Two successful pregnancies in our dialysis center bring us an opportunity to update the related treatment. The success rate of this rare event is more than 30% at the moment. Maternal morbidity is reduced through close follow up. From the second semester, it is necessary to adapt and intensify dialysis, which becomes daily. At the third quarter, full-time hospitalization, extended to a high risk pregnancy unit at first incident, must be provided. Delivery may be normal. The infant is not particularly at risk for malformation, but is 30 to 35 weeks premature; at short time, its psychomotor development seems normal. To our sense, to influence systematically on women a "voluntary" therapeutic pregnancy interruption might be qualified if, when duly informed, women wish to try and keep their child.

Chylous ascites is a rare clinical entity often secondary to a lymphoma. In peritoneal dialysis even mild chylous ascites can cause cloudiness of the peritoneal effluent and thus lead to early diagnosis. Twelve cases of chylous ascites (defined by the presence of chylomicrons) among 230 patients with chronic renal failure treated in two peritoneal dialysis centers (Limoges and Saint-Pierre de la Réunion) are reported. A malignancy was encountered in only two cases (a B-cell lymphoma and an ovarian cancer). In the other cases, a precise diagnosis was difficult to establish and was only presumed. Thus chylous effusion in seven cases seemed secondary to cirrhosis, chronic pancreatitis, systemic amyloïdosis or cardiac failure. In three cases the cause was unknown, although microtrauma due to the Tenckhoff catheter was highly suspected. Chylous ascites lasted more than two years in four cases. The long-term nutritional consequences did not justify a change in the method of dialysis.

The high proportion of diabetic patients is partly responsible for the high frequency of chronic renal failure in Reunion. The confection and maintenance of an arterio-venous fistule is a major problem in those patients. We report herein our experience with temporary vascular access by internal jugular vein catherization with subcutaneous tunnelization using silastic catheters. The mean duration of utilisation of these accesses is 107 days for diabetic patients and 98 days for nondiabetic patients. The major complication observed is sepsis (18.36%). Diabetes mellitus which represent a traditional risk factor does not seem to be responsible here, the climatic and hygienic conditions prevailing in this part of the world could be a contributing factor. Nevertheless, we find the utilisation of these catheters a suitable solution during the waiting period because it is an easy operation, of the good quality of the material used and the comfort brought to the patient.
The prevalence of diabetes mellitus among patients treated for end-stage renal failure by dialysis in France was studied in two stages (UREMIDIAB Study). The first stage consisted of a questionnaire which was mailed to all dialysis centres in mainland France. The response rate was 80.8%, resulting in a study population of 12,903 patients. Of these patients 884 were declared diabetic (6.9%). Later 295 of them were interviewed by seven specially-trained physicians who checked the medical records together with the nephrologist in charge. Plasma C-peptide was measured in almost all of the patients. Effectively, 1.4% were found to have Type 1 diabetes and 5.5%, Type 2. Diabetic nephropathy was found to be the only primary renal diagnosis among 93.9% of Type 1 diabetic patients and 36.8% of Type 2. Of the latter 51.6% had a non-diabetic cause of renal failure. In the second stage a survey was later conducted in 13 of 14 dialysis centres located in the remote overseas French territories. Among 934 patients 1.04% were Type 1 diabetic and 19.67% Type 2 (22.9% altogether). Type 2 diabetic patients treated overseas were essentially non-Caucasians (92.6%). The sex ratio was 0.54 in the overseas territories vs 1.4 in the mainland. We conclude that the prevalence of diabetes among people on dialysis is low in mainland France. But there are striking differences in the prevalence of Type 2 diabetes among dialysis patients in mainland France and its overseas territories. These differences are not related to access to dialysis facilities.

In order to evaluate the prevalence of human leptospirosis in Reunion island and to identify possible risk factors, a study was realised on a representative population sample of 3.338 persons. The prevalence of leptospirosis, diagnosed by ELISA and confirmed by Micro Agglutination Test, was 1.1%. Male predominance and higher prevalence in rainy parts of the island, were confirmed. It has not been possible to display risk factors such as housing conditions or professional exercise. The serological repartition showed not only icterohaemorrhagiae serovar, but also canicola, panama and sejroe, especially in women. These results, compared with clinical studies (showing nearly exclusive male repartition, in agricultural workers, due to icterohaemorrhagiae serovar), confirm the double look of human leptospirosis in Reunion island: clinical leptospirosis, severe, concerning males, often countrymen, due to icterohaemorrhagiae serovar, and infraclinical leptospirosis, concerning principally females, which is a domestic illness, due to other serovars.
