[Bronchoalveolar lavage in pneumoconiosis of coal miners. Biochemical aspects].

Broncho-alveolar lavage (LBA) was performed in 17 pneumoconiotics. The liquid obtained was analysed after gentle centrifugation to remove the cellular element, so that biochemical factors might be sought contribution to the evolution and progressive transformation to fibrosis. The percentage of liquid gathered was generally greater. Among the glycosidases found in all the 17 LBA analysed, the beta-D-glucuronidase, which was not detected in the LBA control subjects, was also found during the course of other pulmonary disorders. The elastolytic activity was characterized in 12 out of 17 LBA. In part it could originate from alveolar macrophages. An elevated number of macrophages (greater than 20 X 10(6) for the whole lavage) allied to the presence of elastolytic activity was found in 7 of 8 patients presenting with a pneumoconiosis and signs of progressive pulmonary disease. The collagenase and cathepsin B were present in the LBA of certain pneumoconiotics, but the significance of their presence is still unknown. The three major antiproteases of the serum exist in the LBA of pneumoconiosis. The quantity of alpha 1-antiprotease has identified a group of 6 patients whose LBA showed raised alpha 1-antiprotease, an elastolytic activity and for 5 of them progressive outcome.

[Bifocal excision in the treatment of bronchial carcinoma revealed by cerebral metastasis (author's transl)].

Between 1964 and 1980, 17 patients operated upon for cerebral metastasis as initial presentation of bronchial cancer underwent excision of the thoracic tumour 24 to 131 days later. One patient died post-operatively of acute pneumopathy. Nine patients showed signs of recurrent cerebral malignancy after a 55 to 904 days' delay. Increase in life expectancy and regression of neurological symptoms justify this bifocal surgical strategy. Using cerebral and thoracic radiotherapy as well as chemotherapy as supplementary measures seems rational and likely to improve the present results.

Use of lectins for detection of glycoconjugates in the glandular cells of the human bronchial mucosa.

Paraffin-embedded human bronchial biopsies, obtained in macroscopically healthy areas, were examined using nine peroxidase-bound lectins. These were either isolated or purified by affinity column chromatography (Ulex europeus, Triticum vulgare, Glycine max, and Arachis hypogatea lectins) or commercial preparations (Lotus tetragonolobus, Canavalia ensiformis, Helix pomatia, Phaseolus vulgaris, and Lens culinaris lectins) and conjugated to peroxidase (except for concanavalin A). These labeled lectins were used as specific molecular probes to localize differences in carbohydrate-containing components present in the different types of glandular cells of human bronchial mucosa. The choice of fixative was crucial and tests used for this study have shown that Carnoy's solution seems to be the most appropriate solution to preserve mucous glycoproteins in situ. Comparison of the affinity of several lectins for human bronchial glycoproteins and for bronchial mucosa demonstrates the predominance of serum-type glycoproteins in serous cells of the submucosal glands and mucin-type glycoproteins in mucous cells of the submucosal glands and in goblet cells of the bronchial epithelium. Furthermore the data obtained with some lectins, such as Helix pomatia agglutinin, suggest that there are some differences in the mucins synthesized by goblet cells and by mucous cells.

[Bronchial secretions: biochemical study. Physiopathological aspects].

Among the chief constituents of bronchial secretions, serum or mucus proteins and mucins are the parameters enabling definition of the functional state of the tracheo-bronchial mucosa. The use of bronchial detergents in normal subjects or subjects with moderate secretions leads to the study of physiological sputum or to definition of the evolution of chronic bronchitis. Results of immunoglobulins G and A dosages, values of IgA 7S/IgA 11S and of IgA/serum albumin ratio are reported. In the same way, the equilibrium of secretion of mucins estimated by the percentage of the different classes of mucins separated according to their acidity degree is an evaluation element of the evolution degree of chronic bronchitis. The studies of correlation between rheological properties and chemical structure of bronchial secretions are reported. They postulate an original concept on the use of mucolytics.

[Rheumatoid pleurisy. Apropos of 13 cases].

The extra-articular manifestations of RA are numerous and various. Among the latter, pleural involvement is not exceptional, but it is often overlooked for it may be latent. There have been few recent studies of this relationship. The authors report 13 cases of rheumatoid pleurisy collected over the last 7 years in rheumatology and chest units. They recall the frequency of pleurisy, their constitutional background, their clinical and radiological appearances and emphasise the diagnostic interest of a biochemical study of the pleural fluid: a fall in the glucose content and of complement levels are the main changes. A study of the complement system carried out in 11 patients showed a fall in total complement and of the C3 and C4 fractions in the pleural fluid, which shows local consumption of immunological origin. This factor and the clinical picture may be important in diagnosis. The course of these pleural effusions is favourable, complications are exceptional; their treatment is mainly the basic treatment of RA.
