Potency of antileukoprotease and alpha 1-antitrypsin to inhibit degradation of fibrinogen by adherent polymorphonuclear leukocytes from normal subjects and patients with chronic granulomatous disease.

We have studied the relative efficacy of antileukoprotease (ALP) and alpha 1-antitrypsin (alpha 1AT) to inhibit the degradation of substrate by polymorphonuclear leukocytes (PMN) attached onto a fibrinogen matrix. PMN elastase activity was assayed by radioimmunoassay of a specific 21-residue cleavage product from the amino terminus of the A alpha chain, A alpha (1-21), of fibrinogen. The adherence of PMN (1.0 x 10(6)) to a fibrinogen matrix was facilitated by incubation with recombinant tumor necrosis factor-alpha (1 nM). Subsequently, the cells were exposed to inhibitors before stimulation with cytochalasin B and formylmethionyl-leucylphenylalanine. Under these conditions, ALP inhibited A alpha (1-21) formation with an IC50 of 85 +/- 30 nM and alpha 1AT gave an IC50 of 220 +/- 98 nM (mean +/- SD). The effect of oxidant production on A alpha (1-21) formation was evaluated by comparing the effect of PMN from normal subjects with PMN from subjects with X-linked NADPH oxidase deficiency. Stimulation of PMN from the latter subjects in a similar fashion as described above resulted in the formation of 40 +/- 4 pmol/ml A alpha (1-21), or approximately twice the amount seen with cells from normal subjects. Preincubation with ALP or alpha 1AT in a concentration range between 10 to 900 nM resulted in an IC50 of 50 +/- 13 nM for ALP compared with 150 +/- 21 nM for alpha 1AT. Both inhibitors are more effective to prevent fibrinogen degradation caused by chronic granulomatous disease (CGD) PMN than by normal PMN despite the fact that CGD PMN generated more A alpha (1-21) than did normal PMN.(ABSTRACT TRUNCATED AT 250 WORDS)

A compound nevus in a benign cystic teratoma of the ovary.

The occurrence of a compound nevus in an ovarian benign cystic teratoma is presented. This appears to be the first report of this lesion in the ovary without coincident melanoma. The case illustrates a probable origin for ovarian melanoma and raises a question concerning the origin of the melanocyte.

Anatomical methods in the study of cardiovascular pathology: a refined technique.

A refined technique is described for the anatomical study of the heart and coronary arteries after perfusion with agar-barium sulfate contrast medium. The method includes dissection and isolation of the perfused coronary arteries with sequential cross-sectioning and radiography of intact and cross-sectioned vessels. This technique allows better evaluation of the extent of cross-luminal narrowing and improved selection of myocardium for histologic sectioning. This affords better correlation of autopsy and clinical findings than conventional methods and permits easier retrieval of data and better standardization of results for use in pathological studies.

The use of monoclonal antibodies in the diagnosis of peripheral T-cell lymphomas.

Sixteen cases of peripheral T-cell lymphomas are described which are characterized immunologically by various methods including the use of monoclonal antibodies (Ab) to a variety of lymphocyte antigens. Seven cases were classified as helper/inducer and two as suppressor/cytotoxic phenotype. Four were considered T-cell, not otherwise specified, owing to lack of expression of subset phenotypic markers. Three cases were diagnosed prior to the advent of monoclonal Ab methods and were classified as being of T-cell origin by the E-rosette technique. In none of the cases was monotypic surface immunoglobulin or B-cell antigens expressed. The freeze-dried technique to prepare tissues for immunoperoxidase studies was found to be advantageous. The benefits and limitations of monoclonal Ab in the diagnosis of T-cell lymphomas are discussed.

Blood group antigens in urothelium in transitional cell carcinoma.

A study is reported relating clinical behavior of transitional cell carcinoma of the urinary bladder to blood group substance expression on the surface urothelium using an immunoperoxidase technique. Eighty-seven biopsies from 60 patients were studied. An average follow-up of 84.7 months was obtained. Blood group substance was detected in 61 percent of low grade (I) tumors and in 41 percent of high grade (II to III) tumors. Low grade (I) tumors which exhibited the blood group substance followed a benign clinical course in 70 percent of the cases and an aggressive clinical course in 30 percent. The high grade (II to III) tumors which exhibited blood group substance followed a more benign clinical course in 54 percent of the cases whereas behavior was clinically aggressive in 46 percent. It is concluded that the presence of blood group substance on urothelial malignancies, although appearing to correlate in some degree with the aggressiveness of the disease, is not statistically significant in all tumor grades and is not an absolute finding that can be clinically used in a prospective manner to determine prognosis and treatment for the individual patient. The expression of this antigen does not appear to be statistically significant in low grade (I) lesions as previously reported; however, it may play a more important role in considering higher grade (II to III) lesions.

Necrotizing enterocolitis of the newborn.

The clinical presentation, diagnostic features, therapeutic measures, and results of treatment of 29 infants with the firmly established diagnosis of necrotizing enterocolitis are reviewed. A cohort of control patients are studied in an attempt to identify risk factors which may predispose infants to develop this serious complication of the newborn period. The incidence of necrotizing enterocolitis in our series is 1.2 per 1000 live births, similar to that reported by other investigators. The prognosis is particularly grave in infants of very low birth weight and among those who exhibit radiographic evidence of portal venous air or who develop disseminated intravascular coagulation.
