A series of patients was identified who demonstrated immediate positive skin tests to intradermal Trichophyton extract. These skin responses did not correlate with other fungal skin tests and were present both in atopic and nonatopic patients. The individuals demonstrating positive immediate skin tests included patients with urticaria, angioedema, asthma, and/or rhinitis, as well as five of 34 normal control subjects. Most skin test positive individuals had a history of local fungal infection and clinical signs suggestive of fungal infection. By use of Trichophyton tonsurans extract linked to Sepharose as the immunosorbent, it was possible to measure IgE antibodies in 26/30 sera from skin test positive individuals. With strongly positive sera, RAST bound up to 30% of the radiolabeled anti-IgE added. The results confirm that most skin test positive individuals have IgE-mediated hypersensitivity to Trichophyton. These observations support the older view that absorption of dermatophyte allergen through the skin should be considered as a possible cause of allergic disease.
Using xenon-133 radiospirometry and formulae devised on geometric grounds, the authors predicted postoperative lung function in 12 patients who underwent lobectomy, wedge resection, or pneumonectomy for suspected carcinoma. No correlation was found, however, between preoperative, predicted, and postoperative lung function and immediate postoperative morbidity and mortality.
The immotile cilia syndrome appears to be a congenital defect in the ultrastructure of cilia that renders them incapable of movement. Respiratory tract cilia and sperm are predominantly affected. Bronchiectasis, sinusitis and male sterility are the main clinical findings. Situs inversus may be found. To these findings can be added otitis media. The defect appears to be a complete or partial absence of dynein arms which are believed to be essential for generating movement of cilia or sperm tails. Six patients suspected of having immotile cilia were compared to six patients in a control group. In affected patients, no cilia movement in the middle ear or nasopharynx was observed using the operating microscope. Electron microscopy of cilia from the mucosa of the middle ear and nasopharynx appeared to confirm the ultrastructural defect in two of six patients suspected of having the syndrome.

Asthma in older patients usually is a chronic disease requiring prolonged treatment. A study of 40 asthmatic patients at a university center yields an analysis of diagnostic techniques, drug therapy and its complications, and the incidence of associated diseases.
In order to evaluate drug effects on exercise-induced asthma, and to study associated metabolic and pulmonary effects, a method for inducing a consistent response is needed. A method is presented, consisting of 5 min of treadmill exercise sufficiently strenuous to increase a subject's heart rate to 90% of the predicted maximum for age; the airway response is measured frequently for 20 min after exercise. Using this method, 48 asthmatics and 13 nonasthmatics were evaluated. Although none developed severe asthma, a significant airway response occurred in 71% of asthmatics. The severity of exercise-induced asthma depended on intensity and duration of exercise but not on time of day. The method described allows a consistent stress to be applied to a wide age range, and response to this stress was consistent at various ages. Variation of a subject's response following repeated testing was less than that reported with other methods, and could be further reduced by selecting only those subjects with greater than 20% change in one-second forced expiratory volume (FEV1) and by completing studies in less than a month.
