Determination of 17-oxosteroid sulphates in serum by ion-pair extraction, prelabelling with dansylhydrazine and high-performance liquid chromatography with fluorescence detection.

A new high-performance liquid chromatographic (HPLC) method with fluorescence detection is described for the direct determination of four serum 17-oxosteroid sulphates. Each serum sample was deproteinated with methanol, the methanol was evaporated and 17-oxosteroid sulphates in the residue were extracted with benzene as ion pairs in the presence of tetrapentylammonium ion. The ion pairs were labelled with dansylhydrazine and the hydrazones were separated by HPLC on a Capcell-Pak C8 (silicone polymer-coated silica gel modified with octyl groups) reversed-phase column using methanol-0.5% (w/v) sodium acetate-50% (v/v) acetic acid (57:42:1, v/v/v) as the mobile phase. The eluent was monitored with a fluorometric detector at an excitation wavelength of 330 nm and an emission wavelength of 540 nm.

Duodenal carcinoid (somatostatinoma) combined with von Recklinghausen's disease. A case report and review of the literature.

We report the immunohistochemical and ultrastructural features of a case of duodenal carcinoid (somatostatinoma) combined with cutaneous-type von Recklinghausen's disease in a 65-year-old woman. The duodenal tumor located at the 2nd portion was composed of tumor cells arranged in a trabecular, glandular or nest-like pattern, occasionally associated with eosinophilic globules in the glandular structures. The tumor cells mostly showed strong immunoreactivity with anti-somatostatin antibody. Electron microscopy revealed that every tumor cell contained intracytoplasmic granules with electron-dense cores, 100-300 nm in size, in varying numbers, and intracytoplasmic lumina were rarely detected. The present case including identical cases reported so far suggest that the disease entity is likely to be a combination of duodenal somatostatinoma and von Recklinghausen's disease and/or phaeochromocytoma. This is the tenth case of duodenal carcinoid associated with von Recklinghausen's disease and/or phaeochromocytoma in the world literature and the third case reported as duodenal somatostatinoma.

Sympathetic nervous function and erythrocyte cation transport systems in normotensive individuals with family history of hypertension.

To investigate the influence of heredity to the sympathetic nervous function and the cell membrane cation transport systems, we studied the blood pressure and plasma catecholamine response to supine exercise testings by bicycle ergometer, the pressure response to noradrenaline infusion tests and the heart rate response to isoproterenol infusion tests in 88 healthy Japanese sedentary normotensive men with and without a family history of essential hypertension [FHH(+) and FHH(-)]. Several erythrocyte monovalent cation transport parameters were also measured in 74 of these individuals. In the results, (1) the systolic blood pressure response to exercise testings and noradrenaline infusion tests were larger in FHH(+) than FHH(-): (2) there was no difference between FHH(+) and FHH(-) in the heart rate response to isoproterenol infusion tests: (3) there was no significant difference between FHH(+) and FHH(-) in the increased plasma catecholamine levels to exercise testings: (4) the intraerythrocytic sodium content was significantly higher in FHH(+) than in FHH(-): and (5) several erythrocyte monovalent cation transport systems (Li-Na countertransport, Na-K cotransport and Na-K pump activity) were clearly accelerated in FHH(+). We concluded that in spite of normotension there were abnormalities of sympathetic nervous function, intracellular sodium content and several cell membrane cation transport systems in individuals with a family history of essential hypertension.

[Erythropoiesis and hemolysis in hemodialysis patients].

In order to investigate the pathogenesis of renal anemia, erythroid marrow cellularity, factors affecting erythropoiesis and hemolysis, hemolysis starting point by Parpart method and red cell life-span were studied in 21 patients undergoing hemodialysis (HD). Mean value of serum erythropoietin level (EPO) in HD patients was 28.4 mU/ml, which value was nearly equal to that in healthy subjects. Total erythroblast count was higher than normal up to 25.2% in HD patients with Ht below 25% (A group), on the other hand, in HD patients with Ht above 25% (B group) it was 21 6%, nearly equal to normal. Total erythroblast counts positively correlated to EPO level, but did not correlate to ribonuclease, aluminium and parathyroid hormone. Red cell life-span was 23.4 days in A group, and it was 19.8 days in B group Hemolysis starting point was observed at 0.61% NaCl in B group, and at 0.56% in A group. Hemolysis starting point negatively correlated to red cell life-span, but did not correlate to BUN, serum creatinine and serum guanidino compound. Hb level negatively correlated to nuclear cell counts of bone marrow in HD patients, and positively correlated to hemolysis starting point. These results suggested that erythroblast count was controlled by both erythropoietin and hemoglobin levels in HD patients. Hemoglobin level in HD patients was maintained by balance of counteracting factors between erythropoiesis and hemolysis.

[A case of lymphangiomas arising contiguously from the rib and thoracic spine].

Reported is a rare case of lymphangiomas arising contiguously from the left 6 th rib and thoracic spine of a 33-year-old female. The localization of the osteolytic lesions that had been observed by chest X-rays were more precisely detected by subsequent computed tomography. Histologically, dilated vessels lined with endothelial cells were observed in the bone marrow. Immunostaining uncovered factor VIII-related antigens in the endothelial of the small vessels, but not in those of the larger vessels. In order to differentiate lymphangiomas from hemangiomas of the bone, not only should the histological features of the lymph vessels be observed at time of operation also the ability these vessels to retain lymph is important. Primary lymphangioma of the bone should be differentiated from lymphangiomatosis and massive osteolysis, which show a poor prognosis.

[A study on hemolysis in hemodialysis patients].

In order to investigate the pathogenesis of hemolysis in chronic renal failure, red cell life span and hemolysis starting point (HSP) by coil planet centrifuge method were studied in 32 hemodialysis (HD) patients and 16 healthy subjects. Mean red cell life span was 22.0 days before recombinant human erythropoietin (r-HuEPO) therapy, and prolonged up to 28.1 days after r-HuEPO therapy. Mean HSP in HD patients was significantly elevated than healthy subjects. Mean HSP was 106.7 mOsm before r-HuEPO therapy, and improved to 101.0 mOsm after r-HuEPO therapy. HSP improved from 106.8 mOsm to 100.8 following the correction of bicarbonate level with HD. HSP was negatively correlated to bicarbonate level pre- and post-r-HuEPO therapy, but HSP was not correlated to BUN, serum creatinine, hydrogen ion, anion gap and amount of body water removal during HD. When blood PH in 18 HD patients was adjusted by 7% NaHCO3 from 7.21 to 7.40, HSP improved to normal range in all. These data suggested that osmotic fragility was normal in younger erythrocyte in HD patients, and a decrease of plasma bicarbonate level resulted in an increase of hemolysis.

Direct determination of four sulfates and seven glucuronides of 17-oxosteroids in urine by fluorescence "high-performance" liquid chromatography.

We describe a direct method for determining four sulfates and seven glucuronides of 17-oxosteroids (17OS) in urine without hydrolysis, by use of "high-performance" liquid chromatography (HPLC) with fluorometric detection. After pretreatment of urine samples with a Sep-Pak C18 cartridge, four 17OS sulfates and seven 17OS glucuronides in the pretreated urine samples were reacted with tetrapentylammonium ions to form ion pairs. Ion-paired 17OS sulfates were extracted with benzene. By adding sodium sulfate to the remaining sample, we could then extract ion-paired 17OS glucuronides with dichloromethane. Each extract was labeled with dansyl-hydrazine in an acetic acid-acetonitrile solution. The labeled steroids were separated by HPLC on a reversed-phase Capcell-Pak C8 (silicon-polymer-coated silica gel modified with octyl groups). We monitored each effluent with a fluorometric detector (330 nmexcitation, 535 nmemission).

[A case of malignant transformation of gastric hyperplastic polyp with metaplastic bone formation].

A case of malignant transformation of a gastric hyperplastic polyp with a metaplastic bone formation is reported. A large gastric polyp was revealed by X-ray examination and by endoscopy in the cardia of a 89-year-old female who had complained of nausea. A polypectomy was performed since the biopsied materials had contained some atypical glands intermingled with hyperplastic glands. Histologically, the polyp, 1.7 x 1.0 x 0.6 cm in size, was a hyperplasic polyp that was found to contain superficial dysplastic foci associated with a moderately differentiated type of a tubular adenocarcinoma. A metaplasic bone formation was found in stroma beneath the hyperplastic glandular epithelium. Thus, polypectomy of hyperplastic polyps of the stomach of more than 1 cm with dysplastic foci is recommended.

[Acute and chronic effects of a new oral inotropic agent, ibopamine hydrochloride, on hemodynamic and metabolic responses to ergometer exercise in patients with severe congestive heart failure].

The hemodynamic and metabolic effects of ibopamine, an orally active 3,4-diisobutylic ester of N-methyl-dopamine, at rest and during exercise, were evaluated in 8 patients with severe congestive heart failure, initially at 2 hrs after oral 100 mg and after 4 weeks of maintenance therapy (100 mg, 3 times a day). At rest, a single dose of ibopamine increased CI, SVI and SWI, and reduced PCWP, PAm, RA and PVR. These beneficial hemodynamic effects were maintained during supine bicycle exercise: CI and SVI increased markedly (p less than 0.05) and PCWP, PAm, RA and SVR increased at a lower extent (p less than 0.05) after ibopamine than on control conditions. After chronic therapy with the drug, LVDd and CTR decreased (p less than 0.05) and exercise tolerance time increased (p less than 0.01). HR, BPm, double product, norepinephrine, epinephrine, renin activity, aldosterone and lactate/pyruvate did not change significantly after administration of ibopamine. Clinical responses after 4 weeks of maintenance therapy were good in 4 cases and fair in 1 case. No adverse effects were observed. Therefore, ibopamine seems to have not only a positive inotropic action, but also a potent vasodilating action, leading to both afterload and preload reduction and so to be useful for the treatment of severe congestive heart failure at rest and during exercise after acute and chronic therapy with the drug.

Immunocytochemical and ultrastructural characterization of human T-lymphotropic virus type I (HTLV-I)-producing rabbit lymphoid cell lines.

Fine structural and immunocytochemical characterization of rabbit lymphoid cell lines transformed by human T-lymphotropic virus type I (HTLV-I) was carried out. All nine cell lines tested were reactive with anti-HTLV-I-positive human, monkey, and rabbit sera and monoclonal antibody to HTLV-Ip 19, but not with anti-HTLV-I-negative sera and monoclonal antibodies to human Ia and pan-T antigens. All cell lines were strongly positive for monoclonal antibodies to rabbit Ia and pan-T antigens. Ultrastructurally, each cell line contained C-type virus particles in varying numbers in the extracellular space. These particles showed replication patterns similar to those in HTLV-I or simian T-lymphotropic virus type I (STLV-I)-producing human or monkey cells. In addition, anti-HTLV-I-positive rabbit serum gave positive immunoreactivity to HTLV-I or STLV-I by indirect immunoferritin method. These results indicate that the ultramorphology and replication patterns of HTLV-I in rabbit cell lines are indistinguishable from those of HTLV-I in human and monkey cell lines, HTLV-I in rabbit cells shares the common surface antigenic determinants with HTLV-I or STLV-I in human or monkey cells, and that these cells are definitely rabbit T cells bearing their own Ia antigens.

Dissociation of associative and nonassociative concomitants of classical fear conditioning in the freely behaving rat.

An acoustic stimulus previously paired with footshock elicits stereotyped increases in arterial pressure and heart rate and induces freezing behavior in freely behaving rats. Although the arterial pressure and freezing responses differ between groups given paired and random presentations of the tone and shock, the increases in heart rate do not. These observations, if taken at face value, suggest that the arterial pressure and freezing responses reflect associative learning but that the heart rate change is a nonassociative or a pseudoconditioned response. In this article we describe three experiments aimed at determining why the CS elicits similar increases in heart rate in groups given paired and random training. The first study demonstrates that regardless of the pseudoconditioning control procedure used (random, backwards, shock-alone, or naive), the same pattern of results is obtained: the increases in arterial pressure are greater in the paired than in each control group, but the heart rate rises to the same extent in all groups. The second study determined that the context in which the responses are tested (conditioning apparatus vs. novel test chamber) does not affect the general pattern of results obtained. The third study demonstrates that the superficially similar increases in heart rate in conditioned and pseudoconditioned rats are achieved by different physiological mechanisms: coactivation of the sympathetic and parasympathetic nervous systems in conditioned rats and sympathetic excitation alone in pseudoconditioned rats. Thus, the heart is influenced by associative emotional processes, but heart rate is not, under these conditions, a particularly useful index of those influences.

Sodium sensitivity of blood pressure and baroreceptor reflex function in patients with essential hypertension.

In order to define the role of the baroreceptor reflex function in the sodium sensitivity of blood pressure in patients with mild essential hypertension, 25 patients were classified into two groups, salt-sensitive and salt-insensitive, depending on the difference in the averages of the resting systolic blood pressure, taken hourly on the fifth day of 7 days of sodium depletion and on the fifth day of 7 days of repletion. Increases in urinary sodium excretion and body weight and a decrease in haemotocrit during the sodium repletion period were similar in both groups. Baroreceptor reflex function, estimated from the baroreceptor slope and the blood pressure change on a 70 degrees tilting test, was enhanced by the sodium repletion period in the sodium-insensitive group but not in the sodium-sensitive group. These results suggest that sodium sensitivity might be due to differences in the ability of the baroreceptor reflex function to become sensitized during a high sodium intake.

Immunoelectron microscopic study of S-100 beta-positive human T-lymphocytes with special reference to double immunostaining.

S-100 beta-positive T-lymphocytes in human peripheral blood were shown to be definitely T-cells containing the S-100 beta subunit in their cytoplasm by double immunostaining using immunoferritin labeling for CD8 antigen and immunoperoxidase labeling for cytoplasmic S-100 beta subunit at the ultrastructural level. These results ultrastructurally confirmed the presence of double markers of S-100 beta-positive T-lymphocytes seen by light microscopic double immunostaining. Furthermore, this double immunostaining method for electron microscopy is applicable to other kinds of cells for the simultaneous detection of two different antigens present both on the cell surface and in the cytoplasm.

Glycogen-rich clear cell carcinoma arising from minor salivary glands of the uvula. A case report.

Glycogen-rich clear cell carcinoma arising from minor salivary glands of the uvula in a 35-year-old woman is reported. This tumor was composed of nests and cords of clear cells containing cytoplasmic glycogen but no mucin. The epithelial nature of this lesion was obvious because of positive immunoreactivity for cytokeratin and epithelial membrane antigen, and the presence of immature lumina and intercellular spaces lined by many microvilli, associated with desmosomal junctions and basal lamina, as revealed by ultrastructural study. However, no myoepithelial cells could be detected. From these findings, it may be concluded that this tumor corresponds to glycogen-rich clear cell carcinoma (a variant of clear cell tumor), revealing glandular differentiation.

Postoperative starch granuloma revealed as femoral herniation. A case report.

A rare case of postoperative starch granuloma in a 55-year-old woman, revealed as femoral herniation, is reported. A small, finger-tip-sized tumor showing femoral herniation was noticed 3 weeks after cholecystectomy for cholelithiasis, and no content in the hernia sac was noticed. Histologically, the granuloma situated in the wall of the hernia sac was diagnosed as a starch granuloma consisting of starch granules, which were clearly revealed by polarized light microscopy. These granules were detected in the cytoplasm of macrophages including multinucleated types, some showing a positive immune reaction with antibodies to the alpha-subunit of S-100 protein, vimentin and lysozyme. Starch granules from surgical gloves showed identical features by polarized light microscopy, thus proving the histogenesis of this granuloma. This is the first reported case in the Japanese literature of starch granuloma manifested as femoral herniation.

Behçet's disease with chylothorax--case report.

Behçet's disease is a systemic disease primarily affecting the eye, genital, and oral mucosa but is known to have various rare complications. A patient with a twenty-year history of Behçet's disease was admitted owing to dyspnea caused by massive chylothorax. Administration of corticosteroid, heparin, and urokinase was ineffective, but the thoracic fluid was well controlled by pleuropexy with Broncasma Berna. Chylothorax complicating Behçet's disease is extremely rare, having been reported in only 2 cases.

[A case of juvenile polyp in an elderly person with hepatocellular carcinoma].

A rare case of a juvenile polyp in association with a hepatocellular carcinoma in a 57-year-old male is reported. Both diseases were discovered after admission to hospital because of chronic active hepatitis. A liver biopsy revealed the presence of a trabecular type of differentiated carcinoma. Then, upon colonic fiberscopy, a solitary small pedunculated polyp was detected in the rectosigmoid region. Histologically, the lesion was found to be a typical juvenile one, and composed of cystic dilatations of some glands, including somewhat hyperplastic glands and a wide stroma consisting of chronic inflammatory granulation. The possible histogenesis of this observed polyp in an elderly person also is discussed.
