[Effects of strength- and endurance oriented training during regional chemotherapy in metastatic rectal carcinoma. Case report as a contribution to surgical oncology].

OBJECTIVE: How does adjuvant training of strength and endurance influence the muscular, cardial, respiratory, immunological systems, and the quality of life during intrahepatical chemotherapy in a veteran athlete with liver metastasis after resection of a carcinoma of the rectum. PATIENT AND METHOD: A 58-year-old athlete with metastatic carcinoma of the rectum (pT3, N0, M-liver, G2) underwent regional chemotherapy (folinic acid, 5-fluorouracil) via an intrahepatic port system. Six cycles with 5 days each were applied. During the intervals between cycles (14 days), beginning in the 6th postoperative week, a strength and endurance training (duration 13 weeks) was carried out twice weekly with an intensity of 40-60% of the maximum postoperative individual power and endurance (evaluated with weight and treadmill exercise tests and measurement of lactate). Before and after chemotherapy, an echocardiogram, resting and exercise electrocardiogram, lung function, natural killer cells and the gastrointestinal quality of life index (GLQI) was evaluated. After chemotherapy was finished, the effects of training were assumed. RESULTS: Strength increased between 0% and 144%. Endurance improvement was measured on the last comparable submaximal intensity step, expressed by reduction of heart rate and lactate concentration by 10 and 21.5%, respectively, improvement of lung function regarding FEV 1 and FCV by 12.9 and 11.3%, respectively, and VC IN 11.4%. The relative count of natural killer cells increased by 27.2%, and the GLQI improved from 109 points (pathologic) to 129 points (normal). CONCLUSION: There were positive effects on muscular and cardial mechanisms of adaptation and on illness-related quality of life. Postoperative sports activity during regional chemotherapy is possible.

Acute ulnar nerve compression syndrome in a powerlifter with triceps tendon rupture--a case report.

We report on the case of a bodybuilder and powerlifter who suffered from triceps tendon rupture complicated by acute ulnar nerve compression syndrome. The diagnosis was made clinically, radiologically, and sonographically. Ultrasound was helpful to demonstrate a large hematoma at the site of the injury. Early surgical intervention confirmed the presence of the hematoma compressing the ulnar nerve and led to a complete restoration of ulnar nerve and triceps muscle function. Few reports on distal triceps rupture have been published but its complication by acute ulnar nerve compression has not been reported on yet despite the close anatomical relationship of both structures.

[Iatrogenious arteriovenous fistula: rare complication after knee arthroscopy].

Vascular damage due to arthroscopy is a rare complication. Postoperative occurrence of an arteriovenous fistula after using an electric hook for arthroscopic partial medial meniscectomy is described. Due to uncharacteristic symptoms of the arteriovenous fistula as a vascular complication after arthroscopy, the correct diagnosis may take a long time. In order to avoid damage to the popliteal vessels, the removal of the posterior horn of the medial meniscus should be performed under best visibility conditions with mild flexion and only mild external rotation of the calf. Considering the severe consequences, popliteal vascular damage is worth mentioning during preoperative information, even if it is an extremely rare complication.

A ganglion of the superior tibiofibular joint as a mucoid-cystic degeneration of unusual localization. A case report and review of the literature.

This paper presents the case of a 51-year-old man with a ganglion originating from the superior tibiofibular joint. In the context of the present literature, different theories of the pathogenesis, clinical symptoms, various kinds of imaging methods in regard of the differential diagnosis and the broad spectrum of operative and non-operative treatment are discussed.

The NMR-MOUSE: quality control of elastomers.

New applications of the NMR-MOUSE (mobile universal surface explorer) to non-destructive quality control of elastomers are reported. One example concern the thermal aging of fast clutches which was probed by measurements of 1H transverse relaxation time. Novel methodological developments show that 1H double-quantum filtered NMR signals can be generated in the inhomogeneous fields of the NMR-MOUSE for characterization of residual dipolar couplings. This technique was applied to characterize reference natural rubber samples with different crosslink density and carbon black and silica fillers.

[Operative treatment of symptomatic nerve root cysts].

AIM OF THE STUDY: Nerve root cysts are often asymptomatic and show a predilective manifestation for the lumbo-sacral spine. Depending on their size and the anatomic relationship to the nerve roots they may cause symptoms such as pain or even neurological deficits. From the therapeutical point of view, different moldalities of intervention are controversially discussed because the clinical importance of this entity is still unclear. There are curative as well as symptomatic therapeutic options. The curative therapy includes surgical excision of the cyst and duraplasty whereas the symptomatic therapy is limited to drainage of the cyst. The purpose of this study was to evaluate the role of surgical excision of the cyst with duraplasty using microsurgical techniques. METHOD: This study included 13 patients who suffered from pain and neurological deficits with myelographically and MRI proven sacral nerve root cysts. All patients underwent surgery for cyst excision plus duraplasty. RESULTS: Clinical symptoms such as pain and sensory deficits significantly improved in all patients postoperatively. 85 % of the patients showed a full recovery from the previously existing radicular pain and motoric as well as sensory deficits. All patients reported a significant improvement of their preoperative loss in muscle strength. CONCLUSION: Surgical excision combined with duraplasty using microsurgical techniques has proven to be the method of choice in the treatment of symptomatic sacral nerve root cysts.

School sports accidents: analysis of causes, modes, and frequencies.

About 5% of all school children are seriously injured during physical education every year. Because of its influence on children's attitude toward sports and the economic aspects, an evaluation of causes and medical consequences is necessary. In this study, 213 school sports accidents were investigated. Besides diagnosis, the localization of injuries, as well as the duration of the sick leave were documented. Average age of injured students was 13 years. Most of the injured students blamed themselves for the accident. The most common injuries were sprains, contusions, and fractures. Main reasons for the accidents were faults in basic motion training. Playing soccer and basketball were the most frequent reasons for injuries. The upper extremity was more frequently involved than the lower extremity. Sports physicians and teachers should work out a program outlining the individual needs and capabilities of the injured students to reintegrate them into physical education.
