Management of pressure sores. Comparative study in medical and surgical patients.

During a five-month period, a study was performed on medical and surgical floors of 36 beds each to evaluate the effectiveness of a structured multidisciplinary approach to prevention and treatment of pressure sores. Each pressure sore was evaluated by site and degree of severity. Follow-up was made three months later. The results indicate general improvement in patients on both floors, independent of site and severity of sores. During the study, the incidence of pressure sores per week decreased from 12% (surgical) and 29% (medical) to 0%, and the prevalence decreased from 69% (surgical) and 51% (medical) to 0%. On follow-up, prevalence remained low, at 16% (surgical) and 8% (medical). Interestingly, the more severe pressure sores were absent. An understanding of the role of intrinsic and extrinsic factors in production and evolution of pressure sores is essential. Control of these factors is the basis of effective prevention and treatment.

Renal hydatid cysts: communicating with collecting system.

The clinical and radiological features of five patients with renal hydatid disease communicating with the collecting system are described. Rupture into the renal pelvis of a hydatid cyst gives rise to acute flank pain followed by voiding of scolices or daughter cysts with or without hematuria and/or urinary obstruction. Intravenous urography demonstrates a renal mass frequently involving the collecting system; arteriography shows it to be avascular; sonography reveals a multicystic lesion of mixed echogenicity. When daughter cysts are present, computed tomography establishes the diagnosis.

Complications from irradiation of carcinoma of the uterine cervix.

The types and frequency of complications in 2248 patients irradiated for carcinoma of the cervix using different techniques are reported. Proctitis was the most frequent complication. Severe complications, such as fistulas, ureteral stenosis with hydronephrosis, enterocolitis, sigmoiditis, intestinal obstruction and skin necrosis occurred in 81 patients (3.6%). Only 36 patients (1.6%) developed fistulas.

Malignant tumors in Cuban children. Fourth triennial 1973-1975 of the national cancer registry.

The incidence of malignant tumors in children is not as high as that in adults, though in Cuba and other countries they are the most frequent causes of death in this age group. Data of the fourth triennial of the Registro Nacional del Cáncer (Cuban Cancer Registry) in the period 1973-1975 are presented. In the age group 0-14 years, 1038 new cases were diagnosed and registered; this figure represented 2.5% of the total of tumors in the period studied; the mean annual rate was 11.3% per 100 000 population. Male patients were more frequently affected. Morbidity decreased in 0-4 year group and increased in 10-14 year group. As in the first, second and third triennials, malignant diseases as leukemia, lymphoma and tumors of the nervous system prevailed among children: these accounted for a little more than 2/3 of the total of tumors followed by tumors of the kidney and eye.

Predicting improvement in stroke patients referred for inpatient rehabilitation.

An historical prospective study of prediction of improvement and final disposition of 105 patients with a stroke was carried out over a 2-year period in the rehabilitation service of a hospital providing long-term care. Patients were referred a mean of 37.8 days after the stroke, and were evaluated for total function and for mental status, perception, communication and motor ability at the time of admission and every 2 to 3 weeks thereafter. At the time of admission 26% of the patients were able to care for themselves; at the time of discharge 59% were able to do so, but 44% of these could not return home, primarily because of unfavourable social and environmental circumstances. In contrast, 35% of the patients unable to care for themselves went home because their families were willing to provide extra care. Neither the total function score nor the neurologic subtest scores at the time of admission predicted improvement. The presence of sphincter control and a lower age were the only significant predictors of improvement.

Carcinoma of the uterine cervix. Results of treatment in 2 248 cases.

Data on the survival of 2 248 patients with cervical carcinoma, treated between1966 and 1972 at 3 oncologic centers in Cuba, are presented. Intracavitary radium application combined with external 60Co irradiation was the main treatment method. The most frequent complication was radiation proctitis, but fistulas occurred only in 1.6 per cent.

Pressure sores. Prevention and step-up management.

The tissue changes that produce pressure sores may be described as occurring in five stages: blanching hyperemia, nonblanching hyperemia, blister and eschar formation, clean ulcer, and infected ulcer. Tissue breakdown is a direct response to external pressure, friction, or shearing force. Prevention of pressure sores comes down to identifying intrinsic and extrinsic risk factors and taking all possible steps to circumvent them. A "step-up" approach to management takes its name from the components surface agents, thermal agents, exposure, ultraviolet light, and pressure redistribution.

Evaluation of stroke disability.

The disabilities resulting from a stroke are not well understood from the epidemiologic or functional point of view. The stroke may impair mental status, perception, sensation, communication and motor ability; the total resulting disability is related to the extent of impairment in each of these areas. A complete evaluation in all these areas has to be done to determine the degree of disability before any rehabilitation program is planned. A comprehensive approach to evaluating stroke disability is presented that includes correlating the degree of impairment in each of the above-mentioned areas with the overall functional ability of the patient.
