A comparative study on the effects of propofol in emulsion and Intralipid on fat metabolism.

The present study compared the effects on lipid metabolism of propofol in emulsion (Diprivan) and a fat emulsion (Intralipid) in doses containing the same amount of soya bean oil. Twenty patients received epidural anesthesia for gonarthroscopy and were randomly divided in two groups. Before the start of surgery the patients of group P received a bolus of 2 mg/kg of propofol followed by a one hour infusion of 6 mg/kg/h. In group I, a bolus of 0.2 mg/kg of Intralipid 10% was followed by an infusion of 0.6 ml/kg/h for one hour. These figures represent equivalent amounts of soya bean oil administered. Blood samples were obtained for analysis of serum triglycerides, cholesterol, phospholipids, free fatty acids, aproproteins, and the separated lipoprotein fractions. There were few significant differences between propofol in emulsion and Intralipid in the effects on lipid metabolism. The changes in the measured serum lipid components were mild and rarely lasted longer than 2 h after the end of infusion.

[Retrospective studies of resistance patterns of antibiotics in 1971 to 1974 (author's transl)].

A retrospective study was made of the resistance patterns of 16700 pathogenic bacterial strains from the region of Dordrecht (Netherlands) from the years 1971 through 1974. The importance of computer analysis for questions about resistance and antibiotic usage monitoring is supported. The practical importance of those analyses is the fact that antibiotics often have to be given without a known antibiogram. In those instances one must rely on historical data to guide the therapy and to compose a rational antibiotic utilization review. It should be interesting to repeat the study in other areas to establish an overall pattern for a country and to compare the findings on a country-by-country basis.

A computerized study of bacterial resistance patterns (1971-1974). A preliminary report.

In an effort to provide information concerning the bacterial sensitivities in the Dordrecht area, continuing surveys are performed on the laboratory results. In 1974 information was presented on the bacterial sensitivities and resistance patterns during the period between 1968 and 1970. It has now been possible to study data from 1971 to 1974. Approximately 4000 sensitivity reports were surveyed--1000 from each year. A transfer sheet which could be entered into a computer was filled in for each patient who had a bacterial isolate and an antibiotic sensitivity test. This report gives the results of the survey. It is concluded that specific antibiotic testing must be performed. As an example more strains are sensitive to doxycycline than to tetracycline.

A computer study of bacterial resistance patterns to antibiotics.

The study of resistance patterns of strains of bacteria to antibiotics in different regions of a country and, if possible, comparing them at international level appears to be a useful exercise. The patterns of resistance during 1968-71 revealed an increasing resistance for most antibiotics with an unexpected increase for chloramphenicol. Notable is a decreasing resistance, not only of coagulase-positive strains of Staphylococcus aureus but also of other micro-organisms towards doxycycline. For practical purposes there was no difference between the sensitivy of the isolates from hospitalized patients and from out-patients, though less pronounced in the latter. The incidence of resistance among strains isolated from the paediatric ward was low. Comparatively small changes in molecular structure may result in relatively large differences in the sensitivity pattern; for example, between tetracycline and doxycycline (Vibramycin, Pfizer), the greatest activity is seen with doxycycline.
