[The internal mammary artery binds more 125I-albumin than the aorta].

The internal mammary artery is currently the arterial replacement of choice for coronary artery bypass grafts. The authors studied the permeability of the vessel wall of the internal mammary artery and the ascending aorta in 12 patients undergoing coronary artery bypass grafts by comparing the 125I-albumin binding profiles. Their results suggest that albumin crosses the internal mammary artery more easily than in the aorta.

Oncogene expression in cervical intraepithelial neoplasia and invasive cancer of cervix.

Expression of the oncogenes Ha-RAS, c-MYC, and ERB-2 was investigated with an automated image analysis system in 12 specimens of normal cervix, 10 of cervical intraepithelial neoplasia (CIN) grade 1, 24 of CIN 3, and 10 of invasive cancer of the cervix. There was amplification of all three oncogenes in CIN 3 and invasive cancer compared with normal cervix and CIN 1. The difference was most pronounced with an antibody to the RAS p21 protein, with no overlap between CIN 3 and the normal range. This method might be useful in screening for cervical neoplasia, and for the determination of which CIN lesions require treatment.

Aortic 125I-albumin transport in patients with Marfan's syndrome and annuloaortic ectasia.

The morphologic, biochemical, and mechanical abnormalities of connective tissue fibrous proteins in Marfan's syndrome have been well studied, and their role in cardiovascular complications is well accepted. Less is known, however, about the state of the amorphous components of the aortic connective tissue. In the course of a study of transmural transport in blood vessels, we have had the opportunity to study dystrophic aorta from two young men who survived elective surgery; both with aortic insufficiency (AI) histologically compatible with Marfan's syndrome. One had recurrent chronic dissecting aneurysm (RCDA) as well. The aorta of the first (but not the second) was histologically compatible with Marfan's syndrome. Fresh specimens of intact ascending aorta were incubated in Krebs solution, pH 7.4, containing 125I-labelled bovine serum albumin for 2 h at 37 degrees C. The samples were then frozen, and serially sectioned in the plane of the lumenal surface. The radioactivity of the 20-micron thick sections was then determined, and expressed as a tissue/labelled solution concentration ratio. Transmural profiles of these ratios revealed no difference between the aorta of the RCDA patient with non-specific aortic dystrophy, and that of a 70-year-old man undergoing aortocoronary bypass. However, in the patient with aortic histology compatible with Marfan's syndrome, the average media concentration ratio was 5-fold less (4% vs. 20%).

Onco-developmental protein concentrations in the sera of patients with ovarian cancer prior to treatment.

Eight onco-developmental proteins have been measured in the serum of women prior to their operation and treatment for ovarian cancer. Alpha-foetoprotein, CEA, hCG and PAPP-A were not often elevated whereas CPAP, SP1, CanAg 50 and CA 125 were. Many of our findings differed from those of previous workers, possibly due to differences in clinical material and analytical technique. We conclude that large profiles of tumour markers are no longer of use in the investigation of ovarian cancer. At the present time we think that CA 125 could prove the most useful marker in the investigation of ovarian cancer.

Human papillomavirus in paired normal and abnormal cervical biopsies--implications for treatment.

A total of 143 consecutive patients with abnormal cervical cytology was examined at a large colposcopy clinic in Glasgow. Each patient had paired biopsies from normal and abnormal cervical epithelium examined both histologically and by immunoperoxidase staining for human papillomavirus (HPV) infection. More than 71% of the abnormal biopsies and 39% of the normal paired biopsies had histological evidence of HPV infection. Cytological evidence of HPV infection was seen in 38.5% of cervical smears. Immunocytochemistry revealed HPV antigen in 22% of the abnormal biopsies and in 4.2% of the 'normal' biopsies. The presence of HPV infection in colposcopically normal cervical tissue both inside and outside the transformation zone may help to explain why current methods for treatment of cervical HPV infection are often unsuccessful.

Investigation of perchlorate, phosphate and ion-pairing eluent modifiers for the separation of cephalosporin epimers.

The retention behavior of several pairs of 7 alpha- and 7 beta-cephalosporin epimers was investigated using perchlorate, phosphate and ion-pairing eluent modifiers. At pH 2.5, sodium perchlorate, sodium phosphate and sodium pentanesulfonate all provided separation of epimers with free 7-amino groups. When the 7-amino group was blocked, as in cephalexin and cefaclor, sodium perchlorate gave the best separation at pH 2.5. A tetrabutylammonium ion-pairing system at pH 7.0 provided separation of all epimer pairs containing a free carboxylic acid at the 3 position. Hydrophobic, residual silanol and ionic interactions were factors in the retention mechanism of the cephalosporins under the conditions investigated. An ionic interaction of perchlorate with the protonated amine of the cephalosporin was postulated as an explanation of the retention and selectivity effects observed with perchlorate as an eluent modifier.

The early mother-infant relationship and social competence with peers and adults at three years.

The impact of the early mother-infant relationship on social competence was investigated in Ss from a low-income population born in a public hospital in a large city. Assessments of the mother-infant relationship were made at three months and one year. Social competence was assessed in three-week day camp sessions when the children were about three years old. The mother's responsiveness at three months was related to the child's later social competence with adults, while the infant's responsiveness to mother at three months was not correlated with measures of social competence with either adults or with peers. Ainsworth's "strange situation" classification at one year was related to the success of bids for interaction, complexity of bids, and whether the child spent time near peers or near adults at three years, as well as to camp staff members' ratings of social competence with peers and with adults. Staff ratings of social competence with both peers and adults consistently correlated positively with more objective behavioral measures.

Pectus excavatum and polydactyly; an innocent syndrome.

A number of potentially serious congenital cardiac defects are associated with polydactyly. Of these, the best-known is that of Holt-Oram (Sanz 1973; Poznanski et al 1971, 1973) because of its surgical implications, described by Rainer (1972). Familial association has been reported by Yujnovsky (1974) with Polydactyly in three generations. It is the purpose of this report to record a case of accessory thumb and pectus excavatum in an otherwise healthy young woman of normal karyotype who asked that the extra thumb be removed for cosmetic reasons.

Spontaneous labour: when should the membranes be ruptured?

Sixty-eight patients (32 multigravidae, 36 primigravidae) with intact membranes admitted in early spontaneous labour were studied. Patients were randomly allocated to two groups: group I had an immediate amniotomy and group II were allowed to retain their membranes intact until full dilatation. Early amniotomy significantly shortened the length of labour and reduced the need for augmentation and instrumental delivery. There were no differences between the two groups in fetal outcome as measured by Apgar scores, umbilical arterial and venous blood pH, neonatal jaundice or admission to the special-care baby unit. Fetal heart-rate recordings obtained in group I by applying a fetal electrode after amniotomy were of superior quality to those obtained in group II by ultrasound and were more suitable for interpretation. 'Normal' and 'suspicious' tracings occurred equally in the two groups. Our results suggest some benefits from early amniotomy and no adverse effects on the fetus.

Induction of labour: a comparison of a single prostaglandin E2 vaginal tablet with amniotomy and intravenous oxytocin.

In a randomized controlled study of 100 women of low parity and favourable induction features, induction of labour by means of a single vaginal tablet containing 3 mg of prostaglandin E2 (PGE2) was compared with the conventional method of amniotomy and intravenous oxytocin. Four of the patients (8%) who received the prostaglandin tablet required additional intravenous oxytocin to achieve delivery. The prostaglandin group had a longer mean overall induction-delivery interval but a shorter amniotomy-delivery interval than the oxytocin group. One patient in the PGE2 group and two in the oxytocin group required caesarean section. The PGE2 treated patients expressed a higher level of satisfaction with their method of induction, they required less analgesia, had less blood loss at delivery and their babies had a lower incidence of neonatal jaundice.
