Liver infiltrating mononuclear cells in children with type 1 autoimmune hepatitis.

OBJECTIVE: To investigate infiltrating cells in the liver of children with type 1 autoimmune hepatitis (AH-1). METHODS: liver biopsies from 24 untreated AH-1 patients (14 children, 10 adults), five patients with hepatitis C virus related chronic hepatitis (HCV), and 10 control liver specimens (CL) were processed for immunohistochemical cell characterisation. RESULTS: Two different cell distribution patterns were detected in the liver of patients with AH-1: (1) CD4(+) and CD20(+) cells were found in the central areas of the portal tracts (portal distribution); (2) CD8(+) cells were observed at the periphery of the portal space (periportal distribution). Some cell subsets, like CD56, CD57, Fas-L, and Bak, showed a non-defined distribution pattern. The presence of two well defined patterns of cell distribution was not observed in HCV and CL (CD4(+), CD20(+), and CD8(+) cells were uniformly distributed in the portal space). In AH-1 and CL, the NK markers CD56 and CD57 were found scattered throughout the liver parenchyma. However, in HCV biopsies, CD56(+) cells were also clearly increased in both the portal and the periportal areas. Biopsies of AH-1 and HCV patients showed a uniform distribution of Fas-L and Bak in the portal and periportal areas, with Bak staining also detected in the hepatic parenchyma. CONCLUSIONS: Despite clinical and genetic differences, there was a similar distribution of liver infiltrating mononuclear cells in children and adults with AH-1. These results raise the possibility of reclassifying cryptogenic chronic hepatitis by immunohistochemical analysis of infiltrating liver cells.

[Myocardial revascularization with the gastroepiploic artery. The initial experience].

The internal mammary artery is widely recognized as the graft of choice for coronary artery bypass grafting at present. Alternative conduits have been investigated in order to find other adequate long-term grafts. The right gastroepiploic artery has been recently used as a graft to bypass distal coronary vessels. From November 1989 to April 1990 we have implanted this artery in 33 cases (32 in situ and one free grafts). The pedicled grafts were implanted in 17 patients in the main right coronary artery, in 13 we grafted the right distal branches, in one the left anterior descending, and the circumflex branch in one. The free graft was implanted in the first diagonal. Mean grafts per patient was of three in this series, with a mean of 2 arterial grafts per patient. One patient died in the early postoperative period. The remaining patients had an uncomplicated postoperative period. Thirteen patients underwent graft and coronary angiography. Direct or indirect graft patency was confirmed in all cases. The final important issue concerning the long-term patency of this graft will be solved in the future, but short-term patency rates of the right gastroepiploic artery can be anticipated when proper techniques are used.

Langerhans' cells in squamous metaplasia of the human uterine cervix.

The number and shape of Langerhans' cells (LC) were studied by determining cytoplasmic formaldehyde-resistant ATPase activity in whole mounts of normal and metaplastic human cervical epithelium. In normal epithelium the number of LC per square millimeter was 52.75 +/- 2.21. A similar number was found in completely differentiated metaplastic squamous epithelium (49.11 +/- 2.42), but their shape was different with less branching processes. When metaplasia was still incomplete, and numerous mucous cells remained, no LC were present. On the basis of these results it is speculated that mucous cells provide a negative chemotactic stimulus which prevents migration of LC into metaplastic epithelium. When the latter is completely squamous it is repopulated by LC in a fashion similar to normal squamous epithelium.

[Late complications of mechanical and biological prostheses. Comparative study].

The late clinical complications subsequent to cardiac valvular surgery were studied in 150 patients in a period covering from January 1976, to December 1981. Mechanical prosthesis were used in 99 patients and biological in 51. Mortality, thromboembolic events, and haemorrhages, were all considerably higher in patients submitted to mechanical than biological prosthesis, whereas the periprosthetic leaks were more often observed in cases with biological prosthesis. With regard to other complications, such as infectious endocarditis, postperfusion and postpericardiotomy syndromes, etc., as well as the surgery definitive results, appreciable differences were not found. This, we can conclude that any of all now available prosthesis, is free of complications, and the election of a particular prosthesis (mechanical or biological) must probably be individualized for each particular patient.

[Antenatal education and recognition of the onset of labour].

BASIS: In recent years, there has been a progressive increase in the number of emergencies with a preterm labour diagnosis at the Virgen del Camino Hospital. In 1996 the anaesthesia service was introduced in the labour ward. Taking into account that no other factors have changed in this service, it is believed that its introduction might have contributed to the increase of preterm labour emergencies, as well as to a decrease in the number of pregnant women who attend antenatal classes. The aims of the present study are: to check whether there are any differences with respect to attendance of the antenatal classes and the number of pregnant women who come to the hospital in preterm labour or not, and to determine the reasons why pregnant women do not attend these courses. METHODS: A case control retrospective study. The cases were women at the term of their pregnancy who were attended at the Virgen del Camino Hospital in 1998. The sample was obtained by simple random sampling, using the registries of emergencies and the labour ward. The size of the sample was 150 cases and 150 controls, who were interviewed by telephone in order to complete the data collection questionnaire. Chi square was applied for the comparison of proportions and Student's t test for the comparison of means. CONCLUSIONS: It was found that pregnant women with preterm labour were less likely to attend to antenatal classes (p=0.02; OR=0.56;CI 0.33-0.96). Attendance of antenatal classes had a positive effect for the recognition of a suitable time for coming to the hospital because of the onset of delivery, with a reduction in preterm labour emergencies, and a reduction in the corresponding workload.
