Upper airway obstruction and craniofacial morphology.

Otolaryngologists are being asked with increasing frequency to assess adequacy of the upper airway and to treat upper airway obstructive problems in orthodontic patients. The incentive has been provided by recent studies that purport to relate upper airway obstruction to dental and craniomorphologic changes. It is hypothesized that prolonged oral respiration during critical growth periods in children initiates a sequence of events that commonly results in dental and skeletal changes. In the chronic mouth-breather excessive molar tooth eruption is almost a constant feature, causing a clockwise rotation of the growing mandible, with a disproportional increase in anterior lower vertical face height. Such increases in anterior lower vertical face height are often associated with retrognathia and open bites. Low tongue posture seen with oral respiration impedes the lateral expansion and anterior development of the maxilla. Otolaryngologists have the ability to objectively and accurately assess upper airway patency. Rhinometric assessment before and after application of topical nasal decongestant, in conjunction with clinical examination, provides valuable information regarding upper airway patency and the cause of any existing obstructive pathologic condition. Studies should be designed carefully to control the numerous variables that have an impact on the growing face of a young child so that meaningful data can be obtained in our own field regarding this challenging topic.

Cryosurgical treatment of the lymphoid tissue of Waldeyer's ring.

The role of cryosurgery in the treatment of tonsil disease is an unresolved issue. Initial favorable reports failed to provide the necessary incentives for larger well-controlled studies. Bacteriologic and immunologic response to the cryosurgical treatment of tonsils remains unclear. Without appropriate studies, cryosurgery has been relegated to a backup status for conventional tonsil surgery. At present, cryogenic treatment of lymphoid tissue is generally limited to tonsillectomy in patients with blood dyscrasias; elimination of palatine tonsil regrowth and posterior pharyngeal lymphoid hyperplasia; and occasional laryngeal and lingual tonsillectomy.

Pediatric nasal resistance and lower anterior vertical face height.

In an effort to determine whether nasal obstruction in the pediatric population influences craniofacial and orthodontic development, nasal resistance data were collected from 211 consecutive orthodontic patients by means of active anterior rhinometry. Age-specific nasal resistance standards had been previously accumulated from an unselected representative sample of 498 youngsters, drawn from socioeconomic and demographic backgrounds similar to those of the orthodontic group. Resistance data collected from 211 orthodontic patients were found to fall in an almost linear arrangement, varying inversely with age. Age-specific nasal resistance mean data were found to be consistently higher than comparable age-specific mean data of the unselected comparative sample. When orthodontic nasal resistance data were compared with cephalometric measurements of anterior lower vertical face height in 60 patients, there was a definitive correlation between elevation in nasal resistance and increased lower vertical face-height measurements.

Pediatric nasal resistance.

Rhinometry has provided a quantitative and objective means of assessing nasal airway patency. Previous investigators have established normal nasal resistances for adults and newborn infants. Less material and no such normal references are available for the pediatric ages. In this study, nasal airway resistance is assessed by anterior rhinometric technique in 498 children ranging from four to 16 years of age. Collected nasal resistance data are found to vary inversely with age and fall in an almost linear fashion between already established normals of infancy and adulthood. As far as we can determine, pediatric age specific nasal resistance data with calculated standard deviations and 95% confidence levels have not been previously reported. Such data provides initial age specific reference points enabling the rhinologist to more objectively evaluate concerns regarding nasal obstructive problems in children.

Chronic vasomotor rhinitis: cryogenic and other surgical modes of treatment.

Cryosurgical treatment of chronic vasomotor rhinitis provides a safe, effective and uncomplicated mode of management for this very common otolaryngologic disorder. Data relating to three hundred fifty patients is presented with particular reference to patient selection criteria, cryosurgical techniques and results.

Teaching international nursing students: challenges and strategies.

Cross-cultural education presents many challenges and focuses attention on the significant differences in teaching and learning styles around the world. In 1995, the George Mason University College of Nursing and Health Science embarked on the Saudi-US University Project, an effort that brought these theoretical differences to life. The stark contrast between the Saudi Arabian and Western cultures provided administration, faculty, and students the opportunity to develop creative strategies to accommodate learning across cultures.

Biofeedback training and relaxation exercises for treatment of temporomandibular joint dysfunction.

Biofeedback training involves electromyographic recording of the patient's level of facial muscle tension with concurrent presentation of that level to the patient in a visual or auditory manner. The objective of biofeedback training in patients with temporomandibular joint dysfunction is to provide them with an insight into abnormal facial muscle activities, which include clenching and grinding of the teeth and lip- and cheek-biting habits. This insight coupled with relaxation programs affords the patient an opportunity to eliminate abnormal behavior patterns that foster temporomandibular joint symptomatology.
