[A case of transitional cell carcinoma of the bladder with heterotopic bone formation].

A 85-year-old woman consulted our clinic due to asymptomatic macrohematuria on January 6, 1987. In the cystoscopic examination, solitary and pedunculated bladder tumor was revealed. This tumor was located on the retrotrigonal region near the right ureteral orifice. On January 20, transurethral resection of bladder tumor was planned but the base of the tumor was stony hard, so complete resection was not performed. On February 10, transurethral resection of bladder was performed repeatedly. At that time, the cystoscopic finding showed that the tumor size had increased and transurethral bladder ultrasonography showed invasion into the deep muscular layer. On February 17, total cystectomy and ileal conduit was done. Histological examination revealed poorly differentiated TCC with osteoid, bone and cartilage. This patient was discharged in good condition on March 27. Heterotopic bone formation in the bladder tumor is rare. Ten cases were collected from the literature. We report a case of heterotopic bone formation and discuss the mechanism of the etiology.

[A case of Paget's disease of the vulva with bladder metastasis].

A very rare case of bladder metastasis of the Paget's disease of the vulva is reported. A 83-year-old woman first experienced an eczematoid eruption on the vulva 12 years earlier. It was diagnosed as Paget's disease of the vulva and treated with radiation 8 years earlier. In March, 1985 a biopsy of the vulva revealed a recurrence of Paget's disease. In August, 1986 she had a sudden attack of left flank pain. The excretory urogram showed bilateral hydronephrosis and transurethral echogram of the bladder demonstrated large nodular tumors of the bladder. Transurethral biopsies of the bladder revealed Paget's disease. Total cystourethrectomy with bilateral ureterocutaneostomy and wide skin excision of the vulva with skin transplantation were performed. The pathological specimen showed several yellowish-white nodular tumors of the bladder, which obstructed both ureteral orifices. Microscopically typical Paget's cells were seen, some of which were stained with PAS before and after amylase and with alcian blue. Postoperatively she developed a severe complication of necrosis of the small intestine due to thrombosis of superior mesenteric artery and died of panperitonitis on the 55th postoperative day. Related reports were also reviewed.

[Clinical statistics on outpatients, inpatients and operations in 1984].

Statistical studies on 1,893 outpatients, 538 inpatients and 554 operative procedures at our department in 1984 revealed the following. The most frequent diseases among the outpatients were urogenital infections followed by anomalies, tumors and stones. The major diseases among the inpatients were hypospadias, vesicoureteral reflux, benign prostatic hypertrophy, bladder tumor and congenital urethral stenosis. A total of 554 operations were performed on 534 patients, and five major operations were hypospadias repair (68), ureterocystoneostomy (57), optic internal urethrotomy (50), TUR-P (42) and TUR-Bt (33).

Statistical studies on 1,990 outpatients, 574 inpatients and 606 operative procedures at our department in 1983 revealed the following. The most frequent diseases among the outpatients were urogenital infections followed by anomalies, tumors and stones. The major diseases among the inpatients were hypospadias, vesicoureteral reflux, congenital urethral stenosis, benign prostatic hypertrophy and bladder tumor. A total of 606 operations were performed on 559 patients, and the five major operations were hypospadias repair (80), TUR-P (54), optic internal urethrotomy (51), ureterocystoneostomy (40) and orchidopexy (34).

[A case of urinary schistosomiasis in a Japanese male].

Urinary schistosomiasis is an endemic disease mainly distributed in most of Africa and Southwest Asia, and caused by infestation with Schistosoma haematobium. The case presented here was a 33-year-old Japanese male who had suffered from schistosomal infestation in the endemic area, and diagnosed as urinary schistosomiasis by detection of Schistosoma haematobium ova in urine and specimens obtained by mucosal biopsy of the bladder. Radiographic and endoscopic examinations revealed neither obstructive uropathy nor urothelial malignancy. The patient underwent treatment with antimonyl sodium tartrate (Stibnal), and the disease is under good control.

[Vesico-ureteral reflux (VUR) and renal size in children (author's transl)].

UNLABELLED: We analyzed the radiographic renal size of 180 children with vesico-ureteral reflux as compared with that of 159 normal children. We measured on urogram the vertebral length, renal length and thickness of the renal cortex. Further, the ratio of the renal length to the vertebral length and the cortical thickness to the renal length were computed. The latter was named the cortical ratio. IN CONCLUSION: 1. The cortical ratio is the most useful parameter in analysis of the renal size. 2. Cortical damage due to VUR is noted already in newborns. This result suggests that vesico-ureteral reflux must be detected and treated in early childhood.

[Clinical experience with sustained release cephalexin (S-6436) in urology (author's transl)].

Clinical studies of S-6436 were conducted in 30 urinary tract infection patients in urology with the following conclusion. The patients received the drug twice a day after breakfast and dinner. 1. Of the 30 patiens, excellent efficacy was observed in 9 patients, good efficacy in 14, fair in 5 and poor in 2. The effectiveness (excellent or good) was 76.7%. 2. Twenty-seven (27) strains were isolated from the 30 patients and 23 strains (85.2%) of the 27 disappeared with the administration of the drug. 3. Side effects were found in 3 patients (10%) of the 30. The incidence was almost similar to that of regular cephalexin. No remarkable changes in laboratory data were observed. 4. S-6436 which requires the administration of only twice a day is a preparation which is more convenient for patients than regular cephalexin which requires the administration of four times a day.

Cytotoxic and DNA damage-inducing activities of low molecular weight phenols from rhubarb.

Six new phenol (anthraquinone or stilbene) glycosides with an acyl group at 6-position of the glucopyranose moiety were isolated from rhubarb (the roots of Rheum palmatum) cultivated in Japan, together with 22 known compounds. Most of these compounds were evaluated for cytotoxic activity against tumor and normal cells and for induction of DNA damage by spore rec-assay. Among them, emodin and aloe-emodin showed higher cytotoxic activities against human oral squamous cell carcinoma (HSC-2) and salivary gland tumor (HSG) cell lines than against normal human gingival fibroblasts (HGF). Chrysophanol 8-O-beta-(6'-acetyl)glucopyranoside, 4-(4'-hydroxyphenyl)-2-butanone 4'-O-beta-D-(2"-O-galloyl-6"-O-cinnamoyl) glucopyranoside, and 6"-O-(4'''-hydroxybenzoyl) resveratroloside exhibited relatively higher cytotoxic activities against all these cells. The other glycosides of anthraquinone or stilbene showed weaker cytotoxic activity against these tumor cell lines, but may be considered as cancer chemopreventive agents. Spore rec-assay with a recombination deficient mutant of Bacillus subtilis M45 demonstrated the DNA damage-inducing activity of emodin and aloe-emodin 15-O-beta-D-glucopyranoside among, rhubarb phenols.
