[Pharyngeal branchial cyst: description of one clinical case].

Brachial cysts are abnormalities in the development of the branchial apparatus, being the more common ones those of the second arch. Cysts localized in the pharyngeal wall (Bailey's type IV) are extremely rare; therefore its differential diagnosis should include all benign and malignant lesions that can be found in the pharyngeal wall. Treatment is the total excision of the cyst through per oral or external cervical approach in order to avoid recurrences.

[An unusual complication of esophageal foreign body].

Esophageal foreign bodies must be carefully managed because they carry a high risk of potentially severe complications. A case of a sharp foreign body located in a previously unsymptomatic pharyngoesophageal diverticulum is described. The management protocol for esophageal foreign bodies is reviewed. Fiberesophagoscopy is the therapeutic technique of choice, but it may be dangerous if patients are not selected properly.

[Heterotopic brain tissue in the pharynx].

The presence of heterotopic brain tissue in the head and neck region is exceptional. Most reported cases have been observed in the nasal region and are called nasal gliomas. The case of a 10-month-old boy with heterotopic brain tissue on the soft palate and nasopharynx is reported. Surgical treatment was successful and no complications or recurrences have been observed in 7 years of follow-up. The main pathogenic theories and nomenclature are analyzed.

[Laryngo-tracheoplasty in the management of infantile laryngeal stenosis].

We present the results obtained in our center with laryngo-tracheoplasty for the management of laryngeal stenosis in infants and children. Nineteen patients were treated between 1989 and 1995. Their weights ranged from 1,200 g to 36 kg. Twenty-five operations were performed, not including endoscopic maneuvers. Fifteen patients have been decannulated (79%). Complications were infrequent and resolved satisfactorily. We consider that these surgical procedures should occupy a prominent place in the management of pediatric patients with laryngeal stenosis.

[Naso-alveolar cysts. A report of 10 new cases and a review of the literature].

Nasoalveolar cysts, also known as Klestadt's or nasolabial cysts, are uncommon midfacial lesions. Ten cases treated at the 12 de Octubre University Hospital are reported, describing the most relevant clinical and pathological findings are reported. The main theories of the origin of these developmental, non-odontogenic cysts are discussed. A review of the literature is included.

[Primary nasal sinus malignant melanomas].

Primary malignant melanoma of the mucosa of the nasal and paranasal sinuses is rare, difficult to treat, and usually has a poor prognosis. Seven cases, six women and one man (mean age 76.5 years), were treated at the Hospital 12 de Octubre (Madrid, Spain) during the period 1982-1994. The presenting symptoms, diagnostic evaluation, morphology, therapeutic modalities, histological evaluation, and clinical course were reviewed. Three patients were treated surgically and three received palliative irradiation. All had local recurrence or distant metasfases within 15 months after concluding treatment. The seventh patient refused treatment.

[Adenosquamous carcinoma of minor salivary glands. Reporting a case in the tongue].

Adenosquamous carcinoma of the salivary glands is an unusual neoplasm not unanimously included in many classifications of salivary gland malignancies. A case sited in the oral cavity, within the tongue is reported. Clinical and histopathologic features are described together with a discussion about treatment and behaviour. We also include an updated of literature relating to this entity.

[Adjuvant chemotherapy in the treatment of pharyngolaryngeal cancers in advanced stages].

Since 1980 we use adjuvant chemotherapy in advanced laryngopharyngeal carcinomas. These cases were resectable and unresectable lesions with tumoral extent to the pharynx as a common characteristic. We analyze the survival of the first 50 cases treated with chemotherapy followed by: A) surgery and radiotherapy (36 cases) and B) radiotherapy alone (14 cases). Results of this not randomized study are compared with a historic group of advanced laryngopharyngeal carcinomas (T4/N+) treated with conventional therapy (surgery +/- radiotherapy). Response to chemotherapy was complete or partial (> 50% reduction) in 56 percent of the patients. There was improvement in overall survival and five-years disease-free survival in the latter group compared with those who did show any response (p < 0.01). We would point out that disease-free survival of the group A was better than historic group (60% vs 36%, p < 0.05), although these result should be carefully interpreted. Laryngeal preservation was achieved in 47 percent in the group A by modification of the initially scheduled radical surgery, in selected cases.

[Modifications of the technic of endolaryngeal extirpation. The author's own functional technic of laryngectomy].

A brief summary of the original technique will be made incorporating the following modifications and additions: glottis substituted by neoglottis muscles, sternohyoid or external perichondrium. Supraglottis substituted by remaining tissues of the epiglottis or neoepiglottis. Hypopharynx, section medium line of the pharynx constrictor. Suturing of cartilage with homologous donor cartilage. An evaluation will be made of the functional results.

Malignant oncocytoma of a minor salivary gland: an unusual presentation at the base of the tongue.

In contrast to malignant oncocytomas of the parotid gland, malignant oncocytomas arising from minor extraparotid glandular tissue are rare. The latter may display a locally aggressive behaviour as well as a variable tendency to develop secondary cervical lymph nodes. We report a new case of malignant oncocytoma in a 43-year-old man presenting with greatly enlarged cervical lymph nodes accompanied by a poorly defined swelling at the base of the tongue microscopically identified as a malignant oncocytoma. The diagnosis was strongly supported by the ultrastructural features of the cells almost completely filled with mitochondria. The cervical mass was excised, and the patient has suffered no recurrence for nearly 2 years after surgical treatment.

[Prognostic factors in pediatric myringoplasty. A review of 90 cases].

Although tympanic membrane perforations continue to be a significant source of otologic morbidity in the pediatric population, myringoplasty in children still elicits debate. The records of 76 patients under 18 who underwent 90 myringoplasties at the 12 de Octubre University Hospital (Madrid, Spain) over a recent 8-year period were reviewed. The overall short-term surgical success rate was 64.8%. The overall success rate evaluated at least one year after surgery was 58%. The audiological threshold improved in 68% per cent of successful cases. Surprisingly, females had lower graft take rates than males. The outcome of surgery was unrelated to age at operation, duration, mechanism, size, and location of the perforation, or condition of the opposite ear.

[Granular cell tumor: an infrequent cause of dysphonia in childhood].

Granular cell tumors (GCT) are rare and usually benign tumors whose histogenesis is debated. The skin, subcutaneous tissues, and mucosae of the head and neck are areas of predilection for GCT. Laryngeal involvement is uncommon, but may create diagnostic and therapeutic problems when it occurs. Laryngeal GCT are decidedly uncommon in children, only 17 cases having been reported in the literature. A case of GCT of the larynx in an 11-year-old girl is reported. The presenting symptom was hoarseness. The macroscopic tumor and hoarseness disappeared after chemotherapy (EVAIA) for Ewing sarcoma of the knee.

[Lingual abscess with atypical clinical presentation].

Lingual abscesses have become extremely rare since the discovery of antibiotics, with only about 200 cases reported in the last 160 years. Diagnosis on the basis of clinical symptoms alone is quite difficult. Imaging studies are essential for the diagnosis of lingual abscesses and for their differentiation from related pathologies with similar clinical symptoms. We report a case of lingual abscess with otherwise unremarkable clinical findings that was diagnosed by CT scan. The etiology, pathophysiology, bacteriology, diagnosis, and treatment of these uncommon infections is discussed.

[Functional laryngectomy using a technic of subperichondrial laryngeal curettage].

A new technique of functional laryngectomy using sub-perichondral curettage of some or all of the soft parts of the larynx is described. This method has an embryological and anatomical basis. It is supported by a score of cases which have had a satisfactory outcome, all the patients operated on having been decannulated. Histopathological research was continued with a view to confirming the function of the perichondrium and the cartilage as a barrier to neoplastic invasion.
