[Surgical treatment of supraventricular tachycardias (Wolff Parkinson White and Occult Kent). Experience at the "Ignacio Chávez" National Institute of cardiology].

We report our initial experience in the surgical section of the accessory pathway in thirteen patients: eight patients with Wolff Parkinson White syndrome and five with accessory pathway functioning only in direction ventricle to atrium (Kent concealed), with recurrent episodes of paroxysmal supraventricular tachycardia with no response to medical treatment. Three of these patients had episodes of auricular fibrillation with interval RR lower than 250 msec and one patient presented syncope. A total of fifteen accessory pathways were sectioned: 7 left lateral, 5 left posterior, one left posterolateral, one right lateral, and another one right anteroseptal. In the same procedure two patients had correction of another heart malformation: one with patent ductus arteriosus and another with and another with atrial septal defect. All patients had successful outcome, one of them needed a second surgery for persistent accessory pathway. We had two post-operatory complications: one mediastinitis and one patient with ectopic auricular tachycardia.

[Dermatophytosis in a urban setting: prospective study of 135 cases].

During a period of one year all patients referred to the Dermatology department with the diagnosis of dermatophytosis were studied to determine the incidence. Etiology, and epidemiology of these infections in the northeast of Madrid. We have evaluated the sensitivity and specificity of the direct examination with 10% KOH with respect to the culture as well as the correlation between the clinical diagnosis and the result of the mycologic examination. The annual incidence was 8.4 cases per 10,000 inhabitants and the most frequent causal agents in a total number of 135 patients were Epidermophyton floccosum in 48 (35.5%), Microsporum canis in 36 (26.6%), Trichophyton mentagrophytes in 28 (20.7%) and Trichophyton rubrum in 12 (8.8%). In 5 patients there were simultaneous infection by two different agents. In a third part of the cases a previous contact with animals was reported. The direct examination had a specificity of 98% and a sensitivity of 83%. The diagnosis in the nonungual infections was confirmed in the 54% of the cases. The incidence of dermatophytosis in our geographical area is high. Due to the similarities with other diseases, the mycologic examination by microscopy or culture is essential.

[Use of temporary pacemaker programming in VVT mode for the treatment of recurrent ventricular tachycardia].

This is a report of a 59-year-old man with a Björk aortic prosthetic valve and mitral commissurotomy with left ventricular dysfunction and recurrent ventricular tachycardia (VT) in spite of antiarrhythmic therapy with amiodarone. Serial electrophysiologic studies were performed using standard ventricular extrastimulation technique. During these studies, sustained VT was induced and terminated by programmable extrastimulus pacing. The usual rhythm of the patient was atrial fibrillation with slow ventricular rate. The Medtronic Spectrax model 5985 pulse generator was implanted. A temporary program converts this device from inhibited (VVI) to triggered (VVT) mode, permitting programmed ventricular stimulation through synchronization with chest wall stimulation by a standard external programmable stimulator. During episodes of VT the pacemaker was temporary programmed to the VVT mode with a refractory period of 220 msec. With the chest wall stimulation by a electrophysiology stimulator it was possible to convert the VT with 2 or 3 synchronised extra stimulus. Thanks to the triggered mode the pacemaker will fire when the chest wall is stimulated, working as an external electrophysiology stimulator.

Spontaneous rupture of the ascending aorta.

A case of non-traumatic rupture of the ascending aorta in an hypertensive patient is presented. The clinical findings suggested acute aortic dissection but the typical angiographic features of this entity were not found.

Connection between two peripherical markers in a group of asthmatic patients.

We have studied the goniometric value of the tda angle and the alpha 1 antitrypsin phenotypes in a group of thirty atopic patients with the following patterns: 1) they were affected with asthma, atopic dermatitis and rhinitis. 2) they presented positive prick skin tests for pneumo-allergens and had. 3) positive allergic antecedents. There exists a significant statistical value for the lower tda angle in the right hand, in the MZ phenotypes of alpha 1 antitrypsin bearers, with P less than 0.05 significance. In the left hand there are many individual values that are identical (62% of the total test effected) thus the statistical methods cannot be applied satisfactorily, bearing in mind the significant difference as the statistical group recognized. From this and other previous works, we can conclude that an individual whose right hand tda angle reaches values near 75%, and with a MZ phenotype of alpha 1 antitrypsin, has a greater possibility of pertaining to one of the groups of asthma, atopic dermatitis and rhinitis symptomatology.

[Congenital renal arteriovenous fistula].

On the occasion of a recent observation in our Centre of a case of congenital renal arteriovenous fistula, we make a review of the literature and give a detailed description of the clinical case presented. We stress the difficulty in differentiating between congenital and acquired fistulae and explain the course of therapy currently used.

[Adenomatoid tumor of the testicular vaginal tunic: apropos of a case].

The authors present a case of adenomatoid tumour of the vaginal testicle tunica, found by chance during a post-mortem study in a 70-year old male. After reviewing the literature they reveal its infrequency. According to the latest, histochemical and ultrastructural investigations carried out by several authors, its origin is mesothelial and therefore it may be called a benign mesothelioma.

[Applicability of the Spanish version of the Belloc's Physical Status Inquiry-PSI: a perceived health status measurement scale].

The objective of this work is to present the adaptation into Spanish of the Belloc's Physical Status Inventory-PSI-, and to describe the results of the preliminary validity of the Spanish version of this instrument. The process of translation and back-translation of the PSI revealed a high linguistic compatibility of the English and Spanish versions of the instrument. The construct validity was verified, on the same patients, by comparison of the PSI's data with the diagnostic categories and the presence of chronicity reported by the primary care practitioner, with the mental health status established by the GHQ-28, and with the utilization of health resources. We could say dat, in general, the PSI is a reliable measure of health status, being especially useful for its application on the general population and in patients in contact with medical agencies. It is thus justifiable to continue analyzing, by means of more specific methodological designs, the performance of this instrument in different Spanish populations.

[Intestinal complications from vascular prostheses].

Secondary FAE is a rare complication, usually located at the duodenum. The typical clinical presentation is like a digestive hemorrhage or a sepsis. We report two cases of FAE with atypical manifestations. The first case presented a lower digestive hemorrhage produced by the fistulization to the sigma. The second case appeared like an intestinal obliteration caused by the full emigration of a prosthesis to the jejunum. We wish to remark the importance of the clinical suspicion of a FAE (Key of diagnosis), and the sparing relevance of the complementary examinations and the urgency of a surgical treatment in order to avoid the high rate of morbi-mortality associated with this complication.

[The effect of chromium picolinate on the liver levels of trace elements].

Chromium picolinate has been implicated as a lipid and carbohydrate reducing agent, and therefore it may be a valuable adjunct to the treatment and prevention of diabetes and heart disease. This compound is inexpensive and apparently nontoxic. In this work, we have determined the influence of its administration (100, 200, 500 micrograms Cr/ml, for 7 and 21 days) on hepatic content of Zn, Mn, Cu and Fe of male Wistar rats. The results show a variation of the levels of these elements after the administration of chromium picolinate, although the differences are only significantly (p < 0.01) in the case of Mn. This influence is dose-dependent, occurring a decrease of 72% in the group treated with 500 micrograms/ml (Pic-500) respect to the content of control group.

[Epidemic outbreak of hepatitis B from the tattoo in gypsy families].

BACKGROUND: To document an outbreak of Hepatitis B in a gypsy community in the Upper Aragón region, as well as the control measures adopted. METHODS: Documented study of Hepatitis B cases and families, including an epidemiological survey and the determining of hepatitis B viral indicators (MVHB) using immunoenzymatic methods. RESULTS: 84.8% participation (39/45). During the months of February and March 1988, 5 cases of Hepatitis B were detected in a gypsy community in the Upper Aragon region (12.8% attack rate, 5/39), with an average age of 13.0 + 7.3, (4 women and one man). Four of the cases detected had previously undergone tatooing. The fifth case was due to direct transmission from mother to a recently born child. The MVHB study of families showed a further two cases. MVHB rate being 17.9% (7/39). Vaccinations were given to all persons susceptible to the disease. CONCLUSIONS: It is suggested that tatooing could be a significant factor to be considered in relation to the transmission of Hepatitis B in gypsy communities. Due to the high rate of incidence of the disease in this ethnic group, general vaccination is prescribed.
