Severe hypoxemia without evidence of tissue hypoxia in adult respiratory distress syndrome.

A patient with severe arterial hypoxemia in adult respiratory distress syndrome (ARDS) secondary to a fractured femur is reported. A marked discrepancy between the clinical condition and laboratory findings was observed. At one point, PaO2 was below 4 kPa (30 mm Hg) on F1O2 0.21 without signs of tissue hypoxia. This patient illustrates beneficial effects of a leftward shift of the oxyhemoglobin dissociation curve.

Seminal plasma fructose, zinc, magnesium and acid phosphatase in cases of male infertility.

Determinations of fructose, zinc, magnesium and acid phosphatase were performed in seminal plasma from male partners in 225 barren couples. No specific changes in the concentrations of these substances could be observed in men showing either visible or palpatory abnormalities of the external genitals, the prostate or of the seminal vesicles, extreme changes in the number, morphology or motility of the sperms, increased sperm antibody titre or abnormal serum levels of testosterone or gonadotrophins. However, when the prostatic fluid contained numerous leukocytes, the concentrations of zinc, magnesium and acid phosphatase were significantly lower than in men having a cytologically normal prostatic fluid. Determinations of these substances may thus be of aid in the diagnosis of chronic prostatovesiculitis.

A comparison of four different blood gas analysers.

Four automatic blood gas analysers from four different manufactures were evaluated and compared. The measurements were performed on blood representing respiratory acidosis and hypoxemia, normal conditions, and respiratory alkalosis and hyperoxemia. On each level nine complete runs were carried out, each run consisting of six replicates of each parameter (pH, Pco2 and Po2) on each instrument (six rounds). Only the directly measured parameters (pH, Pco2, Po2) were compared. The main conclusion is that the four instruments can be used alternatively, and that the differences between the values measured by the four instruments are of little clinical significance.
