[Trabeculectomy with releasable sutures: initial experience].

The authors present an account on their first results of trabeculectomy with releasable sutures, as described by Osher. The intraocular pressure before operation was on average 21.7 mm Hg (23-40 mm Hg). During the last visit (on average 2.8 months after operation) it was 13.5 mm Hg (13-15 mm Hg), incl. six eyes without treatment. Only in two eyes during the early postoperative period a shallower anterior chamber was observed. The authors recommend more extensive use of the technique of releasable suture in trabeculectomy.

[The Molteno implant].

The authors describe their experience with Molteno implants in 21 patients. The most frequent types of glaucoma were congenital glaucoma--5 times--and glaucoma associated with pseudophakia--5 times. Before the implant as many as four operations of one eye were performed, in the whole group with Molteno implants 28 operations had preceded, mostly trabeculectomies and cyclocryocoagulations. As to postoperative complications, expulsive haemorrhage on the first night after operation was most serious. The follow-up period is 1-15 months, on average 7.7 months. The intraocular pressure before operation was on average 35.8 mm Hg, on discharge 9.7 mm Hg and during the last check-up examination (19 eyes) 19.1 mm Hg. It was under 21 mm Hg in 10 eyes, i.e. 52.6%, during the last check-up examination. Re-operations were made five times (26.3%). As compared with values before surgery, vision deteriorated in three eyes, incl. two because of complications of diabetes. The Molteno implant is an effective procedure in the treatment of complicated and refractory cases of glaucoma in high-risk eyes.

[ERG findings in structured conditions (PERG) in the diagnosis of glaucoma].

In 34 patients admitted to the clinical examination on the basis of the finding of the higher intraocular pressure stated in the private praxis the PERG examination has been performed. After the clinical examination (intraocular pressure, Humphrey perimeter, gonioskopy, C/D ratio) two groups of patients have been classified: 1. Glaucoma patients (21 persons) 2. Patients without glaucoma (13 persons). In the first group a significant lowering of the b65 amplitude of the PERG has been found. Thus the diagnostic value of the PERG in early glaucoma has been confirmed.

[Neovascular glaucoma with ocular ischemic syndrome].

Three cases of the neovascular glaucoma with signs of the chronic ocular ischemia in the occlusive disease of the arteria carotis (communis or interna) document the unfavorable prognosis of the disease. None of the patients was submitted to the carotis surgery. The visual acuity of the first patient decreased to 2/60. The eye of the second patient became blind and the third patient died after the second stroke.

[Quadrant cyclocryocoagulation. Initial experience].

The authors describe a modification of cycloccryocoagulation /CCC/ they used in the treatment of different types of glaucoma in a group of 320 eyes. Cryodestruction of the ciliary body was implemented with a probe at a temperature of -60 degrees C for a period of 2 x 20sec. in two points of opposite qaudrants, using a transconjunctival approach. The probe was applied at a distance of 3 mm from the limbus. Following a period of 12 months to 3 years after operation they investigated the intraocular pressure and visual acuity of the treated eyes. They recorded a hypotonic effect comparable to cryocoagulation performed by the classical procedure; the incidence of complications was lower.

[Long-term experiences with scleral cauterization in anti-glaucoma surgery].

The authors evaluate their experience with basal iridectomy and scleral cauterization on 117 eyes. On 80 eyes the operation was carried out according to the classic method of Scheie, on 37 eyes with a modification using the scleral flap. Intraocular pressure was compensated in 80% of the patients on whom the classic method was used, including the administration of miotica and in 91.8% of those who underwent the modified procedure with scleral flap. Special Consideration is given to the problem of lens opacification in the postoperative course. No postoperative infections were encountered.
