Treatment of neonatal thrombus formation with recombinant tissue plasminogen activator: six years experience and review of the literature.

BACKGROUND: Thrombosis is a relatively rare event in children. However, many conditions in the neonatal period result in an increased risk of thrombus formation. The major risk factor is the indwelling intravascular catheter. Numerous small studies have reported experience of thrombolytic treatment for neonatal thrombotic disease with a wide range of different thrombolytic agents in various forms of administration, dosage, and duration, but no conclusions on the most effective treatment for neonates has been reached. OBJECTIVE: To assess the efficacy and safety of thrombolytic treatment of neonatal catheter related thrombus (CRT) formation with recombinant tissue plasminogen activator (rt-PA). METHOD: Over a six year period, 14 neonates with CRT were treated with the same rt-PA protocol (an initial bolus of 0.7 mg/kg over 30-60 minutes followed by infusion of 0.2 mg/kg/h). RESULTS: Complete clot dissolution was documented in 11 patients, and partial clot lysis in two patients, leading to a patency rate of 94%. In two cases, local bleeding occurred, resulting in treatment failure in one case. Finally, antithrombin III substitution was required in one case. No other complications such as severe bleeding were recognised. CONCLUSION: With the use of close clinical and haematological monitoring on a neonatal intensive care unit combined with serial two dimensional colour echocardiography, the present rt-PA protocol was shown to be a safe and effective method of clot dissolution in neonates.

Comparison of two stent-grafts with different porosity: in vivo studies in a sheep model.

PURPOSE: To compare two stent-grafts with a polyurethane-carbonate (PUC) polymer lining of different pore size and water permeability in the iliac arteries of sheep. MATERIALS AND METHODS: Two stent-grafts with an inner PUC-lining of normal and low water permeability (normal, 1,200 mL/min/cm2; low, 280 mL/min/cm2; pressure gradient, 20 mm Hg) were implanted in each of 16 sheep. Hoop strength was two times higher in low-permeable than in normal-permeable stent-grafts. Patency was monitored with use of angiography and intravascular ultrasound (IVUS) after 1, 3, and 6 months. The sheep were killed after 1 or 6 months. Specimens were studied histologically. IVUS and histologic data were analyzed statistically. RESULTS: IVUS measurements showed a wider patent lumen of the low- compared to the normal-permeable prostheses at all time points (P < .03). This is likely due to the greater hoop strength of the low-permeable stent-graft. After 1 month, both types of prostheses demonstrated complete transprosthetic tissue penetration and were covered with neointima and endothelium. The neointimal area was greater in low- than in normal-permeable stent-grafts (1 month, P < .005; 6 months, P < .03). CONCLUSIONS: Presumably, the higher permeability of the normal-permeable stent-grafts led to the reduced neointimal area by improving transprosthetic capillary ingrowth and endothelialization.

Activities of antioxidants are affected by colloidal properties of oil-in-water and water-in-oil emulsions and bulk oils.

The activity of alpha-tocopherol, Trolox, propyl gallate, gallic acid, methyl carnosoate, and carnosic acid was studied in two oil-in-water (o/w) emulsions, in two water-in-oil (w/o) emulsions, and in bulk oil with and without added emulsifiers. All antioxidants had either moderate or higher activity in bulk oil than in the emulsions. In most emulsions, the most polar antioxidants, propyl gallate and gallic acid, exhibited either prooxidant activity or no antioxidant activity. Methyl carnosoate was the most active antioxidant in w/o emulsions but was less active than Trolox in o/w emulsions. alpha-Tocopherol was less active in bulk oil than in emulsions, but its activity in bulk oil was markedly enhanced by the addition of o/w emulsifiers. Partitioning of antioxidants, hydrogen bonding, interphase transport, surface accessibility, and interaction of emulsifier with antioxidants are considered to be important parameters that determine antioxidant activity in lipid-containing systems.

[Spontaneous lysis of pulmonary aspergillosis: "Aspergillus destroyed by Actinomyces"].

A 79 years old patient developed a large pulmonary aspergilloma in the cavities of his right upper lobe after postinfarctional pneumonia with local abscess formation. The clinical follow-up was characterized by recurrent hemoptysis resulting in marked anemia as well as by a continuous growth of the mycetoma. Suddenly a purulent gangrene of the whole upper lobe occurred infected by actinomyces israelii and staphylococcus but not aspergillus as it could be demonstrated in specimens from repeated transthoracic needle aspirations. After percutaneously inserted chest-tube drainage during 30 days the upper lobe cavity cleared up and the previously impressive aspergilloma had disappeared completely, however, the serum precipitins from aspergillus fumigatus still remained positive. After a course of several months without further pulmonary complications the patient finally died from a ruptured aortic aneurysm. It is suggested, that the spontaneous lysis of the aspergilloma was due to a deprivation of it' nutritive basis by the infected pulmonary tissue. A similar mechanism may also account for a sometimes successful treatment of pulmonary aspergilloma after injection of an amphotericin containing paste as a novel therapeutic strategy which is recommended in the case of patient's inoperable conditions.

Stabilization of Metal Nanoparticles in Aqueous Medium by Polyethyleneoxide-Polyethyleneimine Block Copolymers.

The branched polyethyleneoxide-polyethyleneimine (PEO)n-b-PEI block copolymers are synthesized and fractionated. The (PEO)n-b-PEI block copolymers are found to be able to form micelles in aqueous solution after addition of certain metal compounds (H2PtCl6, Na2PtCl4, RhCl3). The degree of copolymer polydispersity seems to play an important role in metal nanoparticle stabilization. Comparison of diblock and branched copolymers shows that as a rule branched block copolymers are the better stabilizing systems for metal nanoparticles. In the case of micelle formation due to interaction with metal compounds, fine control on colloid morphology can be provided, which leads to very uniform particles. Variation of different parameters (such as concentration, pH, type of reducing agent) affects the nanoparticle morphology as well. Copyright 1999 Academic Press.

Morphological Characterization of the Molecular Superstructure of Polyphenylene Ethynylene Derivatives.

Two different polyphenylene ethynylene derivatives, one partly hydrophobic and one hydrophilic, were investigated with a combination of X-ray and light scattering techniques and hydrodynamic techniques, as well as scanning force microscopy and transmission electron microscopy to elucidate their molecular structure and aggregation behavior in tetrahydrofuran and water, respectively. It turns out that both polymers possess a rod-like molecular architecture which, however, is the basis of a cascade of aggregation steps. Both, electron microscopy and X-ray analysis support the concept of a primary back-to-back aggregation of polymer chains into cylindrically shaped aggregates with high anisometry. The thickness of these aggregates was between 4.0 and 4.5 nm. The aggregates of the hydrophobic polymer further associate into fibrils and these fibrils form clusters of globular shape, though with high internal anisometry. Copyright 1999 Academic Press.

Predictive value of decreased p27Kip1 protein expression for the recurrence-free and long-term survival of prostate cancer patients.

The p27Kip1 gene has been identified as inductor of cell cycle arrest at the G1 checkpoint to prevent entry of somatic cells into the S phase of the cell cycle when substantial DNA damage has occurred. It has been suggested that decreased expression of the p27Kip1 protein may contribute to the development of human malignancies due to loss of critical antiproliferative mechanisms. In the present study, 95 specimens (T1-T4) from 95 randomly selected patients undergoing radical prostatectomy at the Urological Department of Hannover University (82 patients) as well as in the Josef Hospital Regensburg (13 patients) between 1981 and 1992 for whom tissue blocks for immunohistochemical investigation were available, were investigated for different biological and clinical characteristics as possible predictors for recurrence-free and long-term survival: age, depth of tumour infiltration, histological grade, lymph node status, as well as decreased expression of the p27Kip1 protein. After a median follow-up up of 56 months (24-151 months), seven of 21 (33%) patients (Group 1) with loss of p27Kip1 protein expression or a relative amount of <10% of positively stained tumour cells developed recurrent disease in contrast to 17 of 74 (23%) patients (Group 2) with retained p27Kip1 protein expression (> or =10% of positively stained tumour cells). The median recurrence-free survival was 14 months (5-40 months) for patients from Group 1 and 31 months (7-133 months) for Group 2 patients (P = 0.02). In multivariate analysis, loss of p27Kip1 protein expression was identified as the only independent prognostic parameter for recurrence-free survival. In contrast, neither the univariate nor the multivariate analysis showed a correlation between loss of p27Kip1 protein expression and the long-term survival of the patients. Prospective studies are urgently needed to confirm the independent prognostic value of decreased p27Kip1 protein expression together with overexpression of the p53 tumour suppressor protein in patients with localized prostate cancer. The availability of more refined prognostically important biological variables in addition to established prognostic factors like tumour stage or Gleason score might help decision making in patients at high risk for the development of local recurrence or systemic tumour progression.

Improved extraction procedure and RIA for determination of arginine8-vasopressin in plasma: role of premeasurement sample treatment and reference values in children.

We optimized an RIA for measurement of arginine8-vasopressin (AVP) in plasma by use of 100-mg Isolute C18 columns for extraction, addition of a preincubation step, and use of maximal dilution of a commercially available antiserum. The detection limit was 0.06 ng/L when 0.5 mL of acidified plasma was extracted. The within- and between-run CVs (n = 16) at physiological concentrations were 5.8-10. 2% and 6.5-11.7%, respectively. Prolonged storage of blood at 25 degreesC, but not at 4 degreesC, led to a significant increase in measured plasma AVP concentrations. When plasma samples were prepared at several centrifugation speeds, plasma AVP was significantly correlated with the platelet count (r = 0.899; P <0. 001). This emphasizes the need for careful sample preparation to avoid contamination of plasma with platelet-bound AVP. Basal plasma AVP in 203 children and adolescents (105 males and 98 females; ages, 1 day to 18 years) averaged 1.1 +/- 0.6 ng/L. There was no significant difference between males and females and no correlation with age. In 16 healthy adult controls, plasma AVP averaged 1.0 +/- 0.5 ng/L.

Relations between intracellular Ca2+ stores and store-operated Ca2+ entry in primary cultured human glioblastoma cells.

1. In primary cultured human glioblastoma cells extracellular application of ATP triggered elevation in cytoplasmic calcium concentration ([Ca2+]i) mediated entirely by generation of inositol 1,4,5-trisphosphate (InsP3)-dependent Ca2+ release from endoplasmic reticulum Ca2+ stores followed by the activation of store-operated Ca2+ entry into the cells. 2. Sensitivity of P2Y purinoceptors to extracellular ATP was regulated by extracellular Ca2+: in Ca2+-free extracellular solution the threshold concentration of ATP that induced an increase in [Ca2+]i was reduced by one order of magnitude. 3. Activation of Ca2+ release and store-operated Ca2+ entry was dissociated: low concentrations of ATP induced substantial Ca2+ release without activation of Ca2+ entry; activation of the latter required higher ATP concentrations. 4. Mitochondria participated in buffering Ca2+ loads that resulted from store-operated Ca2+ influx; in contrast Ca2+ released from intracellular stores was not accumulated by the mitochondrial depot. 5. We conclude that ATP-induced Ca2+ responses are governed by several pathways with different sensitivities to the agonist. This enables cells to respond either with pure Ca2+ release from intracellular stores (at low ATP concentrations) or (at high ATP concentrations) the response is amplified by plasmalemmal Ca2+ influx. Store-operated Ca2+ entry increases mitochondrial Ca2+ content providing a link between cellular activation and mitochondrial function.

Loss of human hippocampal adenosine A1 receptors in dementia: evidence for lack of specificity.

The density of hippocampal adenosine A1 receptors was measured in autopsy samples of six patients with dementia with either Alzheimer (three) or sclerosis type pathology (three) in hippocampus and six control patients. Kinetic parameters (Bmax and KD) were determined by means of quantitative autoradiography using [3H]8-cyclopentyl-1 ,3-dipropylxanthine as a radioligand. A significant 50% reduction of CA1 adenosine A1 receptors was observed in patients with dementia independent of the specific pathology. These results suggest that loss of hippocampal adenosine A1 receptors in dementia is not specific for Alzheimer type pathology.

The prognostic value of p53 for long-term and recurrence-free survival following radical prostatectomy.

In the present study, 76 specimens (T1-T4) from 76 randomly selected patients undergoing radical prostatectomy at Hannover University as well as in the Josef Hospital Regensburg (13 patients) between 1980 and 1992 for whom tissue sections for immunohistochemical investigation were available, were investigated for different biological and clinical characteristics as predictors for long-term and recurrence-free survival: age, depth of tumour infiltration, histological grade, lymph node status, as well as overexpression of the p53 protein (monoclonal antibody DO-1). After a median follow-up of 50 months, 6 of 18 patients (33%) with more than 20% of tumour cells stained positively for p53 died from tumour progression compared with 9 of 58 patients (16%) with less than 20% of tumour cells positive for p53. During univariate analysis, p53 overexpression (P = 0.011), histological grading (P = 0.009) and tumour stage (P = 0.024) were significant prognostic factors for survival, among which only p53 overexpression (P = 0.026) remained an independent significant predictor in multivariate analysis. Additionally, 18 of 66 patients (27%) with less than 40% positivity for p53 suffered tumour recurrence in contrast to 6 of 10 patients (60%) with more than 40% tumour cells exhibiting a positive staining reaction. In multivariate analysis, p53 overexpression was identified as the only prognostic parameter for recurrence-free survival (P = 0.005). Prospective studies are needed to confirm the independent prognostic potential of p53 overexpression in patients with localised prostate cancer. The availability of more refined prognostic factors should assist decision making regarding the value of radical prostatectomy versus a surveillance strategy for prognostically defined subgroups of patients.

Functional GABA(A) receptors on human glioma cells.

Glioma cells in acute slices and in primary culture, and glioma-derived human cell lines were screened for the presence of functional GABA(A) receptors. Currents were measured in whole-cell voltage clamp in response to gamma-aminobutyric acid (GABA). While cells from the most malignant glioma, the glioblastoma multiforme, did not respond to GABA, an inward current (under our experimental conditions with high Cl- concentration in the pipette) was induced in gliomas of lower grades, namely in 71% of oligodendroglioma cells and in 62% of the astrocytoma cells. Glioma cell lines did not express functional GABA(A) receptors, irrespective of the malignancy of the tumour they originate from. The currents elicited by application of GABA were due to activation of GABA(A) receptors; the specific agonist muscimol mimicked the response, the antagonists bicuculline and picrotoxin blocked the GABA-activated current and the benzodiazepine receptor agonist flunitrazepam augmented the GABA-induced current and the benzodiazepine inverse agonist DMCM decreased the GABA current. Cells were heterogeneous with respect to the direction of the current flow as tested in gramicidin perforated patches: in some cells GABA hyperpolarized the membrane, while in the majority it triggered a depolarization. Moreover, GABA triggered an increase in [Ca2+]i in the majority of the tumour cells due to the activation of Ca2+ channels. Our results suggest a link between the expression of GABA receptors and the growth of glioma cells as the disappearance of functional GABA(A) receptors parallels unlimited growth typical for malignant tumours and immortal cell lines.

Glutamate receptor activation can trigger electrical activity in human glioma cells.

Cells from major types of gliomas, i.e. oligodendrogliomas and glioblastomas, are able to generate action potentials upon a current injection similar to neurons (Patt et al. (1996) Neuroscience, 71, 601-611; Labrakakis et al. (1997b) J. Neuropath. Exp. Neurol., 56, 243-254. Here, we report that activation of ionotropic glutamate receptors by the selective agonist, kainate, or by glutamate itself, depolarized the tumour cells in culture and living slices from tumour tissue, and can elicit volleys of action potentials, as recorded with the patch-clamp technique. Sixty-six percent of the glioblastoma cells, 44% of the astocytoma and 86% of the oligodendroglioma cells responded to glutamate and the specific agonist of AMPA/kainate receptors, kainate. The involvement of non-NMDA (N-methyl-D-aspartate) receptors is further supported by the observation that both kainate and glutamate currents were blocked by CNQX (6-cyano-7-nitroquinoxaline-2,3-dione). The receptor activation was accompanied by an increase in cytosolic Ca2+, as recorded with a fura-2 microfluorometric system. The Ca2+ elevation was mediated by the activation of Ca2+ channels due to membrane depolarization. The presence of voltage-gated Ca2+ channels was confirmed by patch-clamp experiments. Taken together, these findings imply that the electrophysiological properties of glioma cells are more reminiscent of those of neurons than of glial cells.

An educational program in the medical uses of computers for rural physicians.

The Medical College of Ohio and three rural hospitals co-sponsored three educational programs in the medical applications of computers for rural physicians in Northwest Ohio. The College's Outreach Librarian played a major role in the programs. These programs were significant and possibly unique because of the comprehensiveness of the material presented and their focus on training rural physicians. Topics covered in the programs included: basic computer skills; online catalogs; online databases; CD-ROMs; the World Wide Web; and e-mail.

[Melatonin in the human--an overview].

Over the past two decades, the pineal gland with its major hormone, melatonin, has been shown to control the seasonal changes in a wide variety of species. Nevertheless, its place in human physiology is unclear at present. Melatonin is a hormone secreted during the dark period of the day. On comparing different age groups, serum melatonin concentrations are at a peak during early childhood, decline steadily until puberty, remain stable and, finally, decrease in elderly people. Melatonin appears to have a physiologic role as a nocturnal sleep inducer in man, but it might also have a certain effect on the human circadian rhythms, the reproductive and the immune system. Patients suffering from pineal tumors, disturbed sexual maturation or a phase shift in their circadian rhythms may also show altered serum melatonin levels. Exogenous melatonin shows a direct sedative-hypnotic action, with no serious toxic or side effects even after ingestion of large doses. Melatonin may gain importance as a natural hypnotic substance in the near future.

Line-Mixing Effects in Q Branches of CO2

A theoretical model based on the energy corrected sudden (ECS) approximation is used in order to account for line-mixing effects in Delta left and right arrow Pi infrared Q branches of 12C16O2. Its quality is demonstrated by comparisons with numerous laboratory spectra of CO2-He and CO2-N2 mixtures: three Q branches in the 4 and 17 μm regions are investigated at room temperature in a wide pressure range. The influence of mixing between Q(J) lines associated with odd and even values of the rotational quantum number J is demonstrated and analyzed in detail. It is shown that, in contrast to available fitting law approaches, the ECS model correctly predicts the influence of the parity of the rotational quantum numbers J and J' on coupling between the Q(J) and Q(J') lines. Comparisons between the effects of collisions of CO2 with N2 and He are made and analyzed. They show that these two systems involve different line couplings within the Q branch. Line-Mixing Effects in Q Branches of CO2

A theoretical model based on the energy corrected sudden (ECS) approximation is used to account for line-mixing effects in infrared Q branches of parallel bands of CO2. Measurements have been made using a tunable difference frequency spectrometer system in a Pi-Pi transition: spectra of the 11(1)1II <-- 01(1)0I Q branch near 3580.3 cm-1 have been recorded at room temperature for CO2-He and CO2-N2 mixtures in the 0.1-1 atm total pressure range. Comparisons of experimental and computed spectra prove the quality of the ECS approach. It is shown that the influence of line mixing on the shape of parallel Q branches is very weak and much smaller than in perpendicular transitions. This result is analyzed in detail and explained in terms of the effects of the vibrational angular momenta on the dipole transition moments and vector coupling coefficients involved in the relaxation operator elements. Biocompatible membranes preserve residual renal function in patients undergoing regular hemodialysis.

Initiation of hemodialysis causes a further decrease of residual renal function (RRF). To assess the impact of the biocompatibility of the dialysis membrane, we performed a prospective randomized investigation in 20 normotensive patients with tubulointerstitial nephritis. The patients were randomly allocated to treatment with either a high-flux polysulfone or cellulose membrane over a period of 12 months. RRF, defined by creatinine clearance and daily urine volume, was found to decrease in both groups of patients after the initiation of hemodialysis. Of the two membranes used, however, the cellulose membrane caused an accelerated decrease of RRF (P < 0.05). The better maintenance of RRF in patients treated with polysulfone membranes was associated with higher Kt/V values and increased hematocrit values (P < 0.05). The pathophysiologic mechanism underlying the more rapid decline of RRF could not be explained by intradialytic hypotension, but may be related to the nephrotoxic effects of inflammatory mediators due to bioincompatibility. Therefore, the use of polysulfone membranes might be preferable in patients with relevant RRF.
