Increased immunoreactivity and concentration of basic fibroblast growth factor in lansoprazole-treated gastric mucosa.

The purpose of this study was to clarify the effect of lansoprazole on basic fibroblast growth factor (bFGF) during the healing of gastric ulcers in comparison with famotidine and rebamipide. Alteration of the localization and concentration of bFGF was examined in gastric mucosa obtained endoscopically from ulcer patients. The bFGF content was estimated by enzyme-linked immunosorbent assay and the localization of bFGF immunoreactivity was determined by indirect immunohistochemical study using monoclonal antibodies. In the lansoprazole-treated cases, the content and the histochemical immunoreactive area of bFGF significantly increased 4 weeks after the treatment, compared with the group treated with rebamipide and the control group. In the famotidine-treated group, the concentration of bFGF significantly increased, compared with the rebamipide-treated group, and not with control. The number of binding sites for [125I]bFGF, as determined by in vitro autoradiography, also significantly increased in the lansoprazole-treated group. Therefore, an increase in bFGF concentration and receptor distribution was shown to be brought about by treatment with lansoprazole.

Tax protein of human T-cell leukemia virus type I binds to the ankyrin motifs of inhibitory factor kappa B and induces nuclear translocation of transcription factor NF-kappa B proteins for transcriptional activation.

Human T-cell leukemia virus type I causes adult T-cell leukemia and tropical spastic paraparesis, and its regulator protein Tax has been implicated in the pathogenic activity of human T-cell leukemia virus type I. Tax activates transcription of viral and cellular genes through specific enhancers: the 21-bp enhancer of human T-cell leukemia virus type I, the nuclear factor kappa B (NF-kappa B)-binding site of the interleukin 2 receptor alpha gene, and the serum-responsive element of c-fos. Tax binds to enhancer-binding proteins including cAMP-responsive element-binding protein, cAMP-responsive element modulator, transcription factor NF-kappa B p50 and p67SRF, and associates with each enhancer DNA indirectly. In addition to this mechanism, we report here that Tax binds to inhibitory factor kappa B gamma (I-kappa B) gamma, which forms a complex with NF-kappa B protein heterodimer p50-p65 or homodimer p50-p50 and retains them in the cytoplasm. Tax binding to I-kappa B gamma induces nuclear translocation of NF-kappa B p65. In association with this nuclear translocation of p65, transcription directed by the kappa B enhancer is strongly activated. Tax binds to the ankyrin motifs of I-kappa B gamma, suggesting its possible interaction with many other proteins carrying ankyrin motifs contributing to various regulatory processes. This is a different mechanism of transcriptional activation by the oncoprotein Tax and seems to be independent from the trans-activation through indirect binding to enhancer DNAs.

Prospective and randomized clinical trial for the treatment of hepatocellular carcinoma--a comparison between L-TAE with farmorubicin and L-TAE with adriamycin: preliminary results (second cooperative study). Cooperative Study Group for Liver Cancer Treatment of Japan.

A randomized controlled clinical trial was conducted to compare the use of farmorubicin (FARM) and adriamycin (ADR) in Lipiodol transcatheter arterial chemoembolization (L-TAE) as a treatment of hepatocellular carcinoma. In all, 192 hospitals participated, and 415 patients were enrolled in the study during the period from October 1989 through December 1990, and their data were collected. The patients were randomly allocated to group A (FARM) or group B (ADR) by a central telephone registration. Several clinical characteristics were slightly worse in group A than in group B, but there was no statistically significant difference. The actual doses of FARM and ADR were 72 mg/body and 48 mg/body, respectively. Additional treatments, including repeated TAE or surgery, were given to 248 patients. The 1- and 2-year survival rates were 69% and 44% for group A and 74% and 57% for group B, respectively. The difference was marginally significant (P value in the log-rank test, 0.038). When each group of patients was classified into two subgroups, i.e., high-risk and low-risk categories, based on the severity index calculated by the Cox regression model from significant prognostic factors, the ADR subgroup was significantly superior to the FARM subgroup in the low-risk category, but there was no significant difference between the subgroups in the high-risk category. The change in the serum alpha-fetoprotein level, the extent of Lipiodol accumulation in the tumor, and the extent of tumor reduction did not show any significant difference between the groups. At the above-mentioned doses, ADR seemed to have efficacy almost the same as or slightly superior to that of FARM in L-TAE for the treatment of hepatocellular carcinoma.

Dietary NaCl restriction deteriorates oral glucose tolerance in hypertensive patients with impairment of glucose tolerance.

This study investigated whether the change of glycemic response to oral glucose loading with an increase of dietary NaCl intake is different between salt-sensitive and salt-resistant groups, or whether it is related to glucose tolerance on a low NaCl diet independent of salt sensitivity. The plasma glucose and insulin response to 75 g oral glucose intake was assessed on low (34 mmol/day) and high (342 mmol/day) NaCl diets in 31 patients with essential hypertension, and the area under the curve for both variables (AUCglu and AUCins) was calculated. The data on the high NaCl diet were corrected for change in hematocrit. The percentage change in systolic, diastolic, and mean blood pressure between the two diets was defined as the salt sensitivity index (SSI) for systolic blood pressure (SSISBP), diastolic blood pressure (SSIDBP), and mean blood pressure (SSIMBP), respectively. The mean values of both AUCglu and AUCins decreased significantly with increase of NaCl intake; however, there was no significant correlation between SSI (SSISBP, SSIDBP, or SSIMBP) and the percentage changes in AUCglu and AUCins. Meanwhile, the percentage changes in AUCglu and AUCins significantly correlated with the respective values of AUCglu and AUCins on the low NaCl diet. These results suggest that extreme NaCl restriction may deteriorate glucose metabolism in hypertensive patients, especially in those with diabetes mellitus or impaired glucose tolerance.

Timing for administration of an antihypertensive drug in the treatment of essential hypertension.

To find the best timing for administration of long-acting antihypertensive drugs, we gave nitrendipine, a calcium antagonist of the dihydropyridine group, once a day to six hospitalized and drug-free patients with essential hypertension, changing the time of administration and studying the effects on the circadian rhythm of blood pressure. After control values of 24-hour blood pressure variations were taken with patients on placebo, a 10-mg tablet of nitrendipine was given for 3 days on three occasions--at 6 AM on awakening, at 8:30 AM after breakfast, and at 6 PM after supper; 24-hour blood pressure values for each period were recorded on the third day. The 24-hour blood pressure values during the control period showed a biphasic circadian rhythm, with higher values during wakefulness and lower values during sleep. The control period was also characterized by a rapid rise in blood pressure on awakening, the so-called morning surge of blood pressure, and a gradual decline during sleep at night. Although the morning surge was not completely suppressed by nitrendipine given after breakfast, it was diminished by the drug given on awakening or after supper; the latter brought a deeper decline in blood pressure during sleep compared with other times. The average of 24-hour blood pressure values obtained by nitrendipine given on awakening was the lowest among the three occasions. Thus, administration of long-acting calcium antagonists with a rapid onset of action on awakening in the early morning seems to be a more rational and beneficial alternative than the conventional administration after breakfast.(ABSTRACT TRUNCATED AT 250 WORDS)

EFFECT OF MECHANICAL VIBRATION ON ACTIVE TENSION IN THE LONGITUDINAL RETRACTOR MUSCLE OF A SEA CUCUMBER STICHOPUS JAPONICUS

1. The effect of mechanical vibration on active tension in an echinoderm somatic smooth muscle was studied using the longitudinal retractor muscle (LRM) of a sea cucumber Stichopus japonicus. 2. The steady contracture tension in LRM fibres maximally activated with 10(-3) mol l-1 acetylcholine (ACh) was reduced by vibrations (peak-to-peak amplitude, 0.5­2.5 % of l0, where l0 is the slack length of the muscle; frequency, 5­100 Hz). The extent of reduction of active contracture tension increased with increasing amplitude of vibration, but it did not change appreciably with increasing frequency of vibration. 3. The steady contracture tension in LRM fibres submaximally activated with 10(-5) mol l-1 ACh was more markedly reduced by vibrations than was that in maximally activated fibres. 4. The vibration-induced reduction of active contracture tension disappeared when temperature was lowered from 20­23 to 0 °C. 5. The development of contracture tension in LRM fibres activated with ACh was not affected by mechanical vibration. 6. These results are discussed in connection with the vibration-induced decrease in the rate of breakage of the actin­myosin linkages responsible for isometric force generation.

Development of a thermoelectric cooling apparatus for high-voltage isoelectric focusing on a cellulose acetate membrane.

To develop an isoelectric focusing apparatus using a cellulose acetate membrane (Separax EF), we have designed a thermoelectric cooling isoelectric apparatus. This apparatus has two characteristics. Firstly, the cooling system was switched to a thermoelectric cooling system from an ice-cooling system. Secondly, the chamber lid of the electrophoretic apparatus was also devised so that samples could be applied without opening the chamber lid. With this apparatus we could perform the isoelectric focusing without worrying about room temperature and humidity in the laboratory. Applying 2000 V for an extra 5 min with our module cooling system, we achieved a much higher degree of resolution with three sheets of cellulose acetate membrane (Separax EF) overlaid for simultaneous electrophoresis. Thus, three types of information could be obtained from only one electrophoretic procedure.

A case of Sipple's syndrome with malignant pheochromocytoma treated with 131I-metaiodobenzyl guanidine and a combined chemotherapy with cyclophosphamide, vincristine and dacarbazine.

A 39-year-old woman with Sipple's syndrome and a malignant pheochromocytoma (PHE) is presented. She is a member of a big family with this syndrome and had undergone bilateral, subtotal adrenalectomy because of bilateral PHE six years prior to the present admission. In 1989, a small nodule was identified in her right thyroid lobe by ultrasonography and was suspected to be a medullary thyroid carcinoma (MTC). Further examinations revealed the coexistence of multiple lung and liver masses. These tumors were considered to be metastases of PHE because of the increased urinary excretion of catecholamines as well as extremely high catecholamines both in the hepatic venous blood and in the cystic fluid of the liver tumor even though there was no apparent recurrences of PHE in the adrenal region. After surgical removal of the MTC in August, 1989, she was given 3.7 GBq of 131I-metaiodobenzyl guanidine (131I-MIBG) infusions twice--in October, 1989 and in August, 1990. 131I-MIBG showed a strong accumulation in the lung and liver masses on both occasions. Periodic increases in blood pressure and tachycardia with an excessive catecholamine secretion were observed over two weeks after the first treatment. However, this treatment was not effective in reducing urinary catecholamines nor the size of the metastatic tumors. She, therefore, underwent chemotherapy with cyclophosphamide, vincristine and decarbazine in 1991, which slightly but significantly reduced urinary excretion of catecholamines and the size of the lung tumors. She had clinically stable period for one year after the treatment but rapid growth of these metastatic tumors occurred afterwards and she died in August, 1992.(ABSTRACT TRUNCATED AT 250 WORDS)

[Comparative analysis of living conditions of elderly diabetic patients in rural areas].

The present study was performed to examine living conditions of elderly diabetic patients in rural areas in Iwate and Miyagi in northern Honsyu and compared them with similar cases in Tokyo. One hundred and nine cases over 60 years old were interviewed, and results were compared to 109 age- and sex-matched random samples in the Tokyo area. The original Tokyo subjects consisted of 383 outpatients of the Diabetic Clinic of Tokyo Metropolitan Geriatric Hospital. Types of family structure, educational background and total income were quite different in the 3 areas, and patterns of social support depended on area-specific characteristics. Attitudes toward diabetic treatment (acceptance of importance and compliance) in Tokyo were excellent, but the sensation of social burden and dissatisfaction were also at a high level. In contrast, the level of compliance in Iwate was low. Total morale scale scores were similar in the 3 areas. Multivariate analysis (dependent variable is the total morale score) showed significant contribution of levels of ADL and adherent behavioral style only in Tokyo. In contrast, the feeling of social burden was a negative factor in all areas. The quality of life of elderly people with chronic diseases such as diabetes, is affected by ability to adaptation to living conditions.

[Lobectomy under thoracoscopic guidance].

From September '92 to November '93, 15 lobectomies were performed under thoracoscopic guidance. After a thoracoport was inserted through the seventh intercostal space to conduct the operation utilizing a monitor, a six to ten cm incision was made in the fifth intercostal space. The pulmonary arteries and veins were divided after stapling with an Endo GIA or Roticulator 30-VIII. The lobar bronchus was stapled with a Roticulator 30-3.5, 30-4.8 or an Endo TA and affected lobes were resected. In cases of lung cancer, lymph node dissection has become possible as well due to the improvement of endoscopic instrumentation.
