Circadian IOP-lowering efficacy of travoprost 0.004% ophthalmic solution compared to latanoprost 0.005%.

PURPOSE: The primary objective of this study was to determine the intraocular pressure- (IOP) lowering efficacy over two consecutive 24-h periods of travoprost 0.004% ophthalmic solution (Travatan) compared to latanoprost 0.005% (Xalatan) dosed once daily in patients with primary open-angle glaucoma or ocular hypertension. METHODS: This was a double-masked trial conducted at the Hospital Clínico San Carlos, Madrid, Spain. The primary objective of this study was to determine the IOP lowering efficacy of travoprost and latanoprost. During the eligibility visit, patients' IOP was measured throughout two consecutive 24-h periods every 4 h. Patients were then randomized to travoprost or latanoprost (one drop at 8 p.m. daily for 2 weeks). Sixty-two patients were randomized (travoprost n = 32; latanoprost n = 30). IOP was measured at week 2 every 4 h throughout two 24-h periods. All measurements were taken in both supine and sitting positions with the aid of Perkins applanation tonometry. Limitations of the study include a small sample size (due to the difficulty in recruiting patients in a study of this type) which enrolled only Caucasian patients and a short study duration. However, with 25 subjects per group, there was at least 90% power to detect a mean IOP change from baseline of 2.9 mmHg and 80% power to detect a difference of 2.5 mmHg between treatments. RESULTS: Patients on travoprost therapy showed lower mean IOP levels than those on latanoprost. This difference was statistically significant (p < 0.05) at 12, 16, 20, 24, 36, 40, and 48 h after the last dose for the supine position. The mean IOPs in the supine position throughout the first and the second 24-h period of the week 2 visit as well as for the 48-h visit were statistically lower (p < 0.05) for the travoprost group. Adverse events were mild and included hyperemia and corneal staining. Travoprost and latanoprost were both well tolerated. CONCLUSION: Mean IOP values were significantly lower for patients on travoprost for the majority of time points in the supine position.

[Amaurosis secondary to frontal traumatism].

CASE REPORT: We present the case of a patient diagnosed with amaurosis of the right eye secondary to a right frontal contusion. The energy of the impact was projected from the orbital ceiling to the minor wing of the sphenoid bone. This bone was fractured, thus reducing the optic canal diameter and damaging the optic nerve. DISCUSSION: In our case, we describe a mixed mechanism of injury, that is to say, a frontal contusion indirectly transmitted to the optic canal and a direct lesion of the optic nerve secondary to the movement of the minor wing of the sphenoid bone into the optic canal.

[Eyelid tick bite].

CASE REPORT: We describe a patient who was bitten on his right upper eyelid by a common canine tick. The tick was impregnated with gasoline prior to being removed. DISCUSSION: Tick bites are uncommon in ophthalmologic practice. They occur more frequently in rural areas, especially in spring and summer. Correct identification of the problem and treatment is essential. The tick needs to be completely removed in order to avoid the embedding of different parts of the insect. Zoonoses, such as rickettsiosis, arbovirus and paralysis due to ticks must be ruled out.

[Reduction of corneal permeability in patients treated with HP-guar: a fluorophotometric study].

INTRODUCTION: The incidence of dry eye disease, which mainly affects the corneal epithelium, is rising. The main treatment is still the use of artificial tears capable of improving the humidification and lubrication of the corneal epithelium, and avoiding its progressive functional failure. HP-Guar is a new compound used for this purpose. METHODS: We performed a prospective, masked and paired study on the left eye of 10 patients suffering from dry eye disease. Corneal permeability measurements were performed by fluorophotometry after instilling 40 microL of a solution of 2% sodium fluorescein, before and after treatment with the HP-Guar drops. The results were analysed using Wilcoxon test for paired data. RESULTS: A mean decrease in corneal permeability of 45%, after the use of HP-Guar drops was found (p=0.002). CONCLUSIONS: HP-Guar provides a new therapeutic option significantly decreasing corneal epithelial permeability.

[Optic nerve drusen and deep visual fields defects].

PURPOSE: Optic nerve drusen needs to be included in the differential diagnosis of pseudopapilledema. As the identification of this entity by funduscopy may be difficult, ultrasonography has thus become the gold standard for its diagnosis. Severe optic nerve drusen has been correlated with a reduction of the nerve fiber layer measured by optic coherence tomography and with the presence of serious visual field defects. To demonstrate the relationship between extensive optic nerve drusen and visual field defects. METHOD: A prospective observational study of the visual fields of a series of 5 patients with severe optic nerve drusen diagnosed by ultrasonography. RESULTS: Visual field defects of widely differing severity, from inferior nasal quadrant to severe hemivisual field defects, were described in each patient studied. CONCLUSIONS: Visual field defects of diverse severity are common in patients with deep optic nerve drusen. For that reason ultrasonography and/or optical coherence tomography is highly recommended where such visual field defects exist.

[Optical coherence tomography in solar eclipse retinopathy].

CASE REPORT: We describe the case of a patient suffering from acute visual loss soon after watching a solar eclipse. Optical coherence tomography was the main diagnostic tool used. DISCUSSION: Solar retinopathy is now an unusual cause of visual loss, although there are still some cases diagnosed, especially after viewing solar eclipses. Optical coherence tomography is suitable for detecting permanent retinal injuries related to solar exposure, with the outer retinal layers being typically affected.

[Postoperative course in 7 cases of primary sarcoma of the lung].

Unlike lung metastases of extrapulmonary sarcomas, primary sarcoma of the lung is very rare. We analyzed 7 primary sarcomas treated surgically in Hospital Universitario La Paz, Madrid, Spain between 1985 and 2001. Preoperative histologic diagnosis was correct for 2 patients. Surgery was performed in all 7 patients. Resection was extended to the left atrium in 1 patient, to the chest wall in another, and the parietal pleura were removed from 2 others. Surgical resection was considered complete in 6 cases. Histology revealed 4 cases of malignant fibrous histiocytoma, 1 angiosarcoma, 1 osteogenic sarcoma, and 1 undifferentiated sarcoma. Enlarged lymph nodes removed during surgery were tumor free. Three patients received complementary treatment. The 3 longest-surviving patients were treated with surgery alone; at the end of the study, these patients remained alive 16, 9, and 4 years after surgery. One patient has a recurrent lymph node tumor in a single lung. Three patients died within 1 year and another died 24 months after surgery. In conclusion, surgical treatment of primary sarcoma of the lung can achieve good survival.

[A comparative study of 0.15% sodium hyaluronate versus polyvinyl alcohol in the treatment of dry eyes].

PURPOSE: To evaluate the efficacy of two artificial tears for the treatment of dry eyes: 0.15% sodium hyaluronate and polyvinyl alcohol. METHODS: The study included 60 dry eyes in 30 patients. Fifteen of the patients were treated with 0.15% sodium hyaluronate and 15 with polyvinyl alcohol. Five drops a day for one month was given in both groups. Visual acuity, Schirmer's test, tear clearance, dry eye symptoms questionnaire, fluorescein and rose bengal staining, corneal sensibility, bulbar hyperemia, tear detritus, meibomian gland plug, palpebral erythema and edema before and after treatment were evaluated. RESULTS: Symptoms as judged by the dry eye questionnaire, as well as both fluorescein and rose bengal staining, were significantly improved by 0.15% sodium hyaluronate. The polyvinyl alcohol treated patients did not show a significant improvement in any parameter. CONCLUSION: Sodium hyaluronate was more effective than polyvinyl alcohol in the treatment of patients with dry eyes (Arch Soc Esp Oftalmol 2005; 80: 387-394).

[Late endophthalmitis following Ahmed valve].

CASE REPORT: A 71-year-old woman with a history of aphakic glaucoma underwent implantation of an Ahmed valve and scleral grafting in her right eye. Postoperative visual acuity was 0.5 and intraocular pressure was 12 mmHg during treatment with brimonidine tartrate (0.2%). Nine months after implantation she suffered a conjunctival infection which was treated with hygienic measures and topical antibiotic therapy. Four days later, she developed an endophthalmitis which was treated with topical, intravitreous and intravenous vancomycin and ceftazidime. The Ahmed drainage implant was replaced at 72 hours. Laboratory culture yielded Haemophilus influenzae. Four days later, the eye was enucleated. DISCUSSION: Endophthalmitis is an uncommon complication of glaucoma drainage implant surgery. Exposure of the drainage tube represents the greatest risk factor for this condition. Removal of the implant in the first 24 hours is recommended if a good visual prognosis is to be achieved.

[Malignant glaucoma following combined Ahmed valve implant and phacoemulsification surgery for chronic angle-closure glaucoma].

CASE REPORT: Simultaneous Ahmed valve implant, combined with phacoemulsification cataract surgery, is a useful therapeutic option for patients with chronic angle-closure glaucoma, when conventional filtering surgery fails. This combined approach permits both control of intraocular pressure and early recovery of visual function. We report the results in five patients successfully treated with this combined procedure, two of whom developed early postoperative malignant glaucoma. DISCUSSION: Predisposing anatomic features in patients with chronic angle-closure glaucoma, associated with sudden anterior chamber decompression and increased postoperative inflammation, may facilitate the development of malignant glaucoma following combined glaucoma implant and phacoemulsification surgery.

[Ocular infiltration in a patient with Multiple Myeloma].

CASE REPORT: We report the case of a patient with Multiple Myeloma Ig G Kappa stage IIIA and hepatic infiltration, who presented with ocular infiltration. The initial sign was a rapidly growing sub-conjunctival mass that caused ophthalmoplegia and increased intraocular pressure secondary to progressive infiltration of the orbit. DISCUSSION: Multiple Myeloma is a malignant tumor of plasma cells that can affect the eye and orbit, not only as an infiltration secondary to the disease but also primary as an isolated tissue proliferation (extramedullary plasmacytoma). Chemotherapy is the usual treatment given. Radiotherapy with or without excisional surgery is the first option for plasmacytomas.

A comparison of the fixed combination of latanoprost and timolol with the unfixed combination of brimonidine and timolol in patients with elevated intraocular pressure. A six month, evaluator masked, multicentre study in Europe.

PURPOSE: To compare the intraocular pressure (IOP) reducing effect and safety of fixed combination (FC) latanoprost/timolol with unfixed combination (UFC) brimonidine/timolol in patients with increased IOP. METHODS: In this 6 month, randomised, evaluator masked, parallel group European study, patients with glaucoma or ocular hypertension and IOP > or =21 mm Hg on monotherapy or >16 mm Hg on dual therapy received either FC latanoprost/timolol at 8:00AM or UFC brimonidine/timolol at 8:00AM and 8:00PM. The primary outcome was the difference from baseline to month 6 in mean diurnal IOP reduction. RESULTS: 325 of 334 randomised patients were included in intent to treat analyses (FC latanoprost/timolol, 163; UFC brimonidine/timolol, 162). Baseline diurnal IOP levels were similar: FC latanoprost/timolol, 26.4 (SD 2.7) mm Hg; UFC brimonidine/timolol, 26.5 (SD 2.8) mm Hg (p = 0.851). At month 6, levels were 16.9 (SD 2.8) mm Hg in FC latanoprost/timolol patients and 18.2 (SD 3.1) mm Hg in UFC brimonidine/timolol patients (p<0.001). No adverse events were reported by 76.4% and 75.5% of patients receiving FC latanoprost/timolol versus UFC brimonidine/timolol, respectively. Larger proportions of brimonidine/timolol treated patients reported study medication related adverse events (18.6% v 7.3%) and discontinued study participation because of this (10.8% v 1.8%). CONCLUSION: Fixed combination latanoprost/timolol administered once daily is both more effective and better tolerated than twice daily dosing with UFC brimonidine/timolol.

[Ultrasound biomicroscopy in pigmentary glaucoma].

OBJECTIVE: To study the anterior chamber in pigmentary glaucoma by anterior ultrasound biomicroscopy (UBM), and to determine the effect of the YAG-laser iridotomy on intraocular pressure (IOP) and the iris morphology in pigmentary glaucoma. MATERIAL AND METHODS: Seven patients (14 eyes) with a diagnosis of pigmentary glaucoma were studied. YAG-laser iridotomy was performed on all the patients with demonstrable iris concavity by UBM, performing UBM one week post-iridotomy and control of the IOP at one week, six months and at one year following treatment. RESULTS: The existence of iris concavity was demonstrated in 6 of the 7 patients (12 of the 14 eyes), performing a YAG-laser iridotomy in all of them. Remission of the iris concavity was observed in 5 of the 6 patients (10 of the 12 eyes) and good control of the IOP was achieved without additional topical treatment in 3 (6 eyes). At one year, the number of drugs needed for IOP control decreases (p<0,005, Wilcoxon test, correction for small samples). CONCLUSIONS: In our sample, the YAG-laser iridotomy rectifies the iris concavity present in pigmentary glaucoma, reducing the IOP and the number of drugs needed for its control.

Peritubular filtration as cause of severe hypotony after Ahmed valve implantation for glaucoma.

PURPOSE: To report peritubular filtration as the cause of severe hypotony after Ahmed valve implantation. METHODS: Interventional case report. In a 65-year-old man with primary open-angle glaucoma, ultrasound biomicroscopy was used to image the peritubular region of his right eye that had severe hypotony during the early postoperative period after an Ahmed valve implantation. RESULTS: The ultrasound biomicroscopy examination showed peritubular filtration. Additional surgery consisting of tube extraction, closure of the previous insertion opening, reformation of the anterior chamber with viscoelastic, and reintroduction of the tube through a new paracentesis was performed to solve this complication. Twenty-four hours postoperatively, the intraocular pressure increased to 10 mm Hg. During the 6-month follow-up, the mean intraocular pressure was 16.36 mm Hg (standard deviation, 1.45). CONCLUSION: The possibility of peritubular filtration should be considered in eyes with severe hypotony after restrictive valve implantation.

Immunohistochemical study of rabbit choroidal innervation.

Immunocytochemical methods with antibodies to the light (68 kDa), medium (160 kDa), and heavy (200 kDa) chain subunits of the neurofilament triplet have been used to visualize neuronal structures in rabbit choroids. Choroidal nerve fibers were present in the suprachoroid and vascular laminae and absent in the choriocapillary layer. These fibers may be classified as perivascular and intervascular. Perivascular fibers surround all arterial and venous blood vessels and form a network; these fibers were labeled with the three NF antibodies, although they were more easily visualized with anti NF-160 and anti NF-200 than anti NF-68. Intervascular fibers formed two groups. The first group consisted of fibers situated between the blood vessels and parallel to the blood vessel wall surface (paravascular fibers); these fibers were better observed using anti NF-160 and NF-200 than anti NF-68. The second group consisted of fibers which travel the entire length of the choroid until they reach the nerve plexus of the ciliary body (long tract fibers). The plexus was observed with anti NF-68, anti NF-160 and anti NF-200; however, the long tract fibers were more clearly visualized with anti NF-160 and anti NF-200 than with anti NF-68. Two types of choroidal cell were also labeled: ganglion cells and melanocytes. Ganglion cells are small, scarce neurons situated in the peripheral choroid; they were labeled with anti NF-160 and anti NF-200. The melanocytes were only labeled with anti NF-200 and they were the only non neuronal structure visualized using antibodies against neurofilaments.

Immunologic and clinical evaluation of postsurgical necrotizing sclerocorneal ulceration.

PURPOSE: To perform a clinical, laboratory and pathologic evaluation in patients who had developed a postsurgical necrotizing sclerocorneal ulceration to detect a serious associated autoimmune disorder and to treat the ocular disease early. METHODS: Nine patients with postsurgical necrotizing sclerocorneal ulceration after uneventful cataract extraction were studied by means of immunohistochemical techniques on conjunctival resections, immunologic serologic studies, and rheumatologic evaluation. Nine healthy subjects who underwent uneventful cataract surgery were used as controls. RESULTS: The pathologic studies showed a local immunoglobulin M (IgM) and IgG deposition, increased human leukocyte antigen (HLA-DR) expression, and a significant T-helper cell participation in conjunctival biopsies in the most severe ulcerations, which were detected in four patients with underlying autoimmune systemic disorder (rheumatoid arthritis, 45%) and only a macrophagic infiltration in the mildest ulcers in patients (55%) without immune disorders. Serologic features included high titers of rheumatoid factor in the four (45%) patients with rheumatoid arthritis, nonspecific serologic immune alteration in three (33%) patients, and were unremarkable in two (22%) patients. The medical and immunologic evaluations were negative in the control cases. Topically administered cyclosporin A healed the ocular disease. CONCLUSION: A surgically induced local autoimmune reaction could occur in the incision area in patients with systemic vasculitic disease. There was no underlying systemic disorder in the mildest ulcers, and these ulcers could be due to a defect in the surgical technique. Our results suggest the need for a detailed systemic evaluation in patients with severe postsurgical necrotizing ulceration. Early diagnosis and aggressive medical treatment of the ocular disorder improves the visual outcome.
