Surface expression of major membrane glycoproteins on resting and TRAP-activated neonatal platelets.

Platelets of full-term newborns and those of healthy adult donors were compared for constitutive expression of surface glycoproteins (GP) Ia-IIa, GP Ib, GP IIb-IIIa, and GP IV and for their activation responses to an agonist by detection of surface expression of activation markers P-selectin and CD63. Resting neonatal platelets showed significantly lower expression of GP Ia-IIa, GP Ib, and GP IIb-IIIa. In contrast, the expression of GP IV was significantly higher compared with platelets of adults. The expression of activation markers P-selectin and CD63 was assessed after in vitro activating of platelets with 0-15 microM human thrombin receptor-activating peptide. At low concentrations of thrombin receptor-activating peptide, the extent of surface expression of activation markers did not differ significantly between adult and neonatal platelets. However, after activation with 15 microM thrombin receptor-activating peptide, the extent of surface expression of P-selectin and CD63 was significantly lower in neonatal platelets. Because of ethical reasons, our study was conducted on neonates with a moderate neonatal hyperbilirubinemia. The remote possibility that hyperbilirubinemia could influence the expression of platelet surface receptors and the reactivity of neonatal platelet cannot be excluded. The role of higher expression of GP IV on neonatal platelets, also seen in certain hematologic malignancies in adults, remains to be elucidated. The lower expression of platelet adhesive receptors and the limited ability to up-regulate granular glycoproteins may play a role in the impairment of function of neonatal platelets.

[Delayed surgery in congenital diaphragmatic hernia without drainage of the ipsilateral hemithorax].

The objective of the investigation was to evaluate the success of a new therapeutic protocol in patients with congenital diaphragmatic hernia (CDH). During the period from 1/1994 till 12/1998 41 patients with CDH were admitted. In 36 patients (88%) left-sided CDH was diagnosed, in 4 patients (10%) right-sided CDH and one neonate (2%) bilateral CDH. Fifteen cases (37%) of CDH were assessed prenatally. Twenty-two children (54%) were treated by inhalation of nitric oxide (INO) and 4 patients (10%) by extracorporeal membrane oxygenation. The total incidence of associated developmental defects was 20% and the total mortality 34%. On comparison of the surviving (group S, n = 27) and the non-surviving NS, n = 14) patients statistically significant differences were found in the Apgar score during the first minute (S: 5.9 +/- 0.5 vs. NS: 3.4 +/- 0.7, p < 0.008), in the oxygenation index (OI) two hours after admission (S: 11.9 +/- 2.9 vs. NS: 27.7 +/- 8.3, p < 0.03), in the alveolo-arterial oxygen difference (AaDO2) 2 hours and 12 hours after admission (S: 369 +/- 47 torr and 237 +/- 47 torr resp. vs. NS: 552 +/- 29 torr and 557 +/- 26 torr resp., p < 0.02) and in the need to start extracorporeal membrane oxygenation (S: 3.7% vs. NS: 21.4%, p < 0.009). The investigation confirmed a reduced mortality of neonates with CDH by introducing new therapeutic methods. Risk factors are early prenatal diagnosis, the presence of associated developmental defects, high values of oxygenation and ventilation with the necessity to start nitric oxide inhalation.

[Prenatal diagnosis and therapy of congenital diaphragmatic hernia].

The diagnosis and treatment of congenital diaphragmatic hernia, CDH, is an interdisciplinary problem. The patient concentration with prenatally diagnosed CDH for comprehensive examination with subsequent decision on the type of delivery improves the prognosis of patients with CDH. Introduction of corticoid therapy in prenatally assessed CDH reduces dysfunction and surfactant deficiency and can reduce the degree of respiratory failure during delivery. In very early prenatally assessed CDH it is possible to consider the possibility of combined corticoid and TRH (thyroxin releasing hormone) treatment of CDH. The authors submit also contemporary possibilities of prenatal intervention treatment of CDH (indication criteria for reconstruction operations of foetal surgery, intrauterine closure of the trachea and artificial laparoschisis). The concentration of patients with postnatally diagnosed CDH and a severe grade of acute respiratory insufficiency in a department with conventional and non-conventional artificial pulmonary ventilation incl. inhalation of NO oe extracorporeal membrane oxygenation, ECMO, is a further step towards optimation of treatment. Continuous evaluation of parameters of pulmonary functions during the pre- and postoperative period may prove that a therapeutic protocol with delayed surgery is useful.

Antimycobacterial activity of some 2,3-dianilinoquinoxaline derivatives.

The antimycobacterial activity of 2,3-dianilinoquinoxaline derivatives was studied and the substances were found to be mostly wide-spectrum antimycobacterial agents. Replacement of one nitrogen atom in the heterocycle for oxygen or sulfur was usually accompanied by loss of activity. The model structure for the study was the wide-spectrum antimycobacterial drug clofazimine.

[External control of drug sensitivity determination in mycobacteriologic laboratories in the Czech Republic].

In 1992 and 1993 an external control of sensitivity assessment of coded strains of M. tuberculosis to five basic antituberculotics was made: isoniazide, streptomycin, pyrazinamide, ethambutol and rifampicin. In 1992 from 11 participating laboratories an erroneous result was recorded in 9 (3 laboratories made two mistakes, 6 laboratories one mistake), two laboratories did not provide complete results. Unsatisfactory results were obtained during external controls in 1993. Of 15 participating laboratories four laboratories made one mistake, two laboratories two mistakes, four laboratories three mistakes. Five laboratories (i.e. one third) made more than three mistakes. Analysis of the results revealed a low reproductibility of results of drug sensitivity tests. To eliminate these shortcomings it will be necessary to make systematic external controls with subsequent solution of the shortcomings, restriction of the number of laboratories making these examinations to those who will systematically produce correct results. And it will be also necessary to test the sensitivity tests to antituberculotics in resistant strains in the National reference laboratory for mycobacteria.

[Detection of mycobacteria in pathologic material using the radiometric method].

For concurrent detection of mycobacteria by the radiometric method BACTEC and the standard cultivation technique 96 samples of pathological material from patients hospitalized with suspected or confirmed tuberculosis was used. The total number of positive samples was 30, using the BACTEC method only, four samples and using the cultivation method only, 10 samples. The assessed difference was not statistically significant. The mean period of positivity with the BACTEC method was 20.4 days, in cultivations 28.8 days. The higher yield of the cultivation method could be caused by various factors. For the BACTEC system inocula of 0.5 ml were used and one cultivation bottle, for the cultivation inocula 1.0 ml and four test tubes for one sample were used. From five samples, positive only on cultivation, only sporadic colonies were obtained after 42-63 days cultivation and in all instances only on one of four media.

Characterization of a monoclonal antibody to Mycobacterium kansasii.

The characterization of an IgG2/Kappa monoclonal antibody against a protein antigen obtained by gel chromatography of the complex sonicate of M. kansasii biomass is described. It recognized specifically epitope molecules in complex and fractionated M. kansasii antigens ranging from 14.4 to 20 kD. The 20 kD immunoreactive band is common to the complex antigen, its fraction A and to the fraction B which was employed as the immunogenic agent. The 14.4 and 18 kD bands are predominantly present in the fraction B. High dilution of this MoAb (1:200,000) employed successfully in immunoblot studies suggests an antibody of very high affinity which could be useful for affinity purification of mycobacterial antigens in serodiagnosis of mycobacterial infections and in demonstration of antigens in biological materials.

Effects of 90% oxygen exposure on the ultrastructure of the tracheal epithelium in rabbits.

Numerous signs of pathological alteration were induced in the pseudostratified ciliated epithelium by a 2-hour exposure to 90% oxygen. The ciliary border injury was especially conspicuous, the mean number of cilia in a given area was significantly (p less than 0.005) reduced and morphological signs of impaired self-cleaning ability were revealed. Ciliated cells were severely injured showing apical blebbing and swelling of cell organelles. The reaction of goblet cells to exposure to O2 was rapid discharge of mucus, but the mechanism of mucus evacuation was not substantially influenced. At the ultrastructural level, only a mild effect of humidification of the oxygen was observed.

Stimulating effects of a weak constant magnetic field (0.8 Gauss) on the growth of two mycobacterial phages no 308 and 33d.

A liquid culture of ATCC 607 infected with two mycobacterial phages and exposed to constant magnetic fields 0.8 G for 48 hours was replanted on simple agar media without oleic acidalbumin enrichment. In both phages a pronounced stimulating effect was observed demonstrated in the phage No. 308 by almost confluent lysis in magnetized culture and in the phage 33 D by uncountable lytic plaques. Magnetized phage cultures could be preferably used for preparing rich stock suspensions also from poorly growing phages needed for phage classification of mycobacteria.

[The survival of D-29 and MyF2 P/59-mycobacteriophages in continuous cultures].

The cultivation technique of two mycobacteriophages (D-29 and MyF2 P/59) in continuous cultures with a simple synthetic medium is described. The ATCC-607-strain (M. smegmatis) was used as a host strain. The medium was exchanged every 24 hours for 14 days, then the whole cultivation equipment including waste bottle was hermetically closed and preserved in a thermostat for seven years. Every six months about 250 ml of new liquid medium was added into the cultivation container after finishing the passage cultivation. Both mycobacteriophages could be found out still after 6 years in the mixture of the mycobacteriophages preserved in the waste bottle and in the culture container even after seven years. The possibility of using the technique of continuous cultivation of phages for preparing fresh 24 hours old suspensions necessary for phage typing of the mycobacteria is discussed.
