[Use of thrombolytic treatment in patients with acute myocardial infarction in Spain. Observations from the PRIAMHO study].

INTRODUCTION AND OBJECTIVES: Scarce information is actually available in our country regarding the use of thrombolytic treatment in patients with acute myocardial infarction and how consistently the recommendations of the clinical guidelines are being implemented. METHODS: Cohort study with one year follow-up of patients with acute myocardial infarction admitted in 24 Spanish hospitals in 1995. Differences in clinical characteristics and prognosis from patients treated with or without thrombolysis were compared. RESULTS: 2,191 of the 5,242 patients (42%) admitted with an acute myocardial infarction received thrombolytic therapy (range: 23%-63%). Reasons for exclusion in the rest were the absence of ST segment elevation (35%), contraindications (16%), prehospital delay >12 h (35%), and other causes (15%). Thrombolysis treated patients were at lower risk in general because they had shorter prehospital delays and were younger, more likely to be male, less frequently diabetic, with less prior history of angina or infarction. The average delay in administering therapy was of 3 hours while the average in-hospital delay was 50 minutes and depended only on the hospital where patients where admitted, as it was shorter in small centers. t-PA was administered in 49% of patients, streptoquinase in 46% and other drugs in 5%. Although t-PA was given more often to younger patients, smokers, anterior and Q-wave infarctions, and to patients with shorter prehospital delays, the determinant factor was the admission hospital with a frequency ranging from 9% to 96%. Patients not treated with thrombolytics had more complications during the acute phase, and required more invasive procedures. They also had a higher mortality at 28 days (17% vs. 10%, p < 0.0001) and at one-year follow-up (27% vs. 15%, p < 0.0001). Furthermore, a correlation was observed between mortality and delay of treatment application. In multivariate analysis, thrombolytic treatment was an independent predictor of survival at one year, with an odds ratio for mortality of 0.8 (95% CI: 0.66-0.96). CONCLUSIONS: Thrombolytic therapy in Spain does not yet conform to the recommendations of the actual guidelines for the treatment of patients with acute myocardial infarction because it is underused, especially in high-risk patients, the prehospital and in-hospital delays are too long, and a huge variability exists between hospitals in the frequency and delays of administration and selection of the drug that are not sufficiently explained by the characteristics of the patients. In spite of this, mortality of treated patients was 20% lower in comparison to the non-treated patients, after adjusting for the other clinical factors with demonstrated prognostic value.

[The predictive signs of multivessel coronary disease in echocardiography with dobutamine].

AIMS: To assess the ability of dobutamine echocardiography to detect multivessel coronary artery disease and to determine predictive factors for multivessel disease with or without beta-blockers. PATIENTS AND METHODS: A total of 101 patients underwent dobutamine stress echocardiography and coronary angiography (evaluation of chest pain 76, extent of coronary disease after myocardial infarction 19, other indications 6). RESULTS: Ten patients in whom the test was prematurely terminated were excluded. Out of 91 patients who underwent dobutamine echocardiography, 54 patients had multivessel disease (sensitivity of dobutamine test 93%, specificity 46%). Heart rate at the maximum dose of dobutamine or atropine was 88 +/- 21 beats/min for multivessel diseases and 104 +/- 21 beats/min without multivessel disease (p < 0.001). A cut-off value < 94 beats/min discriminated patients at risk for multivessel disease. After adjusting for treatment with beta-blockers, heart rate < 94 beats/min, ECG signs of ischemia, and abnormalities on baseline echocardiogram with remote asynergies during dobutamine testing were independent predictors of multivessel disease in the multivariate analysis (probability > 90% when at least two factors were present). CONCLUSION: A heart rate < 94 beats/min at peak dose of dobutamine or after atropine, ECG signs of ischemia, and the presence of abnormalities on echocardiogram at rest with remote asynergies during dobutamine stress testing were independent predictive factors of multivessel coronary artery disease.

High-dose systemic interleukin-2 therapy in stage IV neuroblastoma for one year after autologous bone marrow transplantation: pilot study.

Despite intensified chemotherapy protocols, including autologous bone marrow transplantation (ABMT), stage IV neuroblastoma has a poor prognosis, and modern therapeutic trends are aimed at the eradication of minimal residual disease, which is though to be the main factor leading to relapse. In this pilot study, we report the systemic administration of high doses of interleukin-2 after ABMT in four patients. Five day cycles of IL-2 at a dose of 18 x 10(6) IU/m2/day were administered at variable time intervals as frequent as it was necessary to maintain the levels of natural killer (NK) cytotoxic activity higher than the median control value (40 LU/ml blood) throughout 1 year from the start of first IL-2 treatment. After IL-2 infusion, NK and LAK activities increased significantly (median 742 x 10(-3) LU/ml blood and 186.8 x 10(-3) LU/ml blood, respectively). Toxicities were transient and no life-threatening complications were observed. Fever, anorexia, skin rash and enlarged liver were always present. Anaemia, thrombocytopenia, leukocytosis, lymphocytosis and and eosinophilia occurred following most of the IL-2 courses. Although the small number of patients does not allow an estimation of the immunomodulatory-antineoplasic effects of IL-2, the results seem promising for the management of neuroblastoma patients.

[Genitourinary rhabdomyosarcoma in childhood].

We present the result obtained by GICOP in the treatment of 12 genitourinary rhabdomyosarcoma, located in vagina-uterus (5), bladder-prostate (4) and paratesticular (3). The age of the patients ranging from 1 to 15 years. Embryonal rhabdomyosarcoma was the most common histologic type (58 for 100) being the major number of cases in stadium II. The treatment was based in the combination of chemotherapy protocolised with vincristine, d-actinomycin, cyclophosphamide (VAC), vincristine, adriamycin (VAC-VAD) and/or iphosphamide, vincristine, d-actinomycin (IVA), use of radiotherapy, marrow auto-transplantation and radical surgery avoidance. Complete remission was achieved in 11 out of 12 cases (92 for 100), relapsing in 5 cases (38 for 100) of which 4 (80 for 100) achieved a second remission. An 82 for 100 disease free survival was achieved in a 5 year period.

[Tumors of the CNS in childhood].

Brain tumors are the most common group of solid malignancies in childhood. The therapeutic arms available nowadays: surgery, radiotherapy and chemotherapy, have achieved considerable improvement in the survival of some types of CNS tumors, mainly due to the first two. We have carried out a retrospective study of 252 CNS tumors, diagnosed in the GICOP group during the last ten years. The primary location was: supratentorial, 42%; infratentorial, 51%; and both, 7%. Biopsies were performed in 190 cases (75%), resulting in the following histologic types: astrocytoma (38%), medulloblastoma (19%), ependymoma (14%), craniopharyngioma (7%), others (22%). The treatment was individualized according to the patient characteristics: age, location, histologic type and tumor spreading. Surgery was performed in 82% of patients (8% only derivative shunt), radiotherapy in 48% and chemotherapy in 23%. The life survival was 60% at 50 months.

[Rhabdomyosarcoma of the bile ducts].

Two children of two and three years old affected with biliary tract rhabdomyosarcoma are presented. In both cases the diagnostic was confirmed by anatomopathologic studies of the tumoural piece obtained by surgical extraction. Clinic evolution, surgical technic and cytostatic treatment are described been noticeable a favourable clinic development in both cases. Authors had made a bibliographic revision on the subject, pointing a minor incidence of the biliary tract in comparison with other tumour locations.
