Malignant triton tumor in the thoracic spine.

We present a 15-year-old patient diagnosed with peripheral neurofibromatosis (NF-1), who was admitted with paraparesis caused by a large intrathoracic tumor with an intracanalicular component that affected the spinal cord. After surgery his condition improved, but a year later he suffered a relapse and died. Histologically the tumor was diagnosed as malignant with neurogenic and myogenic differentiation ("malignant triton tumor"). Malignant triton tumors (MTT) are infrequent; those found in the head and neck and the upper or lower extremities have a better prognosis than those in the retroperitoneum, buttock, or trunk. It is not clear whether this variation is due to a difference in tumor grade, stage, or resectability, or whether it is a consequence of therapy.

[Iron deficiency and pica].

PURPOSE: To study the relationship between pica and iron-lack anaemia in a series of iron-deficiency patients in order to establish the pathogenesis of such relationship. PATIENTS AND METHODS: Four-hundred and thirty-three patients were analysed. Pica was studied by introducing certain diet queries into the clinical history. All patients received oral iron and were periodically controlled with the usual clinico-haematological procedures. RESULTS: Pica was present in 23 patients (5.3%). Eight nourishing (namely, coffee grains, almonds, chocolate, ice, lettuce, carrots, sunflower seeds and bread) and 2 non-nourishing (clay and paper) substances were involved. A second episode of pica appeared in 9 cases upon relapsing of iron deficiency. Both anaemia and pica were cured by etiologic and substitutive therapy in all instances. No clear correlation was found with either socio-economic status or pathogenetic causes of iron deficiency and pica, and no haematological differences were seen between patients with pica and those without this alteration. CONCLUSIONS: (1) The pathogenesis of pica is unclear, although it appears unrelated to the degree of iron deficiency. (2) According to the findings in this series, pica seems a consequence of iron deficiency rather than its cause. (3) Adequate therapy can cure both conditions, although pica may reappear upon relapse of iron deficiency.

Development of physical response after athletics training in adolescents with Down's syndrome.

Starting in January 1994, a group of 20 adolescents with Down's syndrome began a competition-oriented athletics training program at a public sports stadium twice a week as part of their education towards integration. They were given complete medical examinations beforehand and followed medically and professionally throughout the program. The results showed a significant improvement in the test scores measuring strength, speed and endurance and a tendency towards an athletic morphotype.

[Spinal epidural abscesses. Conservative treatment. Three cases].

Spinal epidural abscesses (SEA) are an infrequent disease which should be suspected in patients with spinal and/or radicular pain, neurologic deficits and fever. We present 3 patients with SEA (two lumbar and one dorsal) who recovered following exclusively antibiotic treatment. The complete disappearance of the abscess was proven by magnetic resonance. Patients with SEA and discrete, stable deficits and with known germs, are susceptible to medical and not surgical treatment.

[Kikuchi-Fujimoto necrotizing lymphadenitis associated with brucellosis].

A 22 year-old woman was referred to the hospital for treatment after being diagnosed of Hodgkin's disease, established by lymph node biopsy. She complained of fever and sweats, and no lymph node enlargement or visceromegaly could be appreciated. Kikuchi's necrotising lymphadenitis was diagnosed after reviewing the lymph node specimen. The microbiological and serologic tests performed showed the existence of brucellosis. The clinical symptoms easily disappeared after treatment with streptomycin and doxycycline and she has been well, with no other symptoms or lymph node enlargement, after one year of follow-up. Kikuchi's disease is usually associated with different infections, although it had never been reported in association with brucellosis. This picture can easily be mistaken as malignant lymphoma or Hodgkin's disease.

[Perineural dissemination in labial carcinoma].

We present four cases of lip carcinomas which have been extended to the mandible in between of one to four years through the mental nerve with peripheric neuropathy which was multiple in one of the cases.

The quality and extension of nerve fibre regeneration in the centrocentral anastomosis of the peripheral nerve.

Centro central anastomosis (CCA) is the connection created by means of a nerve autograft placed between the fascicles of the proximal stump of a sectioned nerve. We have used this technique in the laboratory and in the human to avoid the formation of a terminal neuroma after amputation of a limb. The aim of the present study is to know what is the quality and how long is the progression of the regenerating nerve fibers coming from a fascicle and growing inside the nerve graft and into the other central fascicle. 56 male, adult, Sprague-Dawley rats were operated on. The 4 rats of Group I were used for control: the right sciatic nerve was exposed and dissected in its four branches. One piece of 10 mm was taken of each of the sural and the peroneal branches for hystological and histomorphometric studies. In the 16 rats of Group II, after the section of the sural and peroneal branches a terminoterminal suture between their proximal stumps was made. In the 16 rats of Group III a CCA with an interposed nerve graft of 7 mm was achieved. 5.5 and 6 rats of Groups II and III were sacrificed at 10, 40 and 365 days after the operation and the entire graft was taken for histological analysis. In the 20 rats of Group IV, 60 days after CCA, the graft was sectioned at its center to produce wallerian degeneration of nerve fibers. 10 days after this second operation the animals were sacrificed and the nerve specimens were taken for histological study.(ABSTRACT TRUNCATED AT 250 WORDS)
