[Influence of age in mortality of patients with heart failure].

OBJECTIVE: Heart failure (HF) is one of the most frequent diagnosis in Internal Medicine Units. High age of patients is an important characteristic in these admissions. Our objective was to evaluate how age interact with mortality, to medium-term, in HF after a decompensated episode. METHOD: Through the patient medical record we obtained data about patients hospitalised in a Internal Medicine Unit, with HF diagnosis, for a year (from September 2000 to August 2001) and their survival at 1st January 2003. 215 patients were reviewed. RESULTS: During follow-up, 60 patients died (27.9% of overall). In elderly patients, we observed a higher number of women and less use of echocardiography and treatment with beta-blockers and warfarin (in patients with atrial fibrillation). In Cox proportional-hazards model, age (OR 1.043 IC 95% 1.002-1.085), days of hospitalization (OR 1.04 IC 95% 1.003-1.078), diabetes mellitus (OR 2.51 IC 95% 1.37-4.60) and do not prescribe warfarin in patients with atrial fibrillation (OR 2.71 IC 95% 1.10-6.60) were independent prognostic factors. CONCLUSIONS: Age was an important prognostic factor in patients with HF. Clinical trials should be done in patients with these characteristics. So, we can know better clinical evolution of HF in this population.

[Acute transverse myelopathy associated to S. aureus: a difficult differential diagnosis].

Acute myelopathy includes a group of diseases with an important associated morbidity, thus early diagnosis and treatment is important. The most frequent etiology is extramedullary compression. Magnetic resonance (MR) is the most suitable procedure in this type of disease since it also offers information on extramedullar lesions and makes it possible to assess spine involvement. We present the case of a 57 year-old man who was admitted because of back pain for several weeks and systemic infection due to S. aureus. Later, he suffered a sudden neurological deficit with spine involvement but without compressive images on the MRI. We discuss the differential diagnosis among intramedullary abscess, epidural abscess, acute transverse myelitis and spondylodiscitis associated to acute myelopathy. We carry out a brief review of the medical literature on diagnosis criteria of those entities.

[Fluctuant pulmonary nodules as presentation of a MALT lymphoma].

Mucosa associated lymphoid tissue (MALT) lymphomas are a group of non- Hodgkin"s lymphomas of low malignancy degree. The most frequent location is the gastrointestinal tract. Its primary pulmonary presentation is unusual and heterogeneous from point of view radiological. Woman 61 years old with antecedents of vitiligo, gastric ulcus, cirrhosis by VHC, that go into the hospital by sudden disnea, thoracic paint with pleural characterises and fever of 38.5 degrees C, Her thorax radiography and thoracic TAC showed nodes that affect to different pulmonary lobes. The cytology by PAAF confirms their malignant nature. In subsequent radiological controls it was notice the nodels took away completely and returns in different pulmonary place in each recurrence. The presentation like fluctuant pulmonary nodes is exceptional in a MALT lymphoma. It was described a higher incidence of VHC infection and tumour. The evidence of chronic hepatitis by virus C disease, and local chronic inflammatory process as well as autoimmune disorders may be considerate like a factor that contribute to MALT lymphoma.

[Quality study of diagnostic and follow-up procedures of bone involvement in type 1 Gaucher"s disease in Spain].

BACKGROUND: Bone involvement in type 1 Gaucher"s disease can be devastating and is oftenly silent. Bone MR is generally recommended. The aim of the study was to elucidate whether the size and location of hospitals in Spain implies any difference in management of GD patients. MATERIAL AND METHODS: We surveyed the type of facilities in the hospital (namely MRI) as well as for the presence, type, severity and methodology of follow-up of bone involvement associated to GD, according to the category of hospital (local or reference). RESULTS: 31 patients were followed in reference hospitals whereas 16 other were in local hospitals. 70% of cases had some type of bone involvement, 60% had severe bone disease. MRI was the first choice for diagnosis in 65% and for follow-up in 93% of cases. MRI is less indicated among patients from local hospitals. Chitotriosidase is measured in a high, but insufficient, proportion of the followed patients (60%). CONCLUSIONS: The Spanish hospital network, either reference or local hospitals, have an adequate infrastructure for the management of GD patients. However, main diagnostic resources are being currently underused.

[Transforming growth factor beta (TGF-beta) in Gaucher's disease. Preliminary results in a group of patients and their carrier and non-carrier relatives].

BACKGROUND: The poor phenotype/genotype correlation in Gaucher's disease makes difficult therapy-decision-making and prevention of complications. Gaucher's cells and tissue fibrosis are the earliest findings of the disease. Transforming growth factor ss (TGF-beta1) is the key cytokine involved in the regulation of tissular scarring and fibrosis. The aim of the study was to ascertain if there are differences in plasma TGF-beta1 between Gaucher's disease patients, carriers and non-carriers healthy people and whether there is any correlation between plasma TGF-beta1 and clinical phenotype among patients. PATIENTS AND METHOD: Plasma TGF-beta1 was measured in 11 patients with Gaucher's disease, 12 carriers and 10 healthy people. Patients were further evaluated to know their liver and spleen size, bone involvement, hemoglobin, leukocyte and platelet count and the Zimran's severity score index (SSI). Plasma concentration of TGF-beta1 was determined by RIA phenotypic sandwich antibodies assay and quantified by a colorimetric procedure. Sensitivity was 25 pg/ml and specificity (cross reactivity) < 5% with beta2-TGF and beta3-TGF. STATISTICS: ANOVA and T-test were applied for mean comparisons and subgroup analyses. RESULTS: Plasma TGF-beta1 values were increased in Gaucher's disease patients (98.4 [91.4] pg/ml) over carriers (47.2 [21,7] pg/ml; p = 0.04) and healthy relatives (40.8 [9.8] pg/ml; p = 0.02). No differences in patients subgroups, with regard to SSI or bone involvement, were observed. CONCLUSIONS: Plasma TGF-beta1 levels are increased in this group of patients with Gaucher's disease. Since there is no correlation between the plasma values and the phenotypic expression, TGF-beta1 could merely be a marker of macrophage activation.

[Nephrotic syndrome associated with symmetrical multiple lipomatosis (Madelung's disease)].

We present a case of multiple symmetric lipomatosis Type I (Madelung's disease) with severe organic affection, hepatic cirrhosis, sever sensitive polyneuropathy and neuropathic ulceration at the left lower limb. A nephrotic syndrome developed in a larval form due to proliferative glomerulonephritis as the result of a metainfectious complications of the infection at the lower limb. We discuss the etiopathogenicity of the organic affection and we highlight the pathogenic links between the disease and its complications.

[Multiple symmetric lipomatosis].

Multiple Symmetric Lipomatosis (MSL or Madelung's Disease) is a rare entity characterized by the presence of non-capsulated lipomas mainly located at the neck and upper part of the thorax and frequently associated to chronic hepatopathy, neuropathy and alcoholic habit. We review and update the clinical, etiopathogenic, diagnostic and therapeutical aspects of this rare entity. We stress the presence of neuropathy, a typical feature of the disease, which has also a predictive value.

[Medulloblastoma in the adult: presentation of a new case].

A case of hemispheric medulloblastoma of cerebellum in a 22 male patient is presented. The patient complained of headache and an intracranial hypertension syndrome progressively appeared. Diagnosis was established by CT scan. Treatment consisted of radical surgery associated to radiotherapy and chemotherapy. Present survival with this treatment is 4 years being the patient asymptomatic and without signs of relapse.

[Persistent polyadenopathic syndrome secondary to cytomegalovirus infection in an immunocompetent host].

A case of polyadenopathic syndrome is presented which involved practically all the ganglionar territories and hepatosplenomegaly in a 15 year old male without a relevant previous history, non immunedepressed, without drug addiction or having received blood er blood derivatives. The clinical picture caused by a cytomegalovirus infection has persisted for 24 months with no modification of adenopathies. The inclusion of CMV infections in the differential diagnosis of generalized adenopathic syndromes is proposed.

[Mixed cryoglobulinemia secondary to Mediterranean boutonneuse fever].

A clinical case of Mediterranean Boutonneuse Fever (MBF) with circulating cryoglobulins during the acute phase, with no clinical symptoms is described and considered an epiphenomenon of the infection. The possible relationship between the infection and the cryoglobulinemia are discussed. The patient is also added to the increasing list of patients reported of Boutonneuse Fever in the Mediterranean area during the observation period.

[An endobronchial mass as the presentation of tuberculosis. A report of 3 cases and review of the literature].

3 cases of endobronchial TB are described. They were diagnosed as endobronchial mass which deviated the diagnosis to another pathology in absence of sputum culture. The TB treatment produced a complete resolution of the lesions. We highlight the endobronchial mass as a non-frequent clinical feature of EBTB and we should bear this etiology in mind in differential diagnosis of all endobronchial masses even if the clinical and endoscopic data are of bronchial tumor.
