Evaluation of a simple and low-cost external fixator.

Simple external fixation is becoming ever more popular for the treatment of fractures. Current commercially available external fixation devices are complex and expensive, affecting management of injuries in less developed countries. In this study, a new simple and low-cost external fixator called the AG (Alinoor-Goh) fixator, is evaluated biomechanically by direct comparison with a commercially available fixator device (AO). Four modes of loading were applied to each of the fixators: axial compression, anterior-posterior bending, medial-lateral bending, and torsion. The loads were applied directly onto a fractured-bone model made of perspex, which was immobilized by the fixator through six Schanz pins. From this, stiffness values were obtained by plotting load versus displacement. The results from the mechanical testing indicated that in axial compression the AG fixator stiffness was 55.7 +/- 0.197 N/mm (AO fixator was 57.7 +/- 0.186 N/mm); for anterior-posterior bending it was 56.8 +/- 0.153 N/mm (57.4 +/- 0.134 N/mm); for medial-lateral bending it was 11.4 +/- 0.036 N/mm (11.6 +/- 0.053 N/mm); and for torsion it was 1.16 +/- 0.002 N.mm/degree (1.16+/- 0.003 N.mm/degree). No significant difference was found in the stiffness of AG and AO fixators, under all modes of loading. A further comparison in terms of overall design, versatility in application and cost shows the AG fixator to be a viable, inexpensive option for the treatment of long-bone fractures.

The treatment of recurrent pelvic lymphocysts with marsupialization and functioning omental flap.

A new treatment for recurrent pelvic lymphocysts is reviewed. Nine women with recurrent symptomatic lymphocyst were treated by marsupialization of the lymphocyst to the peritoneal cavity and functioning omental flap. The omental flap was dissected off the greater curve of the stomach maintaining its blood supply from the appropriate gastroepiploic vessels. The flap functioned by absorbing the lymph that previously accumulated in the lymphocyst. The efficacy of treatment was accessed both clinically and with diagnostic imaging. The pelvic lymphocysts were successfully treated in all 9 cases. There was no significant morbidity and the average hospital stay was 7 days. Marsupialization of pelvic lymphocysts combined with an omental flap is effective, not associated with increased morbidity and has a very low recurrence rate.

Use of a computerized postural sway measurement system for assessing workers exposed to manganese.

1. Computerized postural sway measurement systems have been used recently in human physiology and pharmacology for determining postural stability, but their applicability for the assessment of the effects of exposure to neurotoxic agents is lacking. 2. We have examined the feasibility of using a computerized postural sway measurement system to assess the postural sway parameters of manganese exposed workers compared with a control group. 3. Sway parameter data were collected using a KISLTER multicomponent measuring platform (Type 9281B) connected to VICON motion analysis system for 13 exposed and 16 control subjects. 4. Significant differences in several of the sway parameters (Px, Py--mean distance (mm), from the centre of the platform along the X-axis [anterior-posterior movement] and Y-axis [lateral movement], respectively) between the exposed and control groups were observed even after adjustment for possible confounders. Computerized postural sway measurement system may be a useful method of assessing workers exposed to neurotoxic agents affecting posture.

A case of spontaneous resolution of nonimmune hydrops fetalis.

Spontaneous intrauterine resolution of severe nonimmune hydrops fetalis has been rarely reported in the literature. A case of severe fetal hemolytic anaemia with hydrops fetalis but subsequent spontaneous resolution is reported along with a discussion of possible aetiologies.

Campylobacter jejuni in pregnancy.

Case notes of pregnancies with proven Campylobacter jejuni infections were collected from 2 Queensland teaching hospitals and reviewed. Of these cases, 2 pregnancies (3 fetuses) resulted in stillbirths, 2 neonates required treatment with antibiotic therapy within 2 days of birth, while the remaining 2 pregnancies were treated at the time of the infection and were not associated with adverse outcome. Maternal Campylobacter infection should be actively sought for in the patient with suspected infectious diarrhoea.

Extramedullary blast crisis in chronic myelogenous leukemia. Demonstration of T-cell lineage and Philadelphia chromosome in a paraspinal tumor.

A case of chronic myelogenous leukemia (CML) with penetrance of the Ph1 molecular alteration into the T-cell lineage of a paraspinal tumor is reported. The T-cell nature of the paraspinal tumor was documented by immunochemical study and genotyping. The Philadelphia chromosome was also detected by genotyping in the same tumor cell population. To the authors' knowledge, this is the first case of extramedullary T-cell blast crisis in CML demonstrated in an extranodal tumor. The clinical significance of detecting extramedullary blast crisis, and the theories for the scarcity of T-cell transformation in CML are discussed.

Gait analysis in cerebral palsy.

Ambulation problems in cerebral palsy have been very difficult to analyse because it is the central control system rather than the motor system that is at fault. Until recently, decisions regarding surgical management has been made on clinical grounds, which by and large, have been subjective. Attempts to remedy this situation has resulted in the setting up of Gait Analysis Laboratories to provide a more objective method of evaluation of disorders of human motion. We have recently been using an opto-electronic computer-based gait analysis system to analyse normal as well as abnormal gait. Gait analysis can provide information regarding movements of both lower limbs in three planes, ground reaction forces, joint torque and dynamic electromyography. Children with various types of Cerebral Palsy have been assessed. The results of four clinical gait assessment cases are presented and discussed. Information provided by the Gait Analysis Laboratory has introduced more objectivity in pre-operative planning for these children.

Soluble adhesion molecules in inflammatory bowel disease.

BACKGROUND: Soluble adhesion molecules are elevated in a number of inflammatory conditions. AIMS: To investigate the correlation of soluble intercellular adhesion molecule-1 (sICAM-1) and sE-selectin with the activity of inflammatory bowel disease (IBD). METHODS: sICAM-1 and sE-selectin were measured by an enzyme-linked immunosorbent assay (ELISA) in 53 patients with ulcerative colitis (UC) and 38 patients with Crohn's disease (CD). RESULTS: Patients with active UC and CD had significantly higher sICAM-1 than patients with inactive disease and controls. Patients with pancolitis had significantly higher levels than patients with distal colitis. There was a significant difference in sE-selectin levels between patients with active CD and control sICAM-1. sE-selectin did not correlate with the Harvey Bradshaw index (HBI). C-reactive protein (CRP) and microalbuminuria were better markers than sICAM-1 or sE-selectin which correlated with serum tumour necrosis factor (TNF)-alpha. CONCLUSION: sICAM-1 and sE-selectin are elevated in the serum of patients with IBD but CRP and microalbuminuria reflect clinical disease activity more accurately. This study does not support the routine use of soluble adhesion molecules as disease activity markers in IBD.

T-cell receptor expression in lymphoid neoplasms. A comparison of phenotypic expression and genotyping.

T-cell receptor antibodies (TCR alpha beta and TCR gamma delta) were used for a flow cytometric study of 114 specimens of lymphoid neoplasms and normal controls in order to find out whether or not the expression of TCR proteins differs in between normal and neoplastic tissues. It was found that TCR alpha beta population was predominant in all categories except T gamma lymphoproliferative disorder (TGLD), T-cell lymphoblastic leukemia (T-ALL), and natural killer-like T-cell lymphoma (NKTL), in which three of 18, one of three, and two of two specimens, respectively, showed a predominant TCR gamma delta population. Natural killer cell lymphoma (NKL) showed essentially absence of either TCR alpha beta or TCR gamma delta protein. Therefore, TCR antibodies can be selectively used in cases of TGLD, T-ALL, and NKTL to substantiate their diagnoses. Furthermore, the absence of TCR protein is characteristic of NKL and helps to distinguish it from NKTL. Without the TCR antibodies, TCR gene analysis is sometimes needed to separate these two entities. When comparing with genotyping, 11 of 12 cases with TCR beta gene rearrangement and one of two cases with TCR alpha gene rearrangement expressed TCR alpha beta protein. One of four cases with TCR gamma gene rearrangement and both cases with TCR delta gene rearrangement expressed TCR gamma delta protein. Thus, TCR antibody phenotyping can reliably predict TCR genotypes under most circumstances.
