to proliferation of cells which normally express already the K-type pyruvate kinase. The first possibility probably occurs in rhabdomyosarcomas. The latter possibility seems to be valid for astrocytomas because astrocytes express the K-type pyruvate kinase in normal brain.

Differential diagnosis between acinic cell carcinoma and renal cell carcinoma is an oft-quoted problem. A case is presented of a 60-year-old woman with metastatic lesions from a renal cell carcinoma to the parotid as well as the submandibular gland. Appropriate diagnosis was delayed due to lack of clinical information.
