Immunogenetic aspects of nasopharyngeal carcinoma. V. Confirmation of a Chinese-related HLA profile (A2, Singapore 2) associated with an increased risk in Chinese for nasopharyngeal carcinoma.

Histocompatibility locus A typing of 43 Malaysian Chinese and 51 Hong Kong Chinese patients with nasopharyngeal carcinoma (NPC) confirmed the association between the occurrence of A2-Sin 2 and the increased risk for NPC that was previously demonstrated in Singapore Chinese. The results support the previous interpretation that the histocompatibility locus A genotype of importance in NPC predisposition is the A2-Sin 2 haplotype. The histocompatibility locus A-linked, genetically determined NPC risk is common to Asian Chinese from at least three geographic locations.

Current status of the Cancer Registry and population-based studies in Hong Kong.

Despite the handicaps of shortage of staff, lack of a broad health insurance program, and the apathy of most of the medical profession, we managed to establish a Cancer Registry that is achieving near completeness in registration of cancers at certain sites. These cancers are among the most common types occurring in the population.

Superinfection epithelial nasopharyngeal carcinoma cells with Epstein-Barr virus.

Attempts were made to superinfect epithelial explant cell cultures prepared from nasopharyngeal carcinomas with Epstein-Barr virus. Virus-specific markers were observed in such cultures 3 days after superinfection. In addition, expression of Epstein-Barr virus early antigens was observed in epithelial cell explant cultures treated with iododeoxyuridine. The results suggest that epithelial cells of at least certain nasopharyngeal carcinomas possess the receptor for the Epstein-Barr virus and that the latent virus genome can be induced from epithelial cells prepared from such tumors.

Nasopharyngeal carcinoma. IX. Antibodies to EBNA and correlation with response to other ebv antigens in chinese patients.

Ninety-five sera from Chinese NPC patients in different stages of the disease, 38 sera from Chinese patients with other cancers, and 50 normal Chinese sera, were titrated for EBNA, VCA, EA and complement-fixing (CF/S) EBV-specific antibodies. The geometric mean (GMT) EBNA antibody titre of patients with NPC at stage I was found to be four times higher than that of normal individuals and increased in parallel with clinical deterioration. Antibody titres against EBNA did not correlate with either VCA or EA antibodies but, in general, correlated with CF/S antibodies. EA antibodies correlated relatively well with VCA antibodies and discriminated better than EBNA titre between NPC at stage I and controls.

Sero-epidemiology of the Epstein-Barr virus: preliminary analysis of an international study - a review.

Samples of Chinese, Indian, African and Caucasian populations, randomly selected in Hong Kong, Singapore, the West Nile District of Uganda, and Nancy, France, were titrated for antibodies to EBV, viral capsid (VCA) and complement-fixing soluble (CF/S) antigens. The age-specific prevalence of infection (as reflected by the proportion of VCA-positive individuals) varied greatly up to the age of 10 years in the four populations studied, the West Nile District of Uganda being outstanding in having an early and massive infection rate. Differences were also observed between ethnic groups in Singapore, where the Chinese appeared to have a delayed infection rate compared to the Indians. Immune response, as measured by the prevalence of CF/S antibodies and by the geometric mean titres (GMT) of VCA and CF/S antibodies, differed significantly between ethnic groups, Ugandan infants (1-3-year age-groups) having a humoral response to VCA and CF/S antigens as high as or higher than that of Burkitt's lymphoma patients, decreasing thereafter to levels lower than those of any other ethnic group observed. Indians in Singapore, known to be at no risk for NPC and BL, exhibited a higher and steadier immune response to VCA in going from young to old age-groups than did the Chinese.

Prospective study of the effects of ventilation tubes on hearing after radiotherapy for carcinoma of nasopharynx.

In patients with nasopharyngeal carcinoma, deafness sometimes occurs following radiotherapy. It is usually conductive, but may be sensorineural. Tinnitus is present frequently and usually is distressing. The role of ventilation tubes (grommets) in relieving these problems was assessed in a prospective randomized controlled trial of 115 patients. In the group with grommets, there was an improvement in hearing, with a reduction of the averaged air-bone gap (p less than .01). This was not found in the control group without ventilation tubes, who developed a larger conductive loss (p less than .01) and, in addition, a slight deterioration of the averaged bone conduction threshold (p less than .01). Surprisingly, the group with grommets did not develop this sensorineural loss (p less than .01). In addition, tinnitus was improved significantly by the insertion of ventilation tubes (p less than .01). Neither necrosis nor stenosis of the external auditory meatus was found in either group.

Cooperative trial of immunotherapy for nasopharyngeal carcinoma with transfer factor from donors with Epstein-Barr virus antibody activity.

A prospectively randomized, double-blind clinical trial was conducted to evaluate the effect of immunotherapy with transfer factor (TF) as an adjunct to radiotherapy of patients with stage III nasopharyngeal carcinoma (NPC). The TF was derived from normal young adults with a proven history of infectious mononucleosis and from normal blood donors with elevated antiviral capsid antigen antibody activity. TF prepared in this fashion was previously shown to convert the leukocytes of patients with NPC to a reactive state in vitro and when administered to NPC patients in vivo was associated with apparent slowing of tumor growth, marked lymphocytic infiltration of the tumor, and reconstitution of delayed cutaneous hypersensitivity reactions. From 1974 to 1977, 100 patients with NPC were entered in the study; one-half of the patients were treated with radiotherapy alone and one-half received radiotherapy and an 18-month course of TF immunotherapy. The patients were followed for at least 5 years. No significant difference in disease-free survival or survival was noted between the two groups of patients. The use of this particular preparation and dose schedule of TF in patients with NPC with regional disease was devoid of any anti-tumor activity.
