Immunoprophylaxis with Simulect (basiliximab) in kidney transplantation: results from routine clinical practice at 18 kidney transplant units.

OBJECTIVE: The objective of this study is to assess a Simulect (basiliximab) regimen in routine clinical practice in the Spanish kidney transplantation units to evaluate efficacy and safety. METHODS: In this prospective, observational study, data on demographics, parameters of efficacy, and safety in patients who under with kidney transplantation treated with Simulect (basiliximab) were collected through an on-line collection system. RESULTS: One hundred sixty three patients at 18 kidney transplant units included 12 months follow-up. The patient mean age was 52 years (DS 13,67) including 96 (58.90%) men and 67 (41.10%) women. Cold ischemia time was 19 hours (DS 6,79). Only 2 patients presented with PRA >50%. For prophylactic immunosuppression, 67.13% of patients received triple therapy with CNI (cyclosporine 49.65% or tacrolimus 17.48%), MMF (66.43%) or AZA (10.49%), and steroids. Incidence of acute rejection (AR) at 12 months was 12.27% (1.84% steroid-resistant). In subgroup analysis, AR was 13.5% in nondiabetics and 4.5% in diabetics, including 3 steroid-resistant episodes (1.84%) in nondiabetics and none in diabetics. In relation to donor age, AR was incidence 10.3% in patients with kidneys from donors aged 50 years or younger and 10.6% when donors were older than 50 years, including 1 (1.73%) and 2 (1.93%) steroid-resistant episodes, respectively. The graft and patient survival rates at 12 months were 90% and 98%, respectively. CONCLUSIONS: Simulect (basiliximab) used in routine clinical practice provided good prophylaxis against acute rejection in several kidney transplant patient populations, similar to that observed in randomized clinical studies with excellent tolerability and safety.

[Incidence and prevalence of inflammatory bowel disease. Asturian study in 5 areas (EIICEA). Spain].

AIMS: To know and to compare Inflammatory Bowel Disease (IBD) Incidence and Prevalence rates in in five areas of Asturias (Spain). We conducted a prospective epidemiologic study of IBD in the Province of Liege (1 million inhabitants). PATIENT AND METHODS: We conducted a descriptive, populational, collaborative epidemiologic study, retrospective between 1954 and 1993 and prospective between 1994 and 1997. All patients diagnosed according to a standard protocol for case ascertainment and definition of IBD, aged 14 years or more are included, in five areas of Asturias (Spain) (461,965 inhabitants). RESULTS: For the period 1954 to 1997, 1018 IBD have been diagnosed [565 ulcerative colitis (UC) (55.5%), 415 Crohn's disease (CD) (40.8%) and 38 undefined IBD (IC) (3.7%)]; [482 women (47.2%), 536 males (52.8%)]. In the 4 year-prospective period, 306 cases were collected: 176 UC (57.51%), 110 CD (35.94) and 20 IC (6.53%); UC/CD: 1.6. Without appreciable and significant differences between Frequency of illness groups and sexes. IBD incidence rate (per 100,000 per year) (1954-97) is 5.12 (95% CI = 3.05-7.18) (UC: 2.84; CD: 2.08; IC: 0.19; UC/CD 1.36). In the 4 years- prospective study, IBD incidence rate is 16.55 (95% CI = 12.84-20.25), (UC: 9.52; CD: 5.95; IC: 1.08; UC/CD: 1.6). IBD prevalence rate in 1997 is 205.21 (95% CI = 182.14-227.29), (UC: 109.96; CD: 87.45; IC: 7.79). Comparisons have settled down among the studied areas, without finding differences statistically significant. CONCLUSIONS: Inflammatory Bowel Disease incidence and prevalence rates of in our region are homogeneous between the cities investigated and superior than those historically reported in Spanish studies. These results were similar to those observed in European studies.

[Epidemiology in inflammatory bowel disease in five areas of Asturias. Spain].

AIMS: The epidemiologic analysis inflammatory bowel disease (IBD) is a powerful research tool to assess the contribution of environmental factors to its etiology. IBD has been reported to have varying frequencies in different parts of the world, and there seem to be significant differences in the disease pattern and clinical course. The aim of the present study was to assess the disease pattern of IBD in Asturias (Spain). PATIENTS AND METHODS: A descriptive epidemiological population based study, retrospective (1954-1993) and prospective (1994-97), was performed to study 1018 patients found, bigger than 14 years, to have IBD, in five areas of Asturias (Spain) (461.965 inhabitants). RESULTS: During the period of time studied, we diagnosed 1018 IBD [565 ulcerative colitis (55.5%), 415 (40.8%) Crohn's disease and 38(3.7%) indeterminate colitis], with 482 females (47.2%), 536 males (52.8%), and male/female: 1.11. Age at diagnosis were 39.49 +/- 1.08 (95% CI : 38.41 +/- 40.57); (UC: 43.95 +/- 1.47; CD: 33.53 +/- 1.51; IC: 38.26 +/- 5.14. p = 0.000. Age at onset previously at diagnosis for UC: 42.84 +/- 1.34; CD: 30.68 +/- 1.40; IC: 36.74 +/- 4.86 (p = 0.000). Diagnosis criteria: UC: syntomatic 97.34% (p = ns), endoscopy 96.63% (p = 0.000 pathology 90.26% (p = 0.000). CD: radiology 83.61% (p =0.000). Study level in CD: 57.57 (p = 0.0005). Family history: 8.4%. The most frequent involvement at diagnosis of UC was proctitis only, in 13.6%, 269% rectum and sigmoid 26% let colitis, 20% pancolitis, and in CD colon only, in 16.7%, 30.3% terminal ileum, 41.3% ileo-colon of the patients. This also helps to explain the differences in severity, need for surgery, and survival noted between community based studies. CONCLUSIONS: We highlight the uniformity of distribution of the inflammatory bowel disease in relation to types and sex. The high frequency of familial Crohn's disease suggests a genetic predisposition. Highlighting a bigger morbilidad for the Crohn's Disease reflected in the surgical requirements, but however with smaller mortality that in ulcerative colitis.

[Cost savings derived from the participation in clinical trials].

BACKGROUND: Medical care of patients with essential hypertension implies considerable expenses for the National Health System (NHS). During the performance of clinical trials (CT), the promoter (usually the pharmaceutical industry) covers the expenses of patient care, which implies savings for the NHS. In this work, we quantitated cost savings derived from the participation of patients in clinical trials on essential hypertension in a tertiary care level Insalud hospital. PATIENTS AND METHODS: Savings were defined as the difference between the economical expenses of a patient before being enrolled in a clinical trial (CT) and that generated during his/her participation in the CT. Expenses of a given patient to the NHS were calculated from his/her clinical record during a time period equal to CT length in which the patient participated. Three financial allottments in four CT were analyzed (three with a 52-week duration and one of 14 weeks) that meant cost savings (total or partial): a) antihypertensive drugs; b) complementary tests, and c) medical visits. RESULTS: Seventy-three patients with essential hypertension were enrolled in the 4 CT; 59 (81%) patients were randomly assigned and 57 patients concluded the CT. The overall savings derived from the participation of the 57 patients was 2,950,423 pesetas. Most of these savings corresponded to drugs (70.7%). Savings derived from laboratory tests (14.1%) and medical visits (15.1%) were similar. The mean savings derived from the participation of one patient in a CT for 12 months was, at least, 70,564 pesetas (5,880 pesetas/patient/month). CONCLUSIONS: The participation of patients with essential AH implies considerable savings for the NHS.

[Paraquat poisoning: report of two cases and literature review].

Cases of poisoning with pesticides, especially suicidal ones, continue to be an important therapeutic problem. The heribicide paraquat (1.1' dimethyl-4.4' bipyridylium dichloride) is the second cause of pesticide poisoning in our country, which is associated with a high mortality rate. We report two cases of suicidal ingestion of paraquat who developed multiorgan failure with a lethal outcome. We also present a brief review of the literature, mainly focused on the different therapeutic options.

[Total neutrophils and age as prognostic factors of meningococcal sepsis].

Meningococcal sepsis is a frequent disease in our population with a bad prognosis even in a few hours. The search for easy and fast to obtain criteria of severity is essential decrease in order to the mortality rates. We have studied in 56 patients ranged from two months to six years, with bacteriological confirmation of meningococcal sepsis, the clinical and analytical parameters at admission, in order to find those parameters with a better correlation with the severity of the process. Global mortality was 25%. Eleven out of 21 children (52%) with less than 2,000 neutrophils in peripheral blood at the time of admission died, while only three died in the 35 resting group (p less than 0.01). When it was associated with a small age, less than two years, mortality raised up to 60%. In our series, the prognosis based upon the total neutrophils at admission, can be compared with that obtained from the five parameters of severity from Sthiem-Samrosch, that take an hour to be obtained. Absolute neutrophil count and age, can be obtained at the time of admission assessing the severity of the disease in short time.

Rubidium and sodium transport across erythrocyte membrane: alterations due to a circulating factor.

Essential hypertension patients can be divided into two groups on the basis of the presence or absence of a plasma factor able to inhibit rubidium uptake and sodium extrusion by the red blood cells. Nearly fifty percent of the patients studied present this factor. Preliminary results show that it is dialyzable, has a molecular weight around 500 daltons and cannot be extracted by chloroform.

[Naloxone and endotoxic shock: a wonder drug?].

Naloxone clorhidrate was administered intravenously to 15 patients with endotoxic shock and refractary hypotension, following meningococcal sepsis. Doses used were between 0.01-0.2 mg/kg. Three patients (two of them with moderate shock) showed an increase in sistolic arterial pressure, which was maintained and above 20 mm/Hg. Among the rest of them (except one with severe shock) naloxone action was non-existent in nine cases, or with short action and without answer to next doses in other three. Eleven patients died. A blood pressure increase was maintained only in two of the four survivors. Usefulness of naloxone in this kind of patients is not clear. Presence in endotoxin shock of a variety of refractory factors besides beta endorphins, greatly limits therapeutic efficiency of this drug.

[Mucocutaneous lymph node syndrome (Kawasaki disease). Presentation of 3 cases].

Authors report three cases of Kawasaki disease who were admitted to hospital during the period of November 1981 and January 1982. Two of these patients had an excellent evolution while the third one died in the second stage of the disease due to an anterior-external and septal myocordial infarction. They could not find etiological factors nor could establish a relationship to histocompatibility antigen. All the three patients were females.
