The role of nonsteroidal anti-inflammatory drugs in the treatment of acute soft tissue injuries.

OBJECTIVE: Nonsteroidal anti-inflammatory drugs (NSAIDs) are frequently used to hasten the return of injured athletes to competition after injury. Evidence demonstrates that while NSAIDs may speed recovery after acute soft tissue injuries, long-term healing may be compromised. This review aims to assess the effects of NSAIDs on inflammation and healing associated with acute soft tissue injury. DATA SOURCES: CINAHL (1982 to 1997), SPORT Discus (1977 to 1997), and MEDLINE (1993 to 1997) were searched using the keywords "NSAIDs," "musculoskeletal," "acute," "sprain," and ";strain." DATA SYNTHESIS: NSAIDs exhibit anti-inflammatory effects via prostaglandin inhibition, neutrophil migration suppression, and oxygen free-radical inhibition. Retardation of inflammatory processes after acute injury may limit the area of secondary tissue damage but may also retard healing. Animal models have demonstrated short-term benefits with NSAIDs after acute injury, along with long-term adverse effects on tissue structure and function. NSAIDs have exhibited few benefits in the treatment of delayed-onset muscle soreness. Clinical trials of NSAIDs in the treatment of acute soft tissue injuries have shown conflicting results and have been highly criticized. CONCLUSIONS/RECOMMENDATIONS: Based on the research literature, the short-term benefits of NSAIDs in the treatment of acute soft tissue injuries must be weighed against the potential long-term adverse effects on tissue healing, structure, and function.

On the specific pattern of long chain polyprenols in green needles of Pinus mugo Turra.

In green needles of Pinus mugo the most abundant polyprenols occur as a mixture of prenologues in which the dominant alcohol is built of 16 isoprene units. The characteristic spectrum of polyprenols (prenol-15, -16 and -17) was the same irrespective of the location of plant and of distinct morphological differences observed in the various selected forms of this species. The constant pattern of the polyprenols spectrum was preserved throughout the 2-year life span of needles, although the level of polyprenols was increased 2-3-fold. The polyprenol pattern in Pinaceae family differs from species to species, thus it may serve as chemotaxonomic criterion within this systematic group.

The occurrence of long-chain polyprenols in leaves of plants of Combretaceae family.

The presence of poly-cis-prenols of chain length 20-60 isoprene units or longer in leaves of plants belonging to Combretaceae family was shown to be a common feature in this group of plants. The polyprenols of this type were found in half of the 20 species studied. In most cases the polyprenols occurred in the form of fatty acid esters. Only in one species--Combretum molle, the polyprenols were found in the form of free alcohols. The amount of long-chain polyprenols varied with the plant species; the richest source was C. molle (about 4% of dry mass of leaves). Polyprenol groups characteristic of other systematic families of plants were not found in the Combretaceae studied.

Eosinophilia and cellular cytokine responsiveness in hemodialysis patients.

Eosinophilia in hemodialysis (HD) patients has been associated with allergy to dialyzers and exaggerated activation of complement during HD. Its etiology, however, remains unknown. Complement activation can lead to cytokine production, and interleukin-2 (IL-2) administration has been shown to cause eosinophilia. Because abnormalities in cellular cytokine production in renal patients were previously demonstrated, the relationship between dialysis-associated eosinophilia and IL production in this HD population was studied. Twelve patients on chronic HD therapy with normal eosinophil counts (mean, 0.23 +/- 0.03 cells/nL) were compared with nine patients with eosinophilia (mean, 0.85 +/- 0.17 cells/nL). Measurements of cellular IL-1 and IL-2 production were performed before (pre) and after (post) HD with cuprammonium dialyzers. In patients with eosinophilia, stimulated cellular IL-1 production increased by 117 +/- 40% (P < 0.01) when post-HD measurements were compared with pre-HD values and IL-2 production increased by 127 +/- 65% (P < 0.05). In contrast, there was no difference in stimulated cellular cytokine production when values before and after HD were compared in patients without eosinophilia. Individual responses were reproducible during subsequent dialysis. It was concluded that cellular cytokine production in response to HD is not uniform. Eosinophilia is a clinically useful marker of exaggerated HD-associated cytokine production. Cytokine production depends on individual responsiveness and is probably related to atopy.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of maternal thyroid autoantibodies and post-partum thyroiditis on the fetus and neonate.

Thirty-five pregnant women with thyroid antibodies were followed during pregnancy and 12 month post partum. Twenty antibody-negative women served as controls and none of these developed postpartum thyroiditis in contrast to 12 of 35 antibody-positive women. Umbilical cord blood was collected at birth for measurements of thyroid antibodies, and blood samples for measurements of thyroid hormones were obtained at 5 and 30 days of age in the infants together with a clinical examination. There were no differences between the infants of antibody-positive and -negative mothers or the infants of those who developed postpartum thyroiditis with regard to gestational age at birth, birth weight, birth length or Apgar score, and no difference in thyroid function of the mothers and infants. However, serum TSH level was significantly higher in mothers who subsequently developed postpartum thyroiditis. Furthermore, a significantly lower growth during the first 30 days of life was observed in their infants. Our observations suggest a relationship between the maternal thyroid status in pregnancy and early growth in infancy.

Allergy to dialysis materials.

One hundred and six unselected patients were screened for allergic symptoms, specific IgE against ethylene oxide (ETO), isocyanates (ISO), formaldehyde (FA), phthalates (PHT), total IgE and eosinophil count. Complement activation was measured during cellulosic dialysis in atopic patients and in a control group. Sixteen patients demonstrated mild allergic symptoms during dialysis treatment. Ten of them had IgE elevation and eosinophilia. Eight of these patients had positive RASTs (ETO: n = 5, ETO-ISO(?)-FA: n = 2, ISO-PHT: n = 1) against dialysis material. All eight had an eosinophilia and seven showed an IgE elevation. An amelioration of symptoms could be obtained in three patients with elevated (greater than 15%) ETO-binding values after switching to ETO-free dialysers; avoiding PHT- and ISO-containing dialysis materials allergic symptoms remained constant. Cuprammonium rayon-induced complement activation had a more rapid onset and was more pronounced in atopic patients. The study confirms the role of ETO, but not of the other dialysis materials in the allergic sensitisation of haemodialysis patients.

Warm tub bath during labor. Effects on plasma catecholamine and beta-endorphin-like immunoreactivity concentrations in the infants at birth.

In this study 13 women bathed in a warm tub bath during the first stage labor after a strictly normal pregnancy. The control group consisted of 9 women who did not want warm tub bath. Apgar scores at 1 and 5 minutes were recorded and plasma adrenalin, noradrenalin were measured in umbilical arterial blood at birth, together with pH, pCO2 and base excess and beta-endorphin-like immunoreactivity in umbilical venous blood. No differences were found between the groups. No beneficial effect was found on infant condition of a warm tub bath during labor.

Warm tub bath during delivery.

In a prospective study, 88 women bathed in a warm tub bath for 1/2-2 hours during first stage labor after a strictly normal pregnancy, ending with spontaneous onset of labor at term. A control group consisted of 72 women fulfilling the same criteria of normality during pregnancy and labor, but who did not want to take a warm tub bath during labor. Apart from the bath, the two groups followed the usual obstetric procedures of the Department. The cervical dilatation in the "bath group" was 2 1/2 cm/h compared with 1 1/4 cm/h in the "control group". Mean pain score in the bath group was higher at the start of the study, before the bath, and they experienced a pain relief during bath which was not observed in the control group. This difference may be due to bias. The use of morfica and the need for stimulation of labor contractions were both twice as high in the control group, but this difference was not significant. No differences were observed with regard to operative delivery, vaginal or perineal laceration or in bleeding during labor or postpartum. The total duration of labor was the same in the two groups and no differences in neonatal condition were observed. The bacterial contamination of the bath water was insignificant and there were no significant febrile episodes post partum.

Polyacrylonitrile complement activation attenuated by simultaneous ultrafiltration and adsorption.

The highly permeable synthetic polyacrylonitrile (PAN) membrane (Filtral; Hospal, Basle) is regarded as biocompatible for its slight complement activation and leucocyte sequestration. The low C3a and C5a concentrations during PAN dialysis may be due to a lack of complement activation potential of this polymer, but also to elimination of activated complement components by the dialyser through adsorption and/or ultrafiltration (mol wt of C3a and C4a 9000 daltons; of C5a 11,000 daltons). Comparing arteriovenous differences throughout the study, higher concentrations of C3a (+23%; n.s.) and C5a (+80%; P less than 0.05) were measured in the efferent blood lines, suggesting a slight complement activation by the alternate pathway. A significantly lower C4a concentration in the efferent blood lines (-30%, P less than 0.05) indicates an elimination within the dialyser. Significantly higher arteriovenous concentration differences at the beginning of dialysis and increasing sieving coefficients suggest elimination by adsorption, mainly in the first 20 min of haemodialysis. The continuous decrease of C3a and C4a plasma concentrations in the afferent blood line suggests transport across the membrane and removal by the dialysate. Accordingly, measurable amounts of these complement components were detected in ultrafiltrate.

Intrapartum fetal heart rate monitoring in a combined low- and high-risk population: a controlled clinical trial.

In a prospective clinical randomized investigation 487 women had the condition of the fetus during labour supervised by means of stethoscope (AUS), while 482 women went through labour under surveillance of electronic fetal monitoring, cardiotocography (EFM). 349 women refused to participate in the investigation (NAI) and had delivery conducted according to the normal procedures of the department (70% AUS, 30% EFM). Significantly more pathological fetal heart rate patterns (FHR) were found in the EFM group compared to the AUS group in both the first and the second stage of labour. As a result significantly more vacuum extractions were performed in the EFM group than in the AUS group, while no statistical difference was found between the groups in the incidence of acute cesarean sections carried out for asphyxia. One case of intrapartum death occurred in the AUS group. No differences were found in Apgar scores after 1 and 5 min or in neonatal morbidity at examination on the 2nd and 5th days after delivery. A tendency towards more biochemically compromised children was found in the AUS group. The specificity for both methods was found to be acceptably high (80%), while the predictive value for both methods was low (50%). More research is therefore urgently needed to evaluate supplementary investigations and parameters for the evaluation of the intrapartum fetal condition.

Permeability and secondary membrane formation of a high flux polysulfone hemofilter.

It has been assumed that the molecular weight (MW) cut-off of a newly fabricated polysulfone capillary dialyzer (F60, Fresenius, FRG) is similar to that of the human glomerulus. We recently tested the device in vivo and found this not to be so, based on the device's ability to eliminate substances of a MW of 10,000 to 60,000 daltons. One of the reasons for this discrepancy was found to be the influence of secondary membrane formation on solute permeability. Endogenous marker substances of a defined MW (beta 2-microglobulin, myoglobin, RBP, alpha 1-microglobulin, acid alpha 1-glycoprotein, alpha 1-antitrypsin, prealbumin, and albumin were measured by laser nephelometry or radioimmune assay; sieving coefficients (SC) and protein eliminations were calculated for each low MW protein.

Metabolic adaptations during the neonatal period in infants of diabetic mothers.

Infants of diabetic mothers (IDM's) are prone to develop severe but usually asymptomatic hypoglycaemia during the first postnatal hours. High maternal plasma glucose concentrations at the time of delivery increase this risk, especially if the mode of delivery is cesarean section. Newborn IDM's are hyperinsulinaemic and the neonatal hypoglycaemia seems to result from this together with a retarded increase in plasma glucagon levels. Despite very high plasma insulin concentrations, lipolysis, as indicated by a rise in plasma glycerol concentrations, appears to be normal in IDM's. The IDM's are able to respond to both physiologic and metabolic stress with an increased catecholamine secretion during the first hours of life, and the catecholamines seem to counteract the inhibitory effect of insulin on lipolysis and, at least partly, to oppose the blood glucose lowering effect of insulin.
