Encrusted cystitis and pyelitis.

Encrusted cystitis (EC) and encrusted pyelitis (EP) are rare chronic inflammatory diseases of the bladder and renal pelvis, respectively, and are characterized by mucosal inflammation with deposits of ammonium magnesium phosphate on the urothelium. Corynebacterium urealyticum is the pathogen responsible in the vast majority of cases. We report 4 cases of EC and 1 case of EP. In 1 case of EC Ureaplasma urealyticum was isolated as the microorganism responsible. To the best of our knowledge, U. urealyticum-induced EC has never been reported previously.

Leiomyoma of the bladder: a case report.

A case of leiomyoma of the urinary bladder in a 20-year-old man is reported. The patient presented with severe frequency and perineal burning at the end of urination along with microscopic haematuria. Enucleation of the tumour was performed because transurethral biopsy revealed leiomyoma. The prognosis of these tumours is excellent.

Evaluation of needle biopsy in the diagnosis of prostatic carcinoma in men with prostatic intraepithelial neoplasia.

Prostatic intraepithelial neoplasia (PIN) is considered a premalignant lesion of the prostate. It is often encountered in prostate needle biopsy in cases where no cancer is identified. In order to evaluate its importance 25 patients with PIN in a former prostate needle biopsy underwent a second ultrasound guided needle biopsy. The first biopsy was performed in all patients as a result of positive DRE. In 13 patients (52%), prostate cancer was identified in the second specimen. All presented with high or intermediate grade PIN in the first biopsy. PSA values were compared with PIN grade and cancer presentation in the second biopsy, although no statistically significant difference was proven. In conclusion, when PIN is discovered in prostate needle biopsy in patients with positive DRE, a second biopsy has to be performed in order to exclude the possibility of a prostate carcinoma.

Extracorporeal shockwave lithotripsy or extracorporeal piezoelectric lithotripsy? Comparison of costs and results.

One thousand patients with renal stones up to 3 cm in diameter were divided equally into 2 groups matched for age, sex and stone size. One group was treated on the Dornier HM3 lithotriptor and the other on the EDAP LT01. The results showed that for stones less than 1 cm both lithotriptors were equally effective, with a stone-free rate of 87.5% for the Dornier and 90.4% for the EDAP at 3 months. The success rate fell more steeply for the EDAP machine, however, to become 77.2 and 42.5% respectively as the stone size increased to 3 cm. Treatment time was longer on the EDAP lithotriptor and more sessions were required but patients preferred it to the Dornier. Running costs per patient were higher on the EDAP.

Value of the prostatic LDH levels in the diagnosis of bacterial prostatitis.

Fifty-four patients aged between 21 and 50 years with typical symptoms of chronic prostatitis and 10 healthy men serving as control group had prostatic-fluid cultures according to the Meares-Stamey technique. Prostatic-fluid LDH levels were also estimated by agar gel electrophoresis. Eight patients were found to have bacterial prostatitis and significantly elevated LDH5/LDH1 isoenzymes ratio, 24 patients fulfilled the criteria for nonbacterial prostatitis and 22 patients were classified as suffering from prostatodynia. The LDH5/LDH1 isoenzymes ratio was found to decrease gradually as the inflammatory elements of the prostatic fluid reduced with the lowest value detected in the control group.

Methodology, results and complications in 2000 extracorporeal shock wave lithotripsy procedures.

Since the installation of an extracorporeal shock wave lithotriptor (ESWL) unit 12 months ago, 2000 lithotripsy procedures have been performed. Overall, 68.4% of patients were stone-free by the end of the first month. Residual lithiasis (fragments less than 0.5 cm) was seen in 19.7% of patients and significant lithiasis (fragments greater than 0.5 cm) was observed in 10.1%. Complications were obstruction, infection, headache due to the epidural anaesthesia, anuria, renal failure, perirenal haematoma and cardiac arrest. A number of patients required additional operative treatment; this was always endoscopic and no open procedures were performed. Hospitalisation averaged 1 day.

Absorbability of mucosa in total replacement of urinary bladder with jejunum. An experimental study.

The absorption of urine components in jejunum substitute for the urinary bladder was studied experimentally in dogs. For this purpose, determination of the blood urea, creatinine, and electrolyte levels as well as measurements of the radioactivity in blood samples after intravesical instillations of radioactive iodine, were done at various intervals after total replacement of the urinary bladder with jejunum. The absorption was found to decrease greatly in one month and to be negligible one year postoperatively.

[Estimation of serum beta 2 microglobulin in carcinoma of the bladder (author's transl)].

beta 2 Microglobulin is a low molecular weight protein synthesised by lymphocytes and neoplastic cells. The authors measured levels of this substance in the plasma of 40 patients with carcinoma of the bladder and normal renal function. 26 subjects were used as controls. Plasma levels were markedly higher in the bladder carcinoma sufferers than in the normal controls. Levels were also higher in invasive carcinomas than in superficial lesions. beta 2 microglobulin may therefore not be considered to be a specific indicator of carcinoma of the bladder but rather as a factor offering the possibility of measurement of the secretory activity of tumour cells.
