[Special forms of ectopic pregnancy].

The symptomatology of acute intraabdominal and extrauterine bleeding presents a multiform picture. In the field of gynecology and obstetrics several other diseases are also observed besides the forms of ectopic pregnancies. - Among 554 ectopic pregnancies treated at our Department of Gynecology and Obstetrics between 1962 and 1981 we found 11 rare cases of these pregnancies: 2 cervical, 2 bilateral tubar, 5 primary intrafollicular ovarian, 1 primary, and 1 secondary abdominal pregnancy. These forms have been discussed and compared with international literature with regard to frequency, etiology, diagnostics, differential diagnostics, clinical course, therapy, and lethality.

[Cryotherapy of vulvar precancerous conditions--results of 12 years' treatment].

The recommendations for therapy of the facultative and obligate precancerous diseases of the vulva contain a lot of measurements extending from a local conservative treatment to the vulvectomy designated by a very different share of recurrences. With regard to the sure analgetic and controllable cell destroying effects of the coldness we cryosurgically treated 32 patients suffering from a kraurosis and 3 women with a histological secured Bowen's disease at the Department of Gynecology of the Medical Academy of Erfurt from 1971 to 1982. The present outcome proves this measurement being a very successful method but not always without recurrence. Therefore we consider cryosurgery as a real enrichment in conventional treatment of the precancerous diseases of the vulva but this measure can not be taken for the method of choice.

Glutamine distribution in mitochondria from normal and acidotic rat kidneys.

Mitochondria from control and acidotic rat renal cortex were incubated with labeled compounds to determine the distribution of L-glutamine. The mitochondrial preparations lacked detectable phosphate-independent glutaminase activity. In control mitochondria, the distribution of glutamine exceeded that of mannitol by 8% without an added energy source in the medium and by 9.9% when tetramethyl-p-phenylenediamine (TMPD) and ascorbate were present. The distribution of D-ribose (MW 150) was similar to that of L-glutamine (MW 146), indicating that these molecules penetrate into a slightly larger space than D-mannitol (MW 182). Inhibition of phosphate-dependent glutaminase activity with 5 mM glutamate did not alter the volume of glutamine distribution. In mitochondria from chronically acidotic rats, the glutamine/mannitol ratio was 1.03 in the absence of an energy source. With an energy source this ratio fell to 0.88 due to metabolism of glutamine during the separation process. Inhibition of glutamine metabolism with glutamate or alpha-ketoglutarate resulted in distribution ratios of 1.07 and 0.97, respectively. It is concluded that, although glutamine/mannitol ratios greater than 1.00 can be demonstrated in rat kidney mitochondria, such a finding may arise from causes other than the presence of free glutamine in the matrix space. Thus, it has yet to be established whether the inner membrane carrier for glutamine releases glutamine or glutamate into the matrix space.

Effect of arsenite on renal tissue slice metabolism in chronic metabolic acidosis and alkalosis.

Tissue slices prepared from renal cortex of littermate dogs with chronic metabolic acidosis or alkalosis were incubated in media with or without arsenite and containing 1 mM L-[14C]glutamine or [1,5-14C]citrate. The presence of arsenite increased the concentration of alpha-ketoglutarate in slices by 5--20 times the values found without this inhibitor. alpha-Ketoglutarate concentrations in acidotic slices were 40% or more greater than those in alkalotic ones when arsenited was present. 14C incorporation into alpha-ketoglutarate was also increased manyfold by arsenite with either labeled glutamine or citrate as substrate. 14CO2 production from labeled glutamine by over 90% and from labeled citrate by over 75%; the difference between 14CO2 production by acidotic and alkalotic slices was greatly reduced or eliminated by arsenite. These results suggest that in chronic metabolic acidosis metabolism of both glutamine and citrate is stimulated at a site or sites preceding formation of alpha-ketoglutarate.

Sonographic determination of first trimester twin chorionicity and amnionicity.

OBJECTIVE: To determine the reliability of first trimester sonography in the detection of twin chorionicity and amnionicity. MATERIALS AND METHODS: Sets of twins (179) were evaluated sonographically in the first trimester. Chorionicity was determined by identifying the number of gestational sacs. The presence or absence of an amnion about each embryo/fetus was also recorded. The placentas for each case were evaluated to determine chorionicity and amnionicity. RESULTS: Of the 179 twin pairs, 158 were dichorionic/diamniotic; 19 were monochorionic/diamniotic, and two pairs were monochorionic/monoamniotic. Chorionicity and amnionicity were correctly determined in each case. CONCLUSION: The chorionicity of twin pregnancies can be determined from the time that the gestational sacs are visualized within the uterus. Transvaginal sonography can reliably determine the amnionicity of monochorionic twin pregnancies after 7 weeks, menstrual age.

Hemotympanums secondary to spontaneous epistaxis.

Three cases of hemotympanum that resulted from spontaneous, atraumatic epistaxis are presented. Hemotympanum is a well known physical finding associated with traumatic basilar skull fractures but has not been reported to follow spontaneous nose bleeds that have not been previously treated with nasal packing. A discussion of epistaxis and hemotympanum is presented, and their implication for emergency physicians is discussed.

Effects of low-dose heparin on failure of intravenous infusions in children.

OBJECTIVE: To determine if a dextrose-saline solution manufactured under commercial conditions and containing 1 IU per ml of heparin prolongs the use of infusion sites in children. DESIGN: Double-blind randomized trial. PATIENTS: Eighty children in a medical ward. OUTCOME MEASURES: Failure of infusion sites from phlebitis and/or extravasation. INTERVENTION: Duration of use of infusion sites was calculated to the nearest hour. STATISTICS: Univariate survival analysis. RESULTS: The median time to failure when solutions contained the heparin (97 hours) was significantly increased (p < 0.0001) compared with control solutions (43 hours). CONCLUSIONS: Use of solutions containing low-dose heparin is recommended for children who might have problems with venous access.

The effects of donepezil in Alzheimer's disease - results from a multinational trial.

Donepezil has been shown to be well tolerated and to improve cognition and global function in patients with mild to moderately severe Alzheimer's disease (AD). The current trial was undertaken to investigate further the efficacy and safety of donepezil, in a multinational setting, in patients with mild to moderately severe AD. This 30-week, placebo-controlled, parallel-group study consisted of a 24-week, double-blind treatment phase followed by a 6-week, single-blind, placebo washout. Eight hundred and eighteen patients with mild to moderately severe AD were randomly allocated to treatment with single, daily doses of 5 or 10 mg donepezil, or placebo. The two primary efficacy measures were: a cognitive performance test, the Alzheimer's Disease Assessment Scale-cognitive subscale (ADAS-cog) and a global evaluation, the Clinician's Interview-Based Impression of Change with caregiver input (CIBIC plus). Secondary outcome measures included the Sum of the Boxes of the Clinical Dementia Rating Scale (CDR-SB), a modified Interview for Deterioration in Daily living activities in Dementia (IDDD) and a patient rated quality of life assessment. Statistically significant improvements in cognitive and global function were observed, as evaluated by ADAS-cog and CIBIC plus, respectively, in both the 5 and 10 mg/day donepezil groups, compared with placebo. Treatment-associated changes were also observed in functional skills, as shown by improved scores on the CDR-SB and the complex-tasks component of the IDDD. A dose-response effect was evident, with the 10 mg/day donepezil group demonstrating greater benefits in all outcome measures than the 5 mg/day group. Donepezil was well tolerated by this patient population and did not produce any clinically significant laboratory test abnormalities. The results of this study confirm that donepezil is effective and well tolerated in treating the symptoms of mild to moderately severe AD.
