Sublabial surgical approach to the nasal cavity and paranasal sinuses.

A sublabial approach to the nasal cavity, paranasal sinuses and the nasopharynx degloving the middle third of the face has been described. Its major advantages over the other approaches have been compared and the usefulness of this approach has been stressed. The incidence of complications from this approach has been described and found to be acceptable.

An MMPI typology of male alcoholics: admission, discharge and outcome comparisons.

The admission MMPI profiles of approximately half of 337 male alcoholics treated at the Hazelden Foundation (Center City, Minnesota) were classifiable into four groups: neurotic, classic alcoholic, psychopathic, and psychotic. The discharge MMPI profiles of the neurotic, classic, and psychotic types showed foreshortening but maintained their basic configurations. The psychopathic type's discharge MMPI profile showed no major change either in elevation or shape. Several biodemographic and treatment outcome differences were found among the four profile types.

Alcoholism-related content in the MMPI: item analysis of alcoholics vs. normal and general psychiatric populations.

An analysis of alcoholism-related content in the MMPI was undertaken using an alcoholic population, a psychiatric population and two normal populations. The alcoholic and psychiatric groups were drawn from facilities in the Minneapolis-St. Paul area, and the normal populations were the Hathaway Adult Group and the Mayo Clinic nonpsychiatric patient group. A derivation study and a cross-validation resulted in the identification of three item pools: 1) that discriminate alcoholics from both normals and psychiatric patients, 2) items that discriminate alcoholics from normals only, 3) items that discriminate alcoholics from psychiatric patients only. Only seven items discriminate both male and female alcoholics from both normals and psychiatric patients. These items have substantial face validity. As the core of alcoholism-related content in the MMPI, they can serve as a stem for the development of scales with more specialized purposes.

Tumors of the parapharyngeal space.

A variety of tumors of the parapharyngeal space were operated upon during one training year at the Pack Medical Foundation, New York. These tumors characteristically present as a mass of the upper neck, behind the angle of the mandible and/or as a lateral pharyngeal mass bulging medially. Surgeons who operate in the parapharyngeal space must be thoroughly familiar with the complex anatomy of blood vessels, nerves, and muscles in this area and possess a thorough knowledge of the variety of tumors in this region of the head and neck.

Sex differences and androgyny in fantasy content.

Fantasy stories were composed by 153 undergraduates (93 females, 60 males) who also responded to the Bem Sex Role Inventory. The fantasy stories were collected by group administration of a Thematic Apperception Test. The stories were content analyzed along 17 imagery categories. Males and females differed significantly in ten of these categories. The results indicate a substantial continuity of sex differences in fantasy content between earlier decades and the mid 1970s. However, women had relatively more imagery of a self-assertive, pleasurable, and careerist nature than had been found in earlier studies. The fantasies of sex-typed persons were more situation-bound and more sexual than those of androgynous persons. Sex-typed persons appear to experience limitations in fantasy production which parallel their limitations in overt behavior.

Malignant mixed tumor of the soft palate.

Malignant mixed tumor (carcinoma within a pleomorphic adenoma) is an unusual tumor that rarely occurs in the minor salivary glands of the soft palate. We have reported a cases and a review of the terminology, diagnosis, and treatment.

The uses and limitations of auricular composite grafts.

Composite autografts of skin and cartilage from the auricule are useful in the reconstruction of defects in many areas of the head and neck. However, their use has certain limitations if minimal loss of the graft is to be achieved. A variety of examples are presented by the authors and the limitations are summarized.

Reconstruction following temporal bone resection.

The aggressive initial treatment of external auditory canal and middle ear malignancies using ablative surgery in the form of temporal bone resection combined with postoperative curative doses (6,000 rads) of irradiation has resulted in meaningful palliation and a seeming increase in cure rates for these lesions. This combined therapy regimen has placed new demands on the wound of the temporal bone resection and subsequently necessitated development of a more substantial reconstructive effort. Hypoglossal nerve crossover has proved to be an efficient means of facial reanimation following temporal bone resection, with minimal morbidity associated with the loss of motor function to one half of the tongue. The composite posterior cervical flap, including a portion of trapezius musculature, has satisfied both the functional and cosmetic needs of this combined therapy program.

Subdermal fascial augmentation.

A technique is described whereby limited soft tissue defects may be cosmetically improved using autogenous fascial strips. These strips of fascia lata are threaded onto a cadaveric needle that is tunneled subdermally, in a "basket weaving," criss-crossed fashion, augmenting the soft tissue defect and separating the scar from the underlying tissue. There are minimal incisions in the skin for this augmentation and there is very little local tissue reaction.
