[Immunotherapy of urinary tract infections].

In the discussion on prevention of urinary tract infection, interest is now centered on virulence factors of microorganisms and immunological defense mechanisms of the host organism. In this article current research results and the clinical aspects of an immunological treatment method for urinary tract infection are summarized and discussed.

[Tooth health and periodontal condition in adults with and without root caries].

In a population sample of 2261 men and women aged between 35 and 74 years the prevalence of root caries increased for both sexes consistently with age and ranged between 49 and 73%. In the mean 1.8 to 2.9 teeth were affected. Subjects with root caries had fewer teeth, more teeth with untreated coronal caries, and more teeth which needed conservative treatments. The number of extractions required was the same in subjects affected by root caries. In the age older than 65 years subjects affected by root caries had more teeth in the average of 2 teeth and significantly more fillings in their teeth. If a DMF/T-index of 15 to 19 was recorded nearly half of all subjects were affected by root caries. In individuals with root caries lesions aged between 35 and 54 years significant severe periodontal disease were seen.

Relationship of lipoprotein(a) to variables of coagulation and fibrinolysis in a healthy population.

In the Prospective Cardiovascular Münster (PROCAM) study, serum lipoprotein(a) [Lp(a)] and its relationship to pro- and anticoagulatory as well as fibrinolytic indices were determined in a large group of employees: 864 men (m) and 373 women (f), ages 16-65 years. Univariate statistical analysis showed Lp(a) concentration to be associated with fibrinogen concentrations in both sexes (m: r = 0.08, P less than 0.05; f: r = 0.20, P less than 0.001), but not with euglobulin fibrinolysis activity, tissue-type plasminogen activator, plasminogen activator inhibitor type 1 (PAI-1), or the split products of cross-linked fibrin (d-dimer). In women only, Lp(a) was significantly correlated with antithrombin III (r = 0.15, P less than 0.01) and Protein C (r = 0.17, P less than 0.01). Further sex-related differences were seen in the relationship between Lp(a) and age (m: r = 0.05; f: r = 0.23, P less than 0.001) and body mass index (m: r = 0.01; f: r = 0.19, P less than 0.001), primarily as a consequence of remarkable differences of Lp(a) concentrations between postmenopausal (mean = 79.4 mg/L) and premenopausal women (mean = 51.5 mg/L, P = 0.001). Multiple-regression analysis demonstrated a significant negative correlation of Lp(a) to PAI-1 (m: beta = -0.12, P less than 0.01; f: beta = -0.14, P less than 0.05) and a positive correlation to cholesterol (m: beta = 0.18, P less than 0.001; f: beta = 0.17, P less than 0.01) and systolic blood pressure (m: beta = 0.08, P less than 0.05; f: beta = 0.11, P less than 0.05).

[Will root caries become a treatment problem of the future?].

Based upon the present prevalence of root caries an attempt is taken to evaluate the future importance of this carious type. A population sample of 2,261 men and women aged 35 to 74 years were examined. The participation rate was 81%. Root caries was recorded when the lesion was obviously soft and located on cementum. The prevalence of root caries increased for both sexes almost consistently with age and ranged between 49 and 73%. In the mean 1.8 to 2.9 teeth were affected. Root caries was found more often among men than women. Over the sixth decades the reduction of the relative frequency of root caries was stated. Up to this age the rates of tooth extraction were higher than the rates of incidence. If it succeeded that in the future aged people will retain more teeth than nowadays a increase of root caries can be expected.

[Kininase II (SACE) and renin in sarcoidosis patients and healthy probands in exercise testing].

Over a period of three years, we investigated the behaviour of renin and kininase II in 26 patients with active, histologically confirmed sarcoidosis in comparison with healthy test subjects, during ergometer exercise. Renin activity was determined radioimmunologically, and kininase II by means of spectral photometry (Cushman-Lieberman method). In both groups, a significant increase in renin and kininase II during the course of exercise on the ergometer was observed, which, however, was more marked in the sarcoidosis group of patients. Furthermore, the activities of both enzymes were highly significantly elevated as compared with the group of healthy subjects in each individual phase of exercise. In addition--if we consider the average values of the two enzymes in each of the individual subjects--it would seem possible in most cases to correctly ascribe the person involved to the group healthy subjects or patients, in particular when the average values of renin and kininase II obtained as described above, are multiplied together.

[Mullerian adenosarcoma].

The Müllerian adenosarcoma is an extremely rare entity of tumours of the group of the Müllerian and mesodermal mixed tumours. This tumour seems to be a malignant transformation of the uterine papillary cystadenofibroma. The periglandular malignant transformation of the stromal component and the benign glandular structures are typical signs of the Müllerian adenosarcoma. In accordance with references the prognosis of the adenosarcoma is better than those of other Müllerian mixed tumours. The demonstrated cases warn against undervalue.

Aged

[Treatment of primary dysmenorrhea with Jenapharm piroxicam].

36 patients with 432 menstrual cycles and primary dysmenorrhea were treated with Piroxicam-Jenapharm. Piroxicam-Jenapharm was given in a dose of 40 mg once daily for the first 2 days followed by 20 mg once daily. Piroxicam, a nonsteroidal-anti-inflammatory drug (NSAID) is an effective drug in reducing menstrual pain intensity in 80%. Piroxicam was well tolerated with no patient reporting side-effects. The treatment of primary dysmenorrhea with total 100 mg Piroxicam-Jenapharm over 3 days is recommended.

[Prevalence of root caries in a healthy adult population].

An epidemiological study involving 963 subjects aged 25 to 64 years revealed at least one carious and/or filled root surface in 40.2% of all females and in 42.7% of all males. The number of teeth and tooth surfaces affected by root caries increases continuously with age. In relation to the total number of teeth, males display a higher prevalence of root caries than females, with the exception of men aged between 55 and 64. High-risk susceptibility shows no correlation with age, although there are more surfaces at risk in males. Independent of the sex, root caries prevalence is highest in the molars and premolars of the mandible and in the canine of the maxilla; lower incisors are the teeth with the least caries involvement. Labial root surfaces are most frequently affected by caries in the mandible; the prevalence rate is twice as high in males than females. The lingual surfaces of maxillary molars of females and the proximal and buccal surfaces of incisors of males are the most frequently affected tooth surfaces.

[Proteolytic activities in bronchoalveolar lavage fluid in pneumonia and chronic bronchitis].

In 63 patients with chronic bronchitis, 61 patients with acute bacterial pneumonia, and in 11 control subjects, broncho-alveolar lavage fluid was investigated for its protease content. In smokers, we found the usual pronounced protease burden. As was expected, the two pathological groups also differed markedly on other points. In pneumonia, not only free elastase activity (from granulocytes (PMN] was markedly elevated, but also cathepsin-B-like activity (from alveolar macrophages (AM] was greatly increased, which would point up the important role of AM in this form of inflammatory. In this case of chronic bronchitis, function-analytically determinable obstruction parameters were associated with an increase in the elastase-like enzyme (from PMN), so that chronic obstructive bronchitis probably represents a special form of inflammation.

[Neonatal percentile values for the GDR--1985. Correlation of body weight and length in newborn infants (calculation of birth weight/body length in cm. with reference to gestational age)].

Taking account of the gestational age (28th to 42nd completed week of pregnancy) the sex-related ratio of birth weight to length (g/cm) for newborn infants is reported. This shows that observation of the correlation between birth weight and length with reference to gestational age makes a more accurate classification of newborn infants possible, rather than using a two dimensional classification (birth weight week of gestation).

[Effects of fentanyl peridural analgesia in labor on fetal heart rate, uterine contraction and postnatal adaptation].

The opioid Fentanyl was used in a dosage of 0.15 mg via a peridural catheter for birth analgesia (PDA) in a high risk collective of 40 female patients. A prolonged analgesia with good acceptance could be achieved with low dosage. The advantages of the mobilisation could be fully used because of telemetric monitoring and missing influence of the sympathetic nerve and undisturbed motoric activity. If a caesarean section was necessary we continued the peridural analgesia using 0.5 percent Marcain. Postnatal assessment of the condition of the newborn was not disturbed. Short-term irregularities in CTG--a decrease of the rate of acceleration, a silent oscillation pattern--are interpreted in the sense of a sleeping pattern, without harmful influence on the fetus. At present PDA is the most effective method in birth analgesia. It represents a valuable contribution to the care in the field of psychomotoric preparation of patients to birth. Considering the contraindications PDA with Fentanyl can be recommended in obstetrics.

The c1 repressor of bacteriophage P1 operator-repressor interaction of wild-type and mutant repressor proteins.

The c1 repressor gene of bacteriophage P1 and the temperature-sensitive mutants P1c1.100 and P1c1.162 was cloned into an expression vector and the repressor proteins were overproduced. A rapid purification procedure was required for the isolation of the thermolabile repressor proteins. Identification of the highly purified protein of an apparent molecular weight of 33,000 as the product of the c1 gene was verified by (i) the coincidence of partial amino acid sequences determined experimentally to that deduced from the c1 DNA sequence, and (ii) the temperature-sensitive binding to the operator DNA of the thermolabile repressor proteins. Analysis of the products of c1-c1.100 recombinant DNAs relates the thermolability to an unknown alteration in the C-terminal half of the c1.100 repressor. Binding to the operator DNA of c1 repressor is sensitive to N-ethylmaleimide. Since the only three cysteine residues are located in the C-terminal half of the repressor it is suggested that this part of the molecule is important for the binding to the operator DNA. This assumption is supported by the findings that a 14-kDa C-terminal repressor fragment obtained by cyanogen bromide cleavage retains DNA binding properties.

Amino Acid Sequence

[Dose-reduced antihypertensive agents--use in complex nonmedicamentous therapy of hypertension].

In order to estimate the influence of a non-medicamentous therapy (CNT) on the consumption of medicaments and coronary risk in high blood pressure 73 hypertensives of a medicamentously stabilized CNT-group were examined in comparison to a group of the same size of patients with hypertension who were managed exclusively medicamentously for behaviour of blood pressure, need of antihypertensive drugs and changes of hypertension-associated risk factors. After an exactly controlled 6-month treatment hypertensives with additionally recommended far-reaching CNT showed an economization of medicaments by scarcely the half in comparison to the reference group. By means of suitable control methods a causal non-medicamentously conditioned decrease of blood pressure could be excluded. A different need of antihypertensive drugs was simulated by the exacter intake of medicaments in the index-patients. Notwithstanding the metabolic effects of the additional therapy have induced a positive change of atherogenic lipids. The examinations indicate in general the difficulty of the judgement of efficacy of non-medicamentous therapeutic measures in connection with a rational dose-reduced long-term therapy with antihypertensive drugs.
