Surgical treatment of pigmented melanocytic nevi depending upon their size and location.

Cutaneous melanocytic nevi may cause cosmetic defects and represent a risk of malignant transformation. Most problems are posed by nevi of the face or those covering large areas of the body, the so-called giant congenital nevus. From 1986 until 1995, 295 patients with pigmented nevi underwent surgical treatment. The method employed depended upon the size and location of the melanocytic nevus. Results were evaluated according to a three-stage scale with consideration of the cosmetic outcome and satisfaction of the patients. Good results were obtained in 280 patients, satisfactory ones in 11 patients, and unsatisfactory ones (requiring corrective treatment) in 4 patients. Small and medium size nevi (up to 5 cm in diameter) can be removed in a one-stage procedure with suturing of the wound, local plasty or free-tissue skin graft. Blepharal and central facial lesions are best reconstructed with full-tissue skin grafting. Large nevi (over 5 cm in diameter) mandate a staged excision or removal at one-stage with prior use of a tissue expander or presuturing. Giant nevi require staged treatment with the use of an intermediate thickness skin graft or removal of superficial layers of the nevus, always preceded however by a histopathological examination.

[Evaluation of internal nose deformation in patients with unilateral cleft lip and palate].

The methods of assessment of nose deformation in patients with unilateral cleft lip and palate have been presented. The study covered 45 patients with clefts, of 14-20 years of age who had undergone the infant surgery, and 45 patients at the same age serving as controls. The condition of nasal septum has been clinically examined in both groups and the nasal resistance has been measured by means of front and rear rhinomanometry. In the cleft group 80% of patients presented their cartilaginous septum deviated to the non-cleft side, and 85% had their bony septum deviated to the cleft side. In the cleft group, the front rhinomanometry revealed significantly higher resistance on the cleft side than on the non-cleft side and in controls. The rear rhinomanometry showed no significant differences in both groups, except the patients with clefts, who had been submitted to surgery with pharyngeal flap, in whom higher values of resistance had been found.

Aplasia cutis congenita of the scalp. Report of 16 cases.

BACKGROUND: Aplasia cutis congenita (CCA) is a rare developmental anomaly. Its etiology and pathogenesis remain unclear. There are several hypotheses as to the influence of genetic and external factors on the fetus. Terminology of this anomaly is still a subject of discussion. OBJECTIVES: This paper describes 16 patients with CCA of the scalp, ranging in age from 3 to 15 years, observed over the period of 15 years. Among them, girls prevailed 9:7. CCA was more common on the vertex rather then in the temporal region 13:3. In one girl multiple malformations were observed. METHODS: Location, shape, and size of the lesion determined as CCA were analyzed. Detailed history was taken and cytogenetic examination was performed to reveal etiopathogenetic factors. Treatment consisted of excision of the lesion and reconstruction of the scalp with neighboring skin flaps. The excised lesion underwent histologic examination. RESULTS: No etiopathogenetic factors were found. Histopathologic examinations indicate that the observed congenital scalp anomalies resemble a scar. Surgical treatment produced good cosmetic results. CONCLUSIONS: Literature on CCA will facilitate physicians to make the right diagnosis. Further accumulation of detailed descriptions on CCA patients may contribute to recognition of etiopathogenesis of this anomaly.

Body structure of female-to-male transsexuals.

31 female-to-male transsexuals treated in the Department of Plastic Surgery, Medical University of Lódź, were examined. Anthropometric measurements were carried out according to Martin's technique. 23 measured characteristics of the examined transsexuals were studied; they were compared with identical characteristics in males and females of the control group. The results indicate that the somatic characteristics in transsexual women are between the values typical for man and women.

[The incidence of cleft lip and (or) palate in children of Lodz in the years 1982-1991].

Over 92 thousand newborns (1982-1991) have been analyzed to reveal 186 infants with cleft lip and(or) palate which produced the mean frequency of clefts as 2:1000. Additionally, the forms and sides of clefts, and sex of children with clefts have been defined. The comparison of results from 1951-1965 research showed a slight increase of frequency of clefts which at that time (1951-1965) amounted at 1.7:1000. Our results correspond to the actual data of the world's literature.

Anthropometrical measurements of the face in infants with bilateral clefts.

The anthropometric measurements of face were taken in 21 infants of 6-9 months (15 boys, 6 girls) with bilateral cleft lip, alveolus and palate, prior to surgery. The control group consisted of 30 normal infants, without facial defects. In each child 9 measurements were performed. The comparative analysis revealed an underdevelopment of maxilla and mandible and an increase in nasal width in children with bilateral cleft lip and palate.

Epidemiology of cleft lip and palate in Lodz, Poland, in the years 1981-1995.

Over 132,783 newborns from 1981-1995 have been analysed revealing 267 infants with cleft lip and/or palate resulting in a mean frequency of clefts at 2:1000. Additionally, the forms and sides of clefts, and sex of the children with clefts have been defined. The comparison of the studies in Lodz from 1951-65 and 1981-95 indicates a slight increase in the occurrence of clefts.

Construction of neophallus in male pseudohermaphroditism.

Eight patients with male pseudohermaphroditism were treated between the years 1988-1994. They showed various physical-sexual anomalies. In 4 patients the existing penis was lengthened, the urethra was created and the split scrotum was fused. In the 4 remaining patients a new penis was created from lateral groin or abdominal tubed or skin-gracilis flaps, and made rigid by means of a silicon implant. The scrotum was reconstructed. The results were satisfying. On the basis of our own experience it may be said that the reconstruction of genitals must be planned individually, depending on the abnormalities in the physical-sexual development and the psycho-sexual orientation of the patient.

The indications and the plan of plastic operations in children with Down's syndrome.

During the past ten years (1984-1993), 52 children with Down's syndrome aged 3-16 years were treated surgically at the Department of Plastic Surgery at the Medical University of Lódz. The authors present the indications and the treatment plan in selected patients, the optimal timing of surgical treatment, the choice of surgical methods for the correction of facial deformity and for the limpness of tongue depending on its form and intensity. Elimination of mongoloid features of the face (tongue, lower lip, eyelids, nose) has a positive influence on rehabilitation of children with Down's syndrome. This procedure was used in Poland for the first time.

[Evaluation of surgical techniques used in treating microtia for 35 years].

The author presents an evaluation of the results of surgical treatment in 346 patients with microtia, treated in the previous 35 years, by using different materials to reconstruct the auricle. The reasons for giving-up the application of xenogenic and allogenic cartilage as well as polyester-polypropylene implants to reconstruct the auricular skeleton are explained. A 17-year experience of using autogenic costal-cartilage to reconstruct the auricular skeleton helps to state that early and late complications have been reduced and satisfactory esthetic results have been obtained. To meet their need, the patients were treated for the associated malformations by means of plastic and reconstructive procedures and were referred to other specialists--orthodontic, otologic and prosthetic.

[Selection of patient age for evaluation of treatment results in cleft lip and palate].

The results of the complex treatment of 125 patients with unilateral and bilateral lip and palate cleft, aged 3-5, 10-12 and 14-18 years, were evaluated by analysis of medical documentation. The study showed that the results of lip, vestibule and palate reconstruction and speech can be evaluated already at 12 years of age. However, final esthetic and functional assessment of the nose and maxillo-occlusional conditions is indicated after 14 years of age, best between 18-20 years.

[Assessment of long-term results in cleft lip and palate].

The study concerned 230 patients (175 UCLP and 55 BCLP) aged 14-20, who had been treated in the years 1972-82 in accordance with the Centre for Congenital Defects programme, at the Department of Plastic Surgery, Medical University of Lódź. The follow-up comprised: aesthetic and functional assessment of the lip, upper vestibule and nose as well as evaluation of occlusion and speech abilities (intelligibility, articulation, nasality). The assessment of lip and vestibule gave good and acceptable results in 97% of UCLP patients and in 89% of BCLP. The external nose appeared aesthetically and functionally good in 67% of UCLP and in 94% of BCLP. The defects of occlusion, mainly various forms of cross-bite, were found in 48% of UCLP, who had undergone orthodontic treatment. The maxillo-occlusal defects with no prior orthodontics were found in 63% of UCLP. Alike the defects of occlusion in untreated BCLP were most frequent. The intelligibility and articulation of speech were significantly better in patients who had undertaken the postoperational speech therapy (95%:75%). The assessment in general presented better results in UCLP patients, who had been submitted not only to surgery but also took up orthodontic treatment and speech therapy.

[Evaluation of surgical methods in treating blepharoptosis].

The aim of the study was to evaluate the results of two surgical procedures for the treatment of congenital blepharoptosis. The first type was based on more or less extensive excision of the levator muscle from anterior (Everbusch) or posterior (Blascovics) approach. The second procedure was double-level resection with preservation of levator muscle or with its shortening, by plication of its peripheral part (Mustarde). The study comprises 38 cases with congenital blepharoptosis. Better results were obtained in the cases where the levator muscle has been preserved.
