Evidence that postoperative fibrinolytic shutdown is mediated by plasma factors that stimulate endothelial cell type I plasminogen activator inhibitor biosynthesis.

Postoperative fibrinolytic shutdown has been attributed to an increase in plasma levels of type I plasminogen activator inhibitor (PAI-1) activity and may contribute to postoperative venous thrombosis. The purpose of this study was to determine whether the postoperative increase in PAI-1 is contributed to by a plasma mediator(s) that stimulates PAI-1 synthesis and secretion by vascular endothelium. Plasma samples collected from patients (N = 11) before and after surgery for total hip replacement were (1) assayed for endogenous plasma PAI-1 antigen and activity, and (2) incubated with cultured human umbilical vein endothelial cells (HUVECs) and PAI-1 antigen and activity measured in the conditioned medium (CM). Eighteen hours after surgery, endogenous plasma levels of PAI-1 antigen and activity were increased by 225% (P = .003) and 190% (P = .04), respectively over the preoperative values. In addition, compared with preoperative plasma, postoperative plasma increased HUVEC secretion of PAI-1 antigen and activity by 99% (P = .001) and 66% (P = .002), respectively. This increase in HUVEC PAI-1 secretion reflects an increase in PAI-1 mRNA expression and protein biosynthesis as confirmed by metabolic radiolabeling, immunoprecipitation, and Northern blot analysis. Ultra-filtration experiments indicate that the postoperative plasma mediator(s) that stimulates HUVEC PAI-1 biosynthesis is in a molecular weight (MW) range of approximately 30 to 100 Kd. Heat treatment (56 degrees C; 30 minutes) of postoperative plasma abolished the induction of HUVEC PAI-1 production. Enzyme-linked immunosorbent assay and immunoneutralization experiments indicate that tumor necrosis factor-alpha (TNF alpha) and interleukin-1 alpha (IL-1 alpha) do not contribute to the postoperative plasma effect on HUVEC PAI-1 synthesis. These observations demonstrate that postoperative patient plasma contains a factor(s) that may stimulate endothelial cell PAI-1 biosynthesis in vivo and thus mediate postoperative fibrinolytic shut-down.

[In vitro bactericidal effect of a piperacillin-pefloxacin combination against isolated strains of nosocomial pneumopathies in intensive care].

Combinations of piperacillin and pefloxacin are evaluated in vitro using checkerboard and killing-curves methods against 19 clinical isolates: S. aureus, K. pneumoniae, K. oxytoca, C. freundii, P. stuartii, Ps. aeruginosa, Ps. maltophilia and Ac. anitratum. The static checkerboard method showed more synergistic antibacterial effects than the killing curves method. The best synergistic effects appeared against S. aureus, K. oxytoca, P. stuartii, Ps. aeruginosa Ac. anitratum, including a notable increase of the rapidity of bactericidal activity. The combination PIP 64 micrograms - PEF 4 micrograms prevents the frequent secondary regrowth seen after 6 hours with the antibiotics used alone. No activity is obtained against S. aureus meti-R and Ps. maltophilia.

[Pharmacokinetic and clinical investigation of amoxicillin administered intravenously in the newborn ].

Neonatal antibiotherapy involves special requirements which justify carrying out a clinical and pharmacokinetic investigation of amoxicillin. Our study concerns 34 newborn babies prone to infection divided in 3 groups: 26 newborns at gestational age; 6 hypotrophic and slightly premature newborns (age: 34 at 37 weeks); 1 highly premature baby (28 weeks, 0,9 kg). After direct IV drip of 33 mg/kg, 4 plasma concentrations analyses at time 0,5 1, 3, 8 hours, as well as an analysis of the urine from 0 to 8 hours are carried out by method of disk diffusion in nutrient agar. The main pharmacokinetics parameters are the following: half-live elimination time: 5.l4 +/- 3.43 hours; volume of distribution: 1.38 +/- 1.13 l/kg; total clearance: 0.211 +/- 0.277 l/kg/h. There is no correlation between birth weight and the parameters studied. However in the case of hypotrophic baby the volume of distribution is higher than for the child born after full gestation and may be partly correlated with the weight at birth. The same holds for clearance (correlation index of 0.66 and 0.59 respectively). The extreme variability of the pharmacokinetics parameters for newborn babies leads the authors to recommend a minimum dose of 150 mg/kg per day spread over 3 or 4 injections per day.

Mucinous adenocarcinoma of prostate.

Primary mucinous adenocarcinoma of the prostate is rare. The presence of mucin in prostatic carcinoma is usually associated with decreased tumor aggressiveness and increased survival rates. When mucinous adenocarcinoma of the prostate is found, it is necessary to exclude extraprostatic primary sources particularly from the urinary bladder and the gastrointestinal tract.

Treatment of recurrent and widespread testicular tumor by radical reductive surgery and multiple sequential chemotherapy.

There were 37 patients with stages II and III testicular tumors treated by a combination of multisequential chemotherapy and radical reductive surgery. Two protocols were used: 30 patients received bleomycin, vinblastine, platinum, doxorubicin hydrochloride, cyclophosphamide and actinomycin D (protocol A), and 7 patients received bleomycin, vinblastine, vincristine, platinum and actinomycin D (protocol B). A complete clinical remission was achieved in 19 of the 37 patients (51.3 per cent), for an average survival of 12.9 months to date. A partial clinical remission was obtained in 7 patients (18.9 per cent), for an average survival of 14.2 months. Eleven of the patients (29.7 per cent) escaped from therapeutic control. The collective response (complete and partial clinical remissions) represented 70.2 per cent of the patients (26 of 37), with an average survival of 14.8 months. A new approach for the simultaneous excision of metastases in the abdomen and the chest is described and the rational basis for the combination treatment of these tumors is discussed.
