[Cervical discopathy, cervical migraine and vertebrobasilar arterial insufficiency: Clinical correlations].

The pathogenesis of cervical migraine may suggest that a connection exists between it and cervical discopathy and also vertebrobasilar arterial insufficiency. This correlation was studied in a group of 103 patients subjected to clinical observations. In 83 of them cervical discopathy was found, in 69 cervical migraine and in 40 vertebrobasilar insufficiency were present. Among those with discopathy migraine was present in 63.6% of cases, and vertebrobasilar insufficiency in 31%. In patients with cervical migraine degenerative changes of the type of cervical discopathy were present in 90% of cases, although in only 56.5% of cases clinical signs were present connected with these changes. Vertebrobasilar insufficiency was present in 45% of patients with cervical migraine, that is more frequently than in discopathy. In the group with vertebrobasilar insufficiency radiological changes in the cervical spine were present in 77.5% of cases and the same proportion of patients in this group had cervical migraine. These numbers confirm the connection between cervical migraine and radiological changes and clinical manifestations of discopathy, and they indicate also that cervical migraine may be a successive stage of vascular changes leading to circulatory failure in the area supplied by vertebral arteries. This failure may have a different course and pathological mechanism. In 12.5% of patients in this group no cervical migraine or radiological changes were demonstrated. The term "cervical migraine" seems inappropriate since this type of headache has another substrate and mechanism than true migraine.

[Acupuncture in headache and radicular syndromes].

The reported own experiences are related to acupuncture with injections of 10% NaCl solution (irritating the nerve endings) into selected points. Since 1973 this method was applied in 171 patients with radicular painful syndromes and 120 patients with headaches. In a group of 107 cases of sciatic pain a persistent analgesic effect was achieved in 30 cases (28%) and immediate disappearance of pain occurred in 21 cases (20%). In a group of 64 patients with shoulder pains a persistent effect was obtained in 36 cases (56%) and immediate but transient in 7 (10,9%). Among 43 patients with migraine a persistent good effect was observed in 24 cases (55,8%) and an immediate relief which was transient in 9 cases (20,9%). In 77 cases of vasomotor headaches a persistent effect was noticed in 37 patients (48%) and an immediate transient one in 23 (29,8%). It is not absolutely necessary to insert the needles into the traditional points accepted in classic acupuncture. During one procedure from 3 to 10 needles were inserted leaving them for about 15 minutes. From 3 to 15 procedures were given, mostly on consecutive days until a significant effect was obtained.

[Plasma beta-endorphins under the effect of pain stimulus. Preliminary report].

Using a diagnostic kit N.E.N. (USA) the authors investigated beta-endorphin-like immunoreactivity in the plasma of young, healthy subjects. It was found that the level of beta-endorphin determined in this way in 10 subjects ranged from 15 to 120 pgml-1 and showed relatively constant values in two determinations carried out at an interval of one hour. Psychological tests showed that subjects with low beta-endorphin level had a low neuroticism index and extra-version, while in the subjects with high beta-endorphin level the neuroticism index was high and introversion features were more prevalent. After application of a painful stimulus (ischaemic test) the level of beta-endorphin changed in most subjects increasing or decreasing. The comparison of the differences in the levels of beta-endorphin immediately before and after the test for pain tolerance suggests that the higher was this difference the lower was pain tolerance and conversely.

[Analgesic effect of imipramine in the treatment of ischium pain and circadian rhythm of cortisol release into the serum].

In the paper the results are presented of treatment of ischialgia with Imipramine, taking into account the circadian rhythm of cortisol release into the serum. The obtained results encourage to the use of tricyclic antidepressants in the treatment refractory cases of ischialgia. No significant differences were observed in the circadian rhythm of cortisol release into the serum before and after the treatment with Imipramine.

[Dexamethasone test in the assessment of depression grade in patients with low back pain].

The results of the dexamethasone test were compared with the depression depth and pain intensity in patients with low back pain caused by degenerative changes and/or disc prolapse. In the light of these studies no correlation was demonstrated between the positive results of the dexamethasone test and depression symptoms and the intensity and duration of pain.

[Electrophysiological assessment of pain sensitivity in neurotic patients].

The results are presented of a study of pain sensitivity in neurotic patients comparing them with the results obtained in 40 controls. For the study the electrophysiological method of nociceptive flexion reflex was used in which reflex threshold is closely correlated with pain threshold. It was found that pronounced neurotic symptoms caused an evident lowering of the pain threshold, and its clinical counterpart was increased pain sensitivity.

[Assessment of dexamethasone effectiveness in the treatment of ischalgia].

The authors studied 43 patients admitted to hospital with acute episodes of ischalgia or recurrences caused by intervertebral disc disease. The patients were randomly selected into two groups treated with dexamethasone in high but rapidly decreasing doses, or tramadol. A comparison of the results in both groups showed that dexamethasone was effective and safe when given by this method to patients with ischalgia.

[Recurrent painful ophthalmoplegia (Tolosa-Hunt syndrome)].

A case is presented of recurrent painful ophthalmoplegia of atypical course, with pain in the orbit, eyelid oedema, and paralysis of external eye muscles. Disease symptoms occurred intermittently in both eyeballs. The performed MRI examination showed the presence of inflammatory focus in the posterior ethmoid.
