Mixed germ cell-sex cord stromal tumor with heterologous structures.

A mixed germ cell-sex cord stromal tumor developed in the right ovary of a 4-year-old girl. The patient's cells contained sex chromatin and the karyotype was 46,XX. Clinically, she exhibited mild isosexual pseudopuberty. Some of the tumor cells showed differentiation to Sertoli cells, whereas others had characteristics of germ cells. The neoplasm was composed of solid cords consisting of a mixture of the two tumor cell types and surrounded by a delicate connective-tissue network. In one small area, the tumor contained heterologous development in the form of glands and cysts lined by columnar mucinous epithelium containing numerous goblet cells and occasional argyrophilic neuroendocrine cells. Normal ovarian tissue was present at the periphery of the tumor. Electron microscopic study confirmed the presence of both Sertoli-like and germ cell-type cells. Immunohistochemical studies demonstrated vimentin positivity in both cell types (with preference for the Sertoli-like cells) and cytokeratin positivity in the Sertoli-like cells. The patient was symptom free 4 years after right oophorectomy, radiotherapy, and chemotherapy.

[An ovarian tumor with structural gonadoblastoma, dysgerminoma and choriocarcinoma].

A 14 1/2-year old girl with menometrorrhagia followed by amenorrhea and abdominal pain had a pelvic resistance with limited mobility. Histology of a left ovarian tumour showed gonadoblastoma turning to dysgerminoma and associated with choriocarcinoma (M-9073/1, M-9060/3, M-90101/3). Genuine ovarian tissue was hypoplastic on both sides. In the right ovary, there were additional thecalutein cysts. The patient's karyotype was 46 XX. She has been without any relapse 2 1/2 years after chemotherapy and actinotherapy.

[The effect of thrombocytapheresis on hematologic, biochemical and immunologic indicators].

The authors investigated in 33 separate donors the influence of thrombocytapheresis on haematological, biochemical and some immunological parameters. The paired t-test revealed a statistically significant increase of values of total proteins, aspartate aminotransferase, immunoglobulin A, alpha 1 antitrypsin, transferrin and the C4 component of complement seven days after apheresis and a significant increase of thrombocyte, total protein, alkaline phosphatase, immunoglobulin A, orosomucoid, and transferrin levels two weeks after apheresis. A significant decrease of haemoglobin values was recorded 7 and 14 days after apheresis. As the revealed changes of the investigated parameters are within the physiological range of the healthy adult population, the authors conclude that thrombocytapheresis does not have an adverse effect on the health status of donors.

[Gonadal dysgenesis].

The authors describe six patients, female phenotype with karyotype 45X/46XY. Two girls suffered from pure gonadal dysgenesis, four from mixed gonadal dysgenesis. The authors emphasize the intact axis hypothalamus-pituitary gonads. In the above diagnoses the authors recommend revision and extirpation of the gonads with regard to possible malignant growth in the dysgenetic gonad.

[Hormonal contraception and levels of selected serum proteins].

The authors investigated 116 women who took hormonal contraceptives. All women were subjected to detailed clinical and biochemical examinations. In the serum a number of biochemical and immunological tests was made. Special attention was paid to the estimation of prealbumin, alpha-1-antitrypsin, alpha-2-macroglobulin, orosomucoid and ceruloplasmin. In prealbumin 23 values above normal (from a total of 83 women) were found, in alpha-1-antitrypsin 55 values below normal and 13 values above normal (from 114 women) in alpha-2-macroglobulin 18 values below normal and 3 values above normal (from 67 women) and in orosomucoid 26 values below normal and 2 values above normal (from 116 women) and in the case of ceruloplasmin 2 values below normal and 51 values above normal (from 116 women). The authors discuss the influence of hormonal contraception on the levels of the investigated proteins and on possible subsequent metabolic changes and pathological conditions.

[Juvenile granulosa cell tumor].

Granulosa cell juvenile tumours, defined in 1976 by Scully as a separate unit are found mostly in girls during the first two decades of life. On inspection under an optic microscope they are characterized by the presence of non-differentiated "blastemoid" structures and the formation of solid follicular formations and cysts. In our first case we observed a bilateral juvenile tumour made up of granulosa cells in a 8-month-old girl where the tumour produced symptoms of pseudopubertas praecox isosexualis. In the second case in a 9-month-old girl also with symptoms of pseudopubertas praecox isosexualis the authors detected in the left ovary a tumour which probably is a less differentiated variety of the juvenile granulosa cell tumour. These tumours belong into the group of little differentiated gonadal-stromal tumours with ICD-O code M-8590/1.

[Detection of antibodies and assessment of an international unit of anti-HBs specific immunoglobulin, HEPAGA].

The authors submit information on the presence and titre of antibodies against antigens of the hepatitis B virus [HB] and the Czechoslovak specific anti-HBs immunoglobulin, HEPAGA, and on the presence of the delta-antibody. The presence of the antibody against the surface antigen of the HB virus [HBsAb, anti-HBs] was examined in 32 batches, the antibody against the nucleus of the HB virus [HBcAb] and e-antigen [HBeAb] in a total of 27 batches. HBsAb was positive in the majority of cases still when diluted 1/2,500,000; HBcAb when diluted in the range from 1/25,000 to 1/2,500,000; the positivity of HBcAb ended with the exception of one batch at the dilution of 1/250. The delta antibody was detected only in batches from 1982 and 1983, in a maximum dilution of 1/8. All examinations were made by the radioimmunoassay [RIA] technique. The authors give also an account of the assessment of the international HEPAGA [IU/ml]. The last batch has 170 IU/ml.

Hepatitis Antibodies

[Synthetic vaccines for immunization against infection with hepatitis B virus].

The paper informs on the present state of preparation of a vaccine against infection with the hepatitis B virus, using techniques of genetic engineering. Synthesis of immunogenic peptides derived from the structure of HBaAg amino acids stimulated the administration of several oligopeptides derived from the structure of HBsAg. Experiments with the decapeptide derived from the S-portion of HBsAg revealed that also a small peptide, when administered with adjuvants, is immunogenic, Synthesis of HBsAg protein in a culture of Saccharomyces cerevisiae was already tested clinically and proved to be an effective vaccine. It will be probably also possible to prepare the vaccine by formation of the anti-idiotype, induced by HBsAg anti Ig. The authors' own observation revealed that the administration of anti HBsAg immunoglobulin (Hepaga) causes in rabbits the formation of a component which reacts in serum by the RIA technique as HBsAg.

Circulating immune complexes in acute leukemia.

Circulating immune complexes (CIC) were measured at the time of diagnosis in 81 patients with acute leukemia or blast crisis of chronic myeloid leukemia using precipitation by 3.75% polyethyleneglycol. Elevated CIC levels did not adversely influence complete remission duration and survival, patients with normal CIC levels exhibited mostly shorter remission and survival than those with elevated or borderline levels. No significant correlation was observed between CIC levels and Hb, WBC, CBC, platelet count, age, serum bilirubin, total protein, fibrinogen, AST and ALT levels, presence of hepatosplenomegaly and/or lymphadenopathy, HbSAg positivity, complete remission duration and survival. The lack of correlation may be caused by altered immune response in leukemic patients, but the obtained results may also be affected by the nonspecific nature of the method used for the detection. Simultaneous detection of CIC levels by multiple tests and evaluation not only of the number but also of the composition and size of CIC may decrease the incidence of false results. Nevertheless, only the establishment of antigen-specific assays may resolve the controversies in the detection of CIC and thus contribute to a more precise assessment of the role of CIC in prognosis of cancer, as well as to the verification of reliability of using CIC as a tumor marker.
