Acute myopathy in a type 2 diabetic patient on combination therapy with metformin, fenofibrate and rosiglitazone.

AIMS/HYPOTHESIS: This report describes the case of a 75-year-old male type 2 diabetic Caucasian who was admitted to the clinical ward because of acute pain and cramps in both calf muscles. MATERIALS AND METHODS: Neuromuscular function was assessed by electromyography and electroneurography of the right leg. An open biopsy was taken from the left vastus lateralis muscle for histological and histochemical analyses. Southern blotting was performed to detect defects in mitochondrial DNA and tRNA. Cytochrome P450 (CYP-P450) polymorphisms were analysed in blood cells. RESULTS: Fifteen weeks before admission, the patient's lipid-lowering medication was switched from simvastatin to fenofibrate because of predominant hypertriglyceridaemia; this did not affect creatine kinase levels. Three weeks before admission, rosiglitazone was added to his existing metformin therapy because of worsening metabolic control. Upon admission, serum enzymes indicating myopathy were elevated (creatine kinase 6897 U/l, myoglobin 902 ng/ml) and kidney function was impaired (creatinine 0.116 mmol/l, blood urea nitrogen 2.3 mmol/l). Electrophysiology revealed myopathy and sensory polyneuropathy. Histology showed multiple damage of the myofibrillar architecture. There was no evidence of defects in mitochondrial DNA or tRNA. Furthermore, no functional limitations in CYP2C9, CYP2C19 and CYP2D6 were detected. Following withdrawal of the oral medication and intravenous hydration, clinical symptoms and laboratory parameters gradually decreased. CONCLUSIONS/INTERPRETATION: Until more data from controlled trials are available, we recommend that combination therapy with fibrates and thiazolidinediones should be monitored frequently by measurements of serum creatine kinase and creatinine, specifically in patients with pre-existing nephropathy and polyneuropathy.

[Treatment of diabetic patients with Optisulin-Depot CR in comparison with Depot-Insulin Hoechst klar CR--therapeutic progress?].

During a cross-over-study in 12 ambulant patients the effects of Optisulin -Depot CR in comparison to Depot-Insulin Hoechst klar CR were examined. After a period with Depot-Insulin Hoechst klar CR patients got Optisulin -Depot CR in the same dose and with the same diet. A week later patients took Depot-Insulin Hoechst klar CR once again. The different blood-sugar-curves corresponded to each other, but Optisulin -Depot CR showed more initial effect. The use of Depot-Insulin Hoechst klar CR is an advantage, because, Optisulin -Depot CR is neutral and no depot-substances are contained. Therefore, less allergic reactions are expected.

[The differential diagnosis of chronic bronchitis and emphysema from mutually supplemental functional and radiographic criteria].

In a retrospective study in 91 patients an attempt was made to differentiate the primary and/or predominating disorder "emphysema" or/and "chronic bronchitis" by independent evaluation of a number of X-ray signs and lung function criteria in routine diagnosis. Good congruency of the diagnosis from these two different aspects, could be established. Patients with 3 or more X-ray signs of emphysema were found to have larger lung volume and smaller diffusing capacities than those without. Vice versa, patients with functional diagnosis of predominant emphysema more often showed the full X-ray pattern of emphysema, than those with predominant "chronic bronchitis". In either method of diagnostic procedure, the occurence of the "small heart of emphysema" was obviously much more frequent in patients believed to have predominant or primary emphysema. Assessment of X-ray signs of changes of the lesser circulation can be obtained in a stage of the disease, when structural changes have become irreversible. Considering the different pathophysiologic pattern of predominant panacinar emphysema, the destruction of alveolar walls and, consequently of the capillary bed, is more likely to reduce DCO as well as cardiac output before pulmonary hypertension can develop. The X-ray signs resulting from this effect on the lesser circulation, exist in widened right hilar branch and abrupt narrowing of peripheral vessels, with a small heart and lack of prominence of the pulmonary trunk. In chronic bronchitis, however, signs of pulmonary hypertension are expected to occur early in the disease. The relevance of the X-ray changes of the heart size and the pulmonary circulation in both types of lung disease, could be confirmed by lung function data.

[X-ray documentation of the anamnesis and physical data in patients with rheumatism within the scope of the medical information system WAMIS].

A computerized on-line system of evaluating of symptoms and physical data in rheumatology is presented. Case history, previous treatment, and physical examination are documented. The documentation of the latter may be performed in two ways: one records only joint symptoms, the other includes joint measurements. In comparison to the former off-line method, all data are much better available. Moreover, we believe it to be considerably timesaving. This system should also allow an electronic differential diagnosis of rheumatic diseases, after completing laboratry and X-ray documentation.
