[Mesh-plug operation for treating inguinal hernia. Randomized studies].

In the period from October 1996 to April 1997 a prospective randomized study of one hundred patients with inguinal hernia was performed. 50 patients underwent the Bassini procedure (group I) and others tension-free mesh-plug repair (Lichtenstein in Rutkow's modification) (group II). The type of hernia was classified according to Gilbert's classification in Rutkow's modification. We used t- Student test and Mann-Whitney's test to evaluate significant differences. There was no significant difference in patients' age, types of hernia or the type of anesthesia between to groups. The time of operation was significantly shorter in group II (median 51.8 min) than in group I (median 63 min). During the post operative period more patients from group I required narcotic analgetics (64% versus 22%). The rate of complications was similar (8% and 6% respectively). Hospital stay was significantly shorter in group II-median 4 days versus 6 days in group I. Rehabilitation to normal activity and return to work was also shorter in group II-median 2 weeks versus 3 weeks (not statistically significant). During the follow up period 1-17 months no recurrences were observed.

Effect of saturated air and nitrox diving on selected parameters of haemostasis.

The objective of the study was to evaluate decompression stress after air and nitrox saturated divings on the basis of the parameters of haemostasis. Before and after each diving the following examinations were performed: blood platelet count, aggregation, fibrinogen level and coagulation factors VII, X and XII. After the air saturated dives a slight reduction of factor X and XII and of fibrinogen was observed. Evaluation of haemostatis may be one of the basic elements in the assessment of decompression sickness risk.

The effect of synthetic diether phospholipid on lipid absorption in the rat.

The effect of synthetic diether phosphatidylcholine on lipid absorption in the rat was studied to determine whether this material inhibited absorption of either cholesterol or fatty acid. A lipid emulsion was prepared with sodium taurocholate, phospholipid, 3H-cholesterol, 14C-triolein, and oleic acid. Emulsion given to controls contained pig liver phosphatidylcholine as the source of phospholipid. For experimentals it was supplied as diether phosphatidylcholine. Emulsions were given by gavage into the stomach and recoveries of radioactive lipids in various regions of the gastrointestinal tract were determined with radioactive techniques 4 and 24 hours after receipt of the lipid challenge. Significantly greater amounts of 3H-cholesterol were recovered unabsorbed from the gastrointestinal tract of experimental rats at both 4 and 24 hours after administration of labeled lipids. Somewhat more of the 14C-oleic acid label derived from labeled triolein was also recovered unabsorbed in experimentals, although a significant difference was not observed between experimentals and controls. It is concluded that nondigestible (diether) phosphatadidylcholine inhibits absorption of cholesterol. This phospholipid, however, had no appreciable affect on fatty acid absorption as definitive malabsorption of this material was not observed under the conditions of this study.
