B cell surface marker analysis of synovial fluid cells in a patient with monoarthritis and chronic lymphocytic leukemia.

We describe a women with clinically quiescent chronic lymphocytic leukemia who developed monoarthritis consistent with her known diagnosis of osteoarthritis. Because of the concurrent chronic leukemia, we were concerned with the possibility of leukemic arthritis. Immunofluorescence of synovial fluid cells demonstrated an increase in B cells that failed to demonstrate monoclonality as determined by light chain class expression. However, biopsy of synovial tissue revealed leukemic infiltration. Local radiation therapy resolved the monoarthritis.

The examination of antigens in highly purified immune complexes.

A combination of gel filtration and affinity chromatography was used for preparation of immune complexes (IC) from sera of individuals with systemic lupus erythematosus (SLE) and others with no apparent disease. Overlaying of gels with patient or normal sera, counterstaining with 125I-protein A and autoradiography showed besides immunoglobulins, the presence of 6 unidentified antigens with molecular weights below 42,000 dalton. They were not exclusively associated with SLE IC.

Steroid therapy: still a need for answers.

Various methods of steroid administration are reviewed, with emphasis on single dose alternate-day oral administration as compared with daily administration. Alternate-day therapy produces less adrenal and pituitary gland suppression and a reduced frequency of cushingoid features. Furthermore, the ability to resist infection is evidently better preserved. However, an overall therapeutic advantage cannot be claimed as there are few clinical trials with which to make a direct comparison. The numerous studies dealing with this form of therapy lack the rigorous control needed to answer the question of whether alternate-day steroid therapy should be substituted for daily therapy in certain diseases.
