Short-term estradiol treatment enhances pituitary-adrenal axis and sympathetic responses to psychosocial stress in healthy young men.

Evidence from animal studies and clinical observations suggest that the activity of the pituitary-adrenal axis is under significant influence of sex steroids. The present study investigated how a short term elevation of estradiol levels affects ACTH, cortisol, norepinephrine, and heart rate responses to mental stress in healthy men. In a double blind study, 16 men received a patch delivering 0.1 mg estradiol/day transdermally, and age- and body mass index-matched control subjects received a placebo patch. Twenty-four to 48 h later, they were exposed to a brief psychosocial stressor (free speech and mental arithmetic in front of an audience). In response to the psychosocial stressor, ACTH, cortisol, norepinephrine, and heart rate were increased in both experimental groups (all P < 0.0001). However, the estradiol-treated subjects showed exaggerated peak ACTH (P < 0.001) and cortisol (P < 0.002) responses compared to the placebo group. Also, the norepinephrine area under the response curve was greater in the estradiol group (P < 0.05). Although heart rate responses differences failed to reach statistical significance, they, too, tended to be larger in the estradiol group. Neither mood ratings before or after the stressor, nor ratings of the perception of the stressor could explain the observed endocrine response differences. In conclusion, short term estradiol administration resulted in hyperresponses of the pituitary-adrenal axis and norepinephrine to psychosocial stress in healthy young men independent of psychological effects, as assessed in this study.

Experimental evaluation of a hydroxylapatite reservoir tube shunt in rabbits.

BACKGROUND AND OBJECTIVES: Anterior chamber tube shunts of various design are of increasing importance in the surgical management of refractory glaucoma. However, the reservoirs that are commonly used today are made of a silicone or an acrylic material that may become enveloped by a fibrous capsule, which in turn may impede aqueous resorption. In this study, the authors investigated the properties of hydroxylapatite (HA) as a reservoir in gonioimplants, using three different types of HA reservoir designs. It has been demonstrated that HA orbital implants become vascularized, and the authors hypothesized that this feature of HA might modify the development of the fibrous capsule, enhancing the efficacy and decreasing the size of the implants. MATERIALS AND METHODS: HA reservoirs with Silastic tube shunts were implanted in the right eyes of six Dutch-belted rabbits. Clinical examinations were performed and intraocular pressure was measured periodically for 16 weeks postoperatively. Prior to euthanasia, fluorescein-labeled latex microspheres were injected into the anterior chamber to demonstrate implant patency. Light microscopy was used to study the tissue response to the implants and the pathways of aqueous flow. RESULTS: In general, the authors found a granulomatous inflammatory response to the HA accompanied by neovascularization into the implant and minimal fibrous encapsulation of the reservoir. CONCLUSIONS: The use of HA as a suprascleral reservoir has several advantages, the major one being a theoretical increase in the functional lifetime of the implant.

The use of particulate hydroxyapatite and plaster of Paris in aesthetic and reconstructive surgery.

The authors describe their early investigative results of using a mixture of hydroxyapatite (HA) and plaster of Paris (PP) in skull and frontal sinus defects in a large series of cats. Histologically, bone was found to form and infiltrate the HA-PP implant over a period of months, with gradual resorption of the plaster in 6-8 weeks. Clinically, the HA-PP combination has been used in 24 patients over the past seven years for various skull, zygomatic, and mandibular defects.

Dental lesions in tumoral calcinosis.

Tumoral calcinosis (TC) is a rare inherited autosomal dominant metabolic disease manifested by elevated serum phosphorus and 1,25 dihydroxyvitamin D levels and periarticular cystic and solid tumorous calcifications. The dental findings in a large family have been critical in determining the genetic transmission of the condition. Radiographically the teeth have short bulbous roots, pulp stones and partial obliteration of the pulp cavity. Histologically, coronal dentin and a variable amount of radicular dentin appears to be deposited regularly. At nonspecific points the developing radicular dentin appears to encounter a mass of calcified material and proceed to grow around it. This mass has a unique histologic pattern with ovoid spaces surrounded by amorphous calcification. At levels of further root development the radicular dentin has an irregular bending tubule arrangement. The dental lesion of TC appears to be different from that of radicular dentin dysplasia in histologic structure and in the method of initiation of the dentin defect. These data suggest that the specific dental lesion is a new phenotypic marker for TC.

[Clinical endocrine investigations in patients with pituitary adenoma (author's transl)].

LH-RH tests of hypothalamo-pituitary gonadotropinfunction were performed in 26 patients with pituitary adenomata before and after prolactin suppression and/or surgical treatment. Various degrees of galactorrhea together with menstrual disorders were the key symptoms in all patients. There was a significant association of elevated prolactin levels with impaired LH-responses. Seven patients have undergone transspenoidal dissection of the pituitary adenoma and in five of them the pituitary LH-response reverted to normal (R2) after surgery. Nine of 15 amenorrhoic patients had a recurrence of menstrual cyclicity, partly biphasic within 3 months and a normalized LH-response after Bromocriptin therapy. 19 of our patients were desirous of pregnancy. Five pregnancies were confirmed within 4 months of treatment.

[A case of spontaneous rupture of the uterus (author's transl)].

A 22 year old primigravida at 40+ weeks gestation had an i.v. induction of labour with 5-10 mU/min oxytocin for 3 h which resulted in fetal distress without bleeding at 5 cm dilatation at station -3, vertex transverse. At emergency Caesarean section an occult rupture of the uterus was found. Maternal height 168 cm, weight 71.0 kg. Fetal length 53 cm, weight 4080 g. The problems of spontaneous rupture of the uterus without detectable prior damage of the uterine wall are discussed.

[Enzyme histochemical studies on the rat adrenal cortex, ovary, uterus and vagina following chlormadinone acetate administration, especially cholinesterase activity in myometrium and endometrium].

1. Female albino rats were treated with a total of 28 mg of chlormadinone acetate (CMA) for 28 days. In the adrenal cortex, the ovary, the vagina, and the uterus the activities of 3-beta-ol-steroiddehydrogenase, of dl-beta-OH-butric acid dehydrogenase, of alcaline and acid phosphatases, of DPN-diaphorase, of ATP-ase, and of non-specific esterases do not differ from untreated controls. 2. In the external muscle layer of the myometrium strong cholinesterase (ChE) activity was induced by C.M.A. A corresponding high ChE activity is normally found in immature rats or in estrus. 3. Furthermore, by treatment with CMA, ChE activity was induced in the tubular glands of the endometrium. This activity is found in the small parts of glomerate glandular terminals only but not in the rest of the glandular epithelium, nor in the epithelium of the cavum. It could be demonstrated that a corresponding ChE activity normally appears in the second third of pregnancy. The ChE activity induced by CMA was considerably higher and more widespread than during normal pregnancy. 4. It is concluded that in the endometrial glands a development similar to pregnancy is initiated by CMA. But development stops at the stage of ChE activity, thus leading to accumulation of ChE active cells.

Microwave-accelerated cytochemical stains for the image analysis and the electron microscopic examination of light microscopy diagnostic slides.

Recent studies in our laboratories have shown how microwave (MW) irradiation can accelerate a number of tissue-processing techniques, especially staining, to aid in the preparation of single specimens on glass microscope slides or coverslips for examination by light microscopy (and electron microscopy, if required) for diagnostic purposes. Techniques have been developed, which give permanently stained preparations, that can be studied initially by light microscopy, their areas of interest mapped, and computer-automated image analysis performed to obtain quantitative information. This is readily performed after MW-accelerated staining with silver methenamine by the Giammara-Hanker PATS or PATS-TS reaction. This variation of the PAS reaction gives excellent markers for specific infectious agents such as lipopolysaccharides for gram-negative bacteria or mannans for fungi. It is also an excellent stain for glycogen and basement membranes and an excellent marker for type III collagen or reticulin in the endoneurium or perineurium of peripheral nerve or in the capillary walls. Our improved MW-accelerated Feulgen reaction with silver methenamine for nuclear DNA is useful to show the nuclei of bacteria and fungi as well as of cells they are infecting. Improved coating and penetration of tissue surfaces by thiocarbohydrazide bridging of ruthenium red, applied under MW-acceleration, render biologic specimens sufficiently conductive for SEM so that sputter coating with gold is unnecessary. The specimens treated with these highly visible electron-opaque stains can be screened with the light microscope after mounting in polyethylene glycol (PEG) and the structures or areas selected for EM study are mapped with a Micro-Locator slide. After removal of the water soluble PEG the specimens are remounted in the usual EM media for scanning electron microscopy (SEM) or transmission electron microscopy (TEM) study of the mapped areas. By comparing duplicate smears from areas of infection, such as two coverslips of buffy coat smears of blood from a patient with septicemia, the microorganisms responsible can occasionally be classified for antimicrobial therapy long before culture results are available; gram-negative bacteria are positive with the Giammara-Hanker PATS-TS stain, and gram-positive bacteria are positive with the SIGMA HT40 Gram stain. The gram-positive as well as gram-negative bacteria are both initially stained by the crystal violet component of the Gram stain. The crystal violet stain is readily removed from the gram-negative (but not the gram-positive) bacteria when the specimens are rinsed with alcohol/acetone. If this rinse step is omitted, the crystal violet remains attached to both gram-negative and gram-positive bacteria. It can then be rendered insoluble, electron-opaque, and conductive by treatment with silver methenamine solution under MW-irradiation. This metallized crystal violet is a more effective silver stain than the PATS-TS stain for a number of gram-negative spirochetes such as Treponema pallidum, the microbe that causes syphilis.
