Adrenergic responses of asthmatic and normal subjects to submaximal and maximal work levels.

The response of the adrenergic system of asthmatic subjects to exercise and the role of plasma catecholamines in exercise-induced asthma were investigated. Plasma levels of norepinephrine and epinephrine were measured at rest, during and after exercise in 7 asthmatic and 9 matched normal subjects. Exercise-induced bronchospasm occurred in all asthmatic subjects following exercise, while no significant change was observed in the normal subjects. The results showed that plasma levels of norepinephrine and epinephrine at rest and changes that occcurred during and after exercise were similar in both normal and asthmatic subjects. These data suggest that the adrenergic response of asthmatics to the same relative exercise stress as reflected in plasma catecholamine levels does not differ from that of normal subjects. It appears that changes in the circulating catecholamines do not play a significant role in the pathogenesis of exercised-induced asthma.

Risk factors and angiographically determined coronary occlusion.

An association of coronary artery occlusion, as determined by coronary arteriography with age, prevalence of risk factors and alcohol intake was studied in 1635 male and 371 female heart patients. The degree of coronary artery occlusion was positively related to elevated cholesterol, elevated triglycerides, diabetes, age and history of smoking for both male and female patients. Hypertension was related to the degree of occlusion only for female patients. Male and female patients who had a higher alcohol intake tended to have less extensive occlusion. No positive association was found between obesity and the degree of occlusion. When the patients were divided on the basis of age (less than 50 and greater than or equal to 50 years) the findings did not differ. The lack of finding a relation between obesity and occlusion or between hypertension and occlusion for males differs from the general findings of epidemiologic studies on the relation between these risk factors and coronary heart disease. The authors believe that this discrepancy may either be explained by the way they selected patients (i.e., they selected patients who underwent a diagnostic angiographic examination), or that obesity and hypertension are not directly related to coronary occlusion but influence occlusive disease through some secondary mechanism.

Risk factors and mortality in patients with aortocoronary vein bypass operation.

Patients who died one or more years after aortocoronary bypass surgery showed, at the time of the operation, higher prevalence of hypertension, and history of smoking and had higher plasma lipid levels than the patients who survived the operation for similar periods of time. No such differences in risk factors were seen between patients who survived or have died during the first 30 days after the operation. These findings suggest that pateints with abnormal risk factors have a poorer long-term prognosis.

Anaphylaxis in isolated rabbit lungs.

Lungs from rabbits sensitized to ovalbumin or bovine gamma-globulin were isolated and perfused with autologous blood. The response to antigen challenge via the perfusate was immunologically specific and characterized by a marked increase in perfusion resistance, a moderate increase in airway resistance and a small decrease in lung compliance. The response could also be elicited by specific antigen challenge in lungs from sensitized rabbits perfused with blood from normal rabbits and in lungs from normal rabbits perfused with blood or plasma from sensitized rabbits. The magnitude of the response was greater when blood or plasma from sensitized animals was used as the perfusate. Therefore, blood and/or plasma factors appear to be the major contributors to the response.

Effect of aorta-coronary bypass operation on factors precipitating angina.

About two thirds (65 percent) of 271 male patients and one half of the 44 female patients who had angina pectoris prior to the aorta-coronary bypass operation experienced total relief of symptoms 1 year after the operation. This was especially apparent for angina associated with walking activity and the postprandial angina. In the nonoperated patients who had angina pectoris and only mild or no coronary artery obstruction, 38 percent of the 127 men and 27 percent of the 73 women were free of symptoms 1 year after the original examination.

Atherosclerotic aneurysm in aortocoronary vein graft.

An atherosclerotic aneurysm was found in aortocoronary vein grafts of two patients who had a second revascularization because of reappearance of anginal symptoms five and six years, respectively, after the initial bypass operation.

Risk factors in patients undergoing a second aorta-coronary bypass procedure.

A group of 38 patients with a second revascularization procedure was studied for factors which may have contributed to the reappearance of angina after the first operation. Our data indicate that these patients usually have the first operation at an earlier age and had fewer bypasses at that time. In addition, they had inadequate control of the plasma triglyceride and cholesterol levels.

Alcohol and nutrient intake of elderly men.

The effect of alcohol consumption on the intake of food nutrients was investigated in elderly residents in a Veterans Administration Domiciliary. When compared with a group of abstainers from the same institution, the imbibers ate fewer food calories (2,140 vs. 2,415 kcal per day) and had lower serum albumin levels (54 vs. 58 per cent total serum protein). The lower caloric intake extended to the three main energy sources (fat, protein, and carbohydrate) and occurred even though ample food was available. In view of the reduced nutrient intake and known impairment of the absorptive process by ethanol, institutionalized elderly alcoholics require special attention to maintain a satisfactory nutritional status.

Variability of extracts used in immunoprecipitin tests.

Immunodiffusion reactions of several different commercial fungal extracts from organisms were compared to those obtained from patients' environments. Sera were obtained from patients suspected of having hypersensitivity lung disease (HLD). There was a difference in reactivity among the sources of the antigen and in some lots of antigen from the same commercial supplier. The results demonstrate a wide variation in the reactivity of different antigenic extracts and emphasize the need for uniform standardization of preparations used in precipitin tests.

Coronary artery occlusion and alcohol intake.

A group of over 900 patients who underwent coronary arteriography were asked about their alcohol intake. The patients who were abstainers or consumed less than an equivalent of 180 ml absolute alcohol per week had higher coronary artery occlusion scores than the group consuming more than that amount, though the latter group smoked more and had higher plasma triglyceride levels.

Sarcoidosis in hypersensitivity pneumonitis.

An abnormal chest x-ray film showing hilar adenopathy, diffuse interstitial pulmonary infiltrations, or both, combined with a tissue biopsy revealing noncaseating granuloma, are suggestive of sarcoidosis; however, non-caseating granuloma may also be found in other forms of pulmonary disease. Immunologic and environmental evaluation of three patients with the diagnosis of sarcoidosis made by the above criteria, revealed hypersensitivity pneumonitis in all. Since therapeutic considerations in these two diseases are different (avoidance being the mainstay in hypersensitivity pneumonitis), all methods to ensure a correct diagnosis should be employed.

Late lesions in aorta-coronary artery vein grafts.

Aorta-coronary artery vein grafts, obtained from 2 patients 37 and 59 months after an aorta-coronary bypass operation, showed extensive mural changes characterized by degenerative tissue lesions, loss of normal morphological features of the intima, and extracellular accumulation of lipids. Both patients had increased plasma triglyceride levels.

The spectrum of pathologic changes in aortocoronary saphenous vein grafts.

Aortocoronary vein grafts removed at autopsy of 182 patients who died up to 5 years after the bypass operation were examined by light and electron microscopy. During the first month after surgery, graft occlusion was due to thrombi, some of them recent, some organized and recanalized. Intimal proliferation of the graft first appeared 4 weeks after the bypass surgery; several cases of total occlusion of the lumen by this process were seen within 6 months of the operation. A change in the morphological features of the intimal thickening from cellular to fibrotic and hylain-like was observed in several grafts implanted for more than 1 year. Typical atherosclerosis developed in vein grafts in three of the six patients surviving the operation for more than 3 years.

Changes in occupation after coronary arteriography.

This study, involving 266 white males evaluates the effect of angiographically determined mild coronary artery impairment upon occupational status. Questionnaires filled out at the time of coronary arteriography and about 13 months later showed that in the interim 65% of the patients stayed in the same occupational status, 31% changed occupational status, and 4% retired. In both younger (less than or equal to 55 years) and older men (greater than 55 years) the proportion of retirees increased after angiography. Of those who remained employed, 19% changed to jobs with higher physical demands while 11% took jobs with lower physical demands. The decision to change the job did not seem to be affected by age or the presence of angina pectoris. Comparison of this patient population with U.S. national average has shown that the presence or awareness of coronary artery disease is associated with measured increase in occupational mobility, regardless of the severity of the disease. Our findings suggest that changes in occupational status after determination of mild coronary artery disease are motivated by psychological rather than physiological factors.

Indirect immunofluorescent detection of antibodies against thermophilic actinomycetes in patients with hypersensitivity pneumonitis.

With slide culture preparations of Micropolyspora faeni, Thermoactinomyces candidus, and Thermoactinomyces vulgaris used as antigens, 59 sera from patients with hypersensitivity pneumonitis and 23 sera from normal controls were studied for the presence of specific antibodies by indirect immunofluorescence and immunodiffusion methods. Of the 82 sera, 31 showed precipitins to M. faeni, five to T. candidus, and four to T. vulgaris. A group of 19 sera positive to Aspergillus antigens showed questionable reactions to thermophilic actinomycetes. The 23 sera from normal individuals were negative for specific antibodies when tested by immunodiffusion. In indirect immunofluorescence studies, we found that IgG antibodies in high titers are present only in the precipitin-positive sera whereas other antibody classes either are negative or failed to show consistent differences in titers of the positive, questionable, and control groups. The titer of IgG antibodies varied from 0 to 1:20 in the normal control group, 0 to 1:40 to 1:640 in the precipitin-positive group.

High-performance liquid chromatography of pyruvic and alpha-ketoglutaric acids and its application to urine samples.

Pyruvic and alpha-ketoglutaric acids were quantitatively and rapidly converted to quinoxalones by a new method, to enhance their detectability in the ultraviolet. The quinoxalones were separated by reversed-phase chromatography with use of 10 micronm particles packed in a 30-cm (length) column. An aqueous solution of ammonium acetate and methanol was used for linear gradient elution. The urinary excretion of pyruvic and alpha-ketoglutaric acids by normal individuals, diabetic patients, and patients with renal dysfunction was determined.
