Placental alkaline phosphatase immunoreactivity in testicular germ-cell neoplasms.

We analyzed the sensitivity of a polyclonal antibody to placental alkaline phosphatase (PLAP) in labelling testicular germ-cell neoplasms, by utilizing the peroxidase-antiperoxidase technique. The immunoreactivity of 89 germ-cell tumors for PLAP was as follows: 98% of cases with seminomatous elements were PLAP-positive; 97% of embryonal carcinomas and 85% of endodermal sinus tumors also showed reactivity. Cytotrophoblastic cells were focally immunoreactive in one of two cases with choriocarcinomatous elements. Staining for PLAP was strongest and most diffuse in seminomas. Intratubular germ-cell neoplasia (ITGCN) or carcinoma in situ was present in 53 (84%) of 63 specimens that had adjacent seminiferous tubules available for evaluation; PLAP was demonstrated in 98% of these. In addition, the germ-cell elements in 11 gonadoblastomas were immunoreactive for PLAP. One of 17 cases of undescended testes had ITGCN that was also strongly immunoreactive, but the remaining 16 cases were negative. Five dysgenetic gonads without ITGCN were studied, and one was immunoreactive for PLAP. Three testicular biopsy specimens from infertile men without ITGCN were PLAP-negative. Our findings indicate that PLAP is a highly sensitive marker for seminomas, for the majority of embryonal carcinomas and endodermal sinus tumors of the testis, and for ITGCN.

Collagenous colitis: physiologic and histopathologic studies in seven patients.

Collagenous colitis is a clinicopathologic syndrome with chronic watery diarrhea, diffuse colitis with surface epithelial injury, and a distinctive collagen band beneath the surface epithelium especially in the proximal colon. The cases of seven patients (including six middle-aged women) with chronic, watery, noninfectious diarrhea were studied. Roentgenographic and endoscopic findings were not diagnostic. Two patients had rectal mucosal inflammation but sparing of the distal colon from subepithelial collagen. Other findings included thyroid disease (four patients), urethral fibrosis (three), elevated erythrocyte sedimentation rate (six), and eosinophilia (three). The colon was thought to be the main source of diarrheal fluid, but bile salt malabsorption, steatorrhea, and net small-bowel secretion were additive factors in some patients. With antiinflammatory treatment the diarrhea abated, the surface epithelial injury decreased, and the subepithelial collagen resolved (two patients), but lamina propria inflammation persisted. Collagenous colitis seems to be a chronic systemic, and perhaps autoimmune, disorder.

Allergic angiitis and granulomatosis (Churg-Strauss syndrome): report of a case with massive thymic involvement in a nonasthmatic patient.

A 14-year-old nonasthmatic boy who presented with multiple skin lesions and a mediastinal tumor was found to have allergic angiitis and granulomatosis (AAG) (Churg-Strauss syndrome), with prominent involvement of the thymus. The occurrence of AAG in the thymus merits recognition because of possible confusion with a variety of other lesions characterized by proliferation of histiocytes and tissue eosinophilia that affect this organ. In addition, this case illustrates that the absence of asthma should not be used as a definitive criterion for exclusion of the diagnosis of AAG.

Pseudosarcomatous changes in inflammatory pseudopolyps of the colon.

The occurrence of pseudomalignant changes in biopsy specimens from two patients with ulcerative colitis and pseudopolyposis is described. One lesion was characterized by the proliferation of large ganglion cell-like cells, similar to those observed in proliferative fasciitis. Ultrastructural and immunohistochemical findings supported a fibroblastic origin. Biopsy specimens of an inflammatory pseudopolyp from the second patient showed proliferation of oval to spindle cells that were initially interpreted as a possible neoplasm. The site of origin and reactive nature of this lesion became apparent on a subsequent polypectomy specimen. Attention is called to the occurrence of pseudosarcomatous changes in inflammatory pseudopolyps of the gastrointestinal tract that may lead to an erroneous diagnosis of malignancy.

Outgrowing schwannomas arising from tympanic segments of the facial nerve.

PURPOSE: Although there have been numerous reports of clinical cases of facial nerve schwannoma, histopathologic studies of its early stages of development are rare. The purpose of this study was to describe the incidence, origin, and pathology of small, asymptomatic schwannomas of the facial nerve. MATERIALS AND METHODS: Fifteen hundred twenty-six ears from the human temporal bone collection of the Otitis Media Research Center Otopathology Laboratory at the University of Minnesota were studied under light microscopy. RESULTS: Twelve bones with morphologic indications of type A Antoni tissue that mimics facial schwannoma in the tympanic cavity were selected. The 12 ears were from nine individuals with asymptomatic histories. The twelve schwannomas developed from perineurium at dehiscences in the facial canal near the oval window, forming defects in the epineurium by growth of Schwann cells downward and outward toward the stapes. No obvious compressions or lesions within fibers in the facial nerve were observed. CONCLUSION: Because these tumors grow slowly without hearing loss or facial nerve symptoms, they may be mistaken for granulation tissue during surgery for chronic otitis media.

[Cholesterol biliary lithiasis (risk factors)].

The prevalence of gallstone disease in the world is heterogenous, it exist a wide range between different geographic areas, sex and age groups. Its distribution seems to be influenced by various factors such as age, sex, socieconomic class, genetic and ethnic influences and diseases like diabetes mellitus and obesity. Therefore, in this study, we will analyse the importance of each of those factors in the development of cholesterol gallstone disease.

[Prevalence of adenomatous and hyperplastic polyps of the colon in the population of the General Hospital of Mexico. An autopsy study].

A prospective survey of 450 autopsies was conducted to determine the prevalence of colorectal dysplastic polyps in the population of the General Hospital of Mexico City. The colon and rectum were removed and examined for the presence of polypoid lesions; their size, location, and appearance were described. All of the lesions were examined and classified according to their microscopic diagnosis. Results revealed a single polypoid lesion in nine cases; four were classified as regenerative or inflammatory and five as dysplastic. Four of the latter were in females and one in a male; their mean age was 64 years. Macroscopically four were pedunculated and one sessil; all measured less than 0.6 cm; two were located in the transverse colon and three in the left colon. No hyperplastic polyps were found. After standardization for age, comparison with similar studies performed in other countries showed an extremely low prevalence rate for dysplastic colorectal polyps (2.7%) in our patient population, similar to one reported in Iran and higher than that found in the African Bantus, but it is one of the lowest when compared with European and US series.

Primary cardiac lymphoma treated with orthotopic heart transplantation: a case report.

Primary lymphoma of the heart is an extremely rare tumor. It is associated with a high mortality rate because of the advanced stage of myocardial involvement at initial presentation. Conventional surgical and medical treatments have not met with much success. This article reports the first case of primary cardiac lymphoma treated with orthotopic heart transplantation. The patient had hemodynamically significant severe acute rejection, neutropenic sepsis, and tumor recurrence during the late postoperative period, illustrating the difficulties associated with concomitant immunosuppression and tumor chemotherapy. Unfortunately, the patient died after a period of remission. Our approach to the integration of immunosuppression with chemotherapy, posttransplantation follow-up, and complications is discussed.
