Comparison of various physiologic and psychomotor parameters in patients sedated with intravenous lorazepam, diazepam, or midazolam during oral surgery.

Intravenously administered lorazepam (0.05 mg/kg), diazepam (0.25 mg/kg), and midazolam (0.1 mg/kg) were compared for sedation during oral surgery under local anesthesia. Sixty patients were randomly allocated into three groups in this double-blind, parallel study. The results from this trial show that all three drugs provide satisfactory sedation. Average mean arterial pressures, however, decreased significantly with midazolam and diazepam. Statistically significantly higher heart rates during the entire procedure were also found for lorazepam when compared with diazepam and midazolam. At the postblock stage, the midazolam group had respiratory rates that were significantly higher than those of the other two drug groups. Patients in the diazepam and midazolam groups took significantly longer to complete the pegboard test at the preblock stage than those in the lorzepam group. At 1, 1.5, and 2 hours after arrival in the recovery room, an inversion of groups took place, with the lorazepam group taking significantly longer for their tests than the other two groups. Significantly more improvement in anxiety levels was found at 10 minutes postdrug for the patients who had received diazepam and this tended to remain so on arrival in the recovery room. When compared with the other two groups, significantly more patients in the lorazepam group reported giddiness/dizziness and significantly more in the diazepam group reported pain on injection.

Genital tuberculosis associated with female infertility in the western Cape.

Infertility is a common presenting symptom in women with genital tuberculosis. A study was undertaken to determine the prevalence and characteristics of this disease among the infertile patients (A and B income group) attending the Reproductive Biology Unit at Tygerberg Hospital. Between June 1986 and December 1987, the menstrual fluid from 451 infertile women was cultured for Mycobacterium tuberculosis using Löwenstein-Jensen medium. A prevalence of 7.98% (36/451) was found. Laparoscopic examination of the pelvis revealed bilateral tubal occlusion in 3 patients (8.3%); peritubular adhesions were present in 17 patients (47.2%). In the remaining 16 patients (44.5%), the pelvis was considered normal. No culture-positive patient gave a history of previous infection or contact with tuberculosis. Radiography of the chest and histological examination of the endometrium showed no evidence of infection. Of the 34 patients treated, 13 (38.2%) became pregnant after antituberculosis treatment was completed. If tuberculosis occurs in a population, menstrual fluid of infertile women should be cultured routinely for M. tuberculosis, especially if there is evidence of tubal damage.

Paradoxical reactions to rectal midazolam as premedication in children.

Eighty healthy children, between the ages of 2 and 7 years, who were to undergo oral surgical procedures under general anaesthesia, were allocated randomly to 4 groups. Three groups received rectal midazolam, and the other group a placebo (saline) as premedication 30 min prior to induction of anaesthesia. Group A children received midazolam 0.25 mg/kg, Group B 0.35 mg/kg and Group C 0.45 mg/kg. No statistically significant difference was found between the treatment groups as to the effect on systolic- (p = 0.6920) and diastolic (p = 0.8701) blood pressures, respiration (p = 0.0505) and pulse (p = 0.6192) rates at either pre- or post-sedation levels. However, the results indicate that levels of anxiolysis and sedation were significantly associated with midazolam dosage (p less than 0.0001).

Anaphylactoid reaction to vancomycin: report of a case.

Although vancomycin produces excellent prophylaxis against staphylococci, the danger of hypotension, whether secondary to anaphylactoid response or myocardial depression, exists. Therefore, recommendation for the use of vancomycin before dental procedures must include the precaution of administration in a dilute solution over 30 minutes, with frequent monitoring of BP and heart rate.

The relative distribution of fluoride in erupted and unerupted enamel of human third molars from a low fluoride area.

Acid-etch biopsies were taken from the centre of the mesio-lingual, disto-lingual, disto-buccal and mesio-buccal cusps of non-carious erupted (n = 16, aged 18-28 years) and unerupted (n = 21, aged 18-28 years) teeth. The donor subjects had lived continuously in a low fluoride area (F water less than 0.1 parts/10(6] from birth, and had had no systemic fluoride supplementation. Tooth brushing, mouth rinsing and the application of sealers were the only fluoride-containing anti-caries programmes practised singly or in combination. Six successive acid-etchings were taken from each of the four cusps, and the fluoride levels in the etching solution were measured with an adapted, fluoride ion-selective electrode. Up to a depth of about 10 microns, unerupted enamel was etched significantly (p less than 0.05) more deeply than erupted enamel, which might be explained by changes in its composition. Up to a depth of approximately 10 microns, significantly more fluoride was found in erupted than in unerupted enamel. This difference may mainly be attributed to the effect of fluoride from the oral environment on erupted enamel. Fluoride concentrations, up to a depth of approximately 3 microns, were (p = 0.07) higher in the buccal cusps than in the lingual cusps of erupted molars, but not at the other etch depths. There were no significant differences (p greater than 0.10) between the buccal and lingual cusps of the unerupted molars at any depth. No significant correlations between enamel fluoride concentrations and age could be found either for erupted or unerupted third molars.

Comparison of sublingual lorazepam with intramuscular diazepam as sedatives during oral surgery.

Sublingual lorazepam (2 to 3 mg) was compared with intramuscular diazepam (0.25 mg/kg) and placebo for sedation during oral surgery under local anesthesia. Sixty patients were randomly allocated into three groups in this double-blind, parallel study. The results from this trial show that sublingually administered lorazepam provided good sedation and anxiolysis. More side-effects, such as giddiness, dizziness, and ptosis, as well as profound and prolonged psychomotor impairment, were, however, found in the lorazepam group than in those patients who had received intramuscular diazepam (0.25 mg/kg) or placebo.

A comparison of labetalol, acebutolol, and lidocaine for controlling the cardiovascular responses to endotracheal intubation for oral surgical procedures.

Arterial blood pressure and pulse rate changes following tracheal intubation were studied in 80 patients undergoing oral surgical procedures who received an etomidate/suxamethonium anesthetic induction sequence. Three treatment groups and a control group were used. Intravenous labetalol (1 mg/kg), acebutolol (0.25 mg/kg), lidocaine (2 mg/kg), and saline (1 ml), injected prior to anesthesia, were compared with respect to their effect on the hemodynamic consequences of direct laryngoscopy followed by the passage of an endotracheal tube. A pre-induction dose of labetalol was found to be more effective than acebutolol and lidocaine in attenuating the pressor response to instrumentation and intubation. Labetalol should prove most useful for those patients at risk from the transient hypertension and tachycardia following instrumentation and intubation.

Intravenous midazolam in oral surgery.

Intravenously administered midazolam (0.1 mg/kg) was compared with placebo in a randomized study in 50 patients undergoing oral surgical procedures under local anaesthesia. The results obtained from this study showed that midazolam when compared to placebo had slight cardiovascular and respiratory depressant effects, diminished anxiety and caused amnesia. It also provided better operating conditions and possibly stimulated appetite.

An experimental assessment of the tampan tick Ornithodoros moubata as vector of hepatitis B virus.

Wild-caught and colonized tampan ticks, Ornithodoros moubata (Murray), were fed on hepatitis B virus (HBV)-positive blood-means in a series of four experiments. Hepatitis B surface antigen (HBsAg) persisted in nymphal and adult ticks for up to 779 days, while the epsmark antigen (HBeAg) persisted in mature nymphs up to 13 days, in adult males up to 11 days and in adult females up to 16 days. HBsAg was transmitted trans-stadially through two moults during the life cycle but transovarial transmission did not occur. The surface antigen was transmitted by two out of fifteen single ticks into 0.4 ml aliquots of HBV-negative blood, although six groups of ticks failed to transmit into 5.5 ml aliquots of blood: this antigen was not transmitted to hamsters. HBsAg was detected in samples of the ticks' coxal and rectal fluid secretions always at the infecting feed and usually at the second feed. HBeAg was only detected in one of two samples of coxal fluid collected at the infecting feed. The results as a whole indicate that no biological multiplication of virus occurs in O.moubata but that mechanical transmission from ticks to man could occur by: (i) contamination of a person when crushing infected ticks; (ii) infection by bite; (iii) contamination with coxal fluid, especially by scratching bites. This is thought to take place among the Kavango tribe in their village huts in north-eastern Namibia where infestations of infected O.moubata occur.

Blood oxygen saturation levels during conscious sedation with midazolam. A report of 16 cases.

In a double-blind randomized study on 16 healthy individuals, two groups of subjects (8 in each group) received either midazolam (Dormicum; Roche) 0.1 mg/kg or placebo intravenously for conscious sedation during oral surgical procedures. Oxygen saturation of the blood was measured at different stages. Ten minutes after administration of the drug, the percentage oxygen saturation was significantly lower (P less than 0.05) in the midazolam group than in the placebo group.

Prevalence of hepatitis virus and some arbovirus infections in Kavango, northern SWA/Namibia.

A survey of the frequency of hepatitis B markers in 258 subjects from Kavango, northern South West Africa/Namibia, was undertaken during February - May 1983. The hepatitis B surface antigen and hepatitis B e antigen carrier rates were 13,6% and 2,7% respectively. Only 1,9% of the subjects were negative for all the markers tested, indicating that infection had been present at some stage in 98,1% of the sample. No known transmission method of the virus seems to fit this high exposure rate. The possibility that arthropod transmission of the virus may play a role in the epidemiology of the disease in Kavango is being investigated. All subjects tested for antibodies to hepatitis A virus were found to give a positive result. Of the arboviruses tested for, West Nile virus was usually predominant and was probably responsible for most of the arbovirus infections. Antibodies to the Sindbis, Chikungunya, Germiston and Rift Valley fever viruses were also found.

Radiographic profile of the first cervical vertebra.

Lateral cephalographs from 220 normal adolescent white orthodontic patients, and of individuals with craniofacial dysostosis and oculodento-osseous dysplasia were studied. Variations were observed in the morphology of the posterior margins of the superior articular processes, vertebral artery canals being present in 8%. Posterior arch dehiscence was found in eight individuals, including seven cases in the midline. Three patients had accessory ossicles above the posterior arch of the atlas, and two showed evidence of fusion of the second and third cervical vertebrae. The profile of the atlas was well outside the normal range in both syndromes studied.
