[Epidemiology of Menière's disease in Cantabria].

OBJECTIVES: To know the main epidemiologic characteristics of Meniere's disease in Cantabria. METHODS: All the patients diagnosed of "definitive" Meniere's disease between 1992 and 2002 in Sierrallana Hospital of Torrelavega (Cantabria, Spain) were reviewed. Incidence, prevalence, presence of bilateral disease, age at diagnosis and gender were recorded. RESULTS: A total of 75 patients were diagnosed of "definitive" Meniere's" disease" during this time. The incidence was 3/100,000 people/year, the prevalence 75/100,000, male/female ratio was 0.38. Most frequent age at onset was between 40 and 60 years and bilateral cases were 5.3%. CONCLUSIONS: Meniere's Disease is not uncommon in Cantabria. It is more prevalent among middle aged women. Bilateral cases are rare.

[Major ambulatory surgery in otorhinolaryngology].

In the last years we have witnessed a great increase of major ambulatory surgery procedures in our specialty. Some of them are very common as adenoidectomies, tonsillectomies and also the insertion of transtympanic tubes. Nevertheless there is no agreement about which of the procedures should be realized through AMS. The aim of this work is to do a review on the most important features involved in AMS entlightening what kind of operations can be included i such programmes.

[Hypoglossal nerve palsy and infectious mononucleosis].

Neurologic manifestations are infrequent in infectious mononucleosis and specially hypoglossal nerve involvement is very rare. To the best of our knowledge only 6 cases have been previously reported. We describe herein the case of a 20-years-old man who presented an unilateral hypoglossal nerve palsy secondary to infectious mononucleosis. He was treated i.v. steroids without recovery, but the palsy resolved spontaneously 2 months after the onset.

[Head and neck abscesses].

OBJECTIVE: To review the clinical, microbiological and therapeutic characteristics of all head and neck abscess seen in a Community Hospital. MATERIALS AND METHODS: Medical records of all patients admitted to Sierrallana Hospital (Torrelavega, Cantabria) from January 1995 till June 1998, because of head and neck abscess were reviewed. RESULTS: Fyfty-seven head and neck abscess of 54 patients were considered. Age of patients ranged from 14 to 78-year-old. Periamygdaline region was the most frequent location, where as tonsillar and dental infections were the origins of the abscess in the large majority of cases. Most of the abscess cultures yield mixed flora. Intravenous penicillin was used in 77.3 percent of patients and surgical drainage was required in almost 80 percent of cases. Treatment response was good with no serious complications in any case. CONCLUSIONS: Head and neck abscess are a relative common cause of hospital admissions. Among them periamygdaline abscess are frequently found. The treatment is surgical drainage with local or general anesthesia (depending of location) and simultaneous i.v. course of antibiotic (Penicillin G sodium salt) efficacious in the great number of patients. Serious complications are very rare.

[Ambulatory septoplasty].

The AA. admit that the septoplasty under general anaesthesia is a surgical procedure that can be realized ambulatory, without added risks to patient. With viewpoint they have done a retrospective survey of the whole group of septoplasties performed under general anaesthesia at Torrelavega Hospital (Santander) in two years, between April 1995 and August 1997. The total number of cases amounted 56 (49 men and 7 women), lasting the procedure, more or less, one hour; postanaesthesia recovery also about 86 minutes and the hospital permanence about six hours and a half. Readmitted were 14 cases, because vomiting (7), for laryngeal spasm (2), bleeding (1), retention of urine (1) and other because vertigo (1), fever (1) and headache (1).

[Anaplastic carcinoma of the thyroid in a young man. A report of a case].

Anaplastic thyroid carcinoma is a tumor that has a very ominous prognosis, and is characteristic of people older than 60 years. Generally occurs as a neck lump of rapid growth rate, arousing compressive symptoms on nearby structures. We report the case of a 29-year-old patient who presented an anaplastic thyroid carcinoma. He was treated with surgery, chemotherapy and radiotherapy, but died 10 months after being diagnosed.

[Temporal artery biopsy].

The biopsy of the temporal artery is a necessary procedure in order to gain a certain diagnosis of temporal arteritis. Is an easy method and customarily sure, but needing a knowledge of the clinical picture of this arteritis and of it surgical anatomy. The AA. explain the technique they follow.

[Biopsy of the temporal artery. Experience with the use of a protocol at an ENT department].

INTRODUCTION: Temporal artery (TA) biopsy is required for the evaluation of several clinical scenarios because of the therapeutic implications of a positive result. We describe the experience of our department, which is the center for this technique in our hospital. PATIENTS AND METHODS: A retrospective study was made of all TA biopsies performed in the Department of Otolaryngology, Hospital Sierrallana, from January 1995 to April 1997. RESULTS: Thirty-four biopsies of the TA artery were performed in 33 patients for possible temporal arteritis. Mean age was 76 years (range: 61-87 years). Twelve were male and 21 female. Twenty-two patients (66.6%) had recent onset cephalalgia, 11 (33.3%) polymyalgia rheumatica, 5 (15.1%) mandibular claudication, and 8 (24.2%) ocular symptoms. In 75.8% ESR was elevated. An abnormal TA exam was observed in 9 cases (27.3%), 6 of which had a positive biopsy. Only 1 of the 15 patients without local findings at the physical examination had a positive TA biopsy. No information on the physical exam was recorded for the other 2 patients with a positive biopsy. CONCLUSIONS: TA biopsy is a relatively easy surgical procedure with a very low rate of complications. An abnormal physical TA examination correlates with a positive histopathological study.

[Intraparotid facial neurilemmoma].

Neurilemmomas or schwannomas are known to arise from the sheath of peripheral nerves. Out of the cranial nerves, the eighth nerve is the most frequently involved. Neurilemmomas arising from the facial nerve are are, and they usually affect the intratemporal portion of the nerve. Its location in the intraparotid portion is very uncommon. The case of a patient having an intraparotid mass is shown. After surgery it was found to be a neurilemmoma of the facial nerve. These tumors manifest as an asymptomatic mass in the parotid gland. Most of the times, diagnosis is made postoperatively. It is necessary to remove all the tumor with the caution to preserve the original nerve.

[Rendu-Osler-Weber disease (hereditary hemorrhagic telangiectasia). Report of 30 cases].

OBJECTIVE: To study the prevalence and modes of presentation of hereditary hemorrhagic telangiectasia (HHT) in Cantabria, Spain. MATERIALS AND METHODS: A retrospective study was made of all patients diagnosed as HHT in Cantabria in the last 20 years (1976-1995). The presence/absence of family history, recurrent nosebleed, mucosal and cutaneous telangiectasia, visceral involvement, and course of the disease were evaluated. RESULTS: Thirty patients ranging in age from 17 to 75 years were diagnosed as HHT in the study period. Most of them had a family history of recurrent nasal bleeding. The main symptom was nosebleed. Pulmonary arteriovenous fistulas were found in 7 patients and gastrointestinal manifestations in 10 patients. Five patients died of complications directly attributable to the disease. CONCLUSIONS: The minimum prevalence of HHT in Cantabria is 1:12,200, Patients with recurrent nosebleed, particularly if a family history of epistaxis is present, should undergo exploration of the oral cavity for telangiectasia. Early diagnosis of HHT can help to avoid unnecessary diagnostic procedures and contribute to the early detection of associated visceral malformations. The treatment of HHT should be individualized.
